脂联素、血管内皮细胞黏附分子1和同型半胱氨酸水平与冠心病患者冠状动脉粥样硬化斑块类型的相关性研究

摘要

Objective To analyze the correlations of APN,VCAM-1 and Hcy with coronary atherosclerotic plaque types in patients with coronary heart disease.Methods From October 2014 to December 2016,a total of 128 patients with coronary heart disease were selected in the Department of Cardiovascular Medicine,the Affiliated Hospital of Inner Mongolia Medical University,and they were divided into A group(with calcified plaques,n=50),B group(with mixed plaques, n=36) and C group(with soft plaques,n=42) according to the coronary atherosclerotic plaque types;meanwhile a total of 45 healthy people admitted to this hospital for physical examination were selected as control group.General information and laboratory examination results were compared in the four groups,and Spearman rank correlation analysis was used to analyze the correlations of APN,VCAM-1 and Hcy with coronary atherosclerotic plaque types in patients with coronary heart disease.Results No statistically significant differences of gender,age,history of smoking or family history of coronary heart disease was found in the four groups nor was history of hypetension or diabets in A group,B group and C group(P>0.05).No statistically significant differences of TC,TG,LDL or FPG was found in the four groups(P>0.05);HDL and APN in A group,B group and C group were statistically significantly lower than those in control group,while VCAM-1 in A group,B group and C group was statistically significantly higher than that in control group,respectively(P<0.05);HDL and APN in B group and C group were statistically significantly lower than those in A group,while VCAM-1 in B group and C group was statistically significantly higher than that in A group,respectively(P<0.05);HDL and APN in C group were statistically significantly lower than those in B group,while VCAM-1 in C group was statistically significantly higher than that in B gruop(P<0.05);Hcy in B group and C group was statistically significantly higher than that in control group and A group,respectively(P<0.05). Spearman rank correlation analysis results showed that,APN(rs=0.798),VCAM-1(rs=0.862) and Hcy(rs=0.306) was significantly correlated with coronary atherosclerotic plaque types in patients with coronary heart disease,respectively(P<0.05). Conclusion APN,VCAM-1 and Hcy are significantly correlated with coronary atherosclerotic plaque types in patients with coronary heart disease.%目的 分析脂联素(APN)、血管内皮细胞黏附分子1(VCAM-1)和同型半胱氨酸(Hcy)水平与冠心病患者冠状动脉粥样硬化斑块类型的相关性.方法 选取2014年10月—2016年12月内蒙古医科大学附属医院心内科收治的冠心病患者128例,根据冠状动脉粥样硬化斑块类型分为钙化斑块组50例、混合斑块组36例、软斑块组42例;另选取同期在内蒙古医科大学附属医院体检健康者45例作为对照组.比较4组受试者一般资料和实验室检查指标,APN、VCAM-1和Hcy水平与冠心病患者冠状动脉粥样硬化斑块类型的相关性分析采用Spearman秩相关性分析.结果 4组受试者性别、年龄、吸烟史、冠心病家族史及钙化斑块组、混合斑块组、软斑块组患者高血压病史、糖尿病病史比较,差异无统计学意义(P>0.05).4组受试者总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白(LDL)水平及空腹血糖(FPG)比较,差异无统计学意义(P>0.05);钙化斑块组、混合斑块组、软斑块组患者高密度脂蛋白(HDL)、APN水平低于对照组,VCAM-1水平高于对照组(P<0.05);混合斑块组、软斑块组患者HDL、APN水平低于钙化斑块组,VCAM-1水平高于钙化斑块组(P<0.05);软斑块组患者HDL、APN水平低于混合斑块组, VCAM-1水平高于混合斑块组(P<0.05);混合斑块组、软斑块组患者Hcy水平高于对照组、钙化斑块组(P<0.05).Spearman秩相关性分析结果显示,APN、VCAM-1和Hcy水平与冠心病患者冠状动脉粥样硬化斑块类型有关(rs值分别为0.798、0.862、0.306,P<0.05).结论 ANP、VCAM-1和Hcy水平与冠心病患者冠状动脉粥样硬化斑块类型有关.
