吗啡持续静脉泵入法进行快速滴定在重度癌痛患者中的应用与临床疗效

摘要

目的:研究吗啡持续静脉泵入法进行快速滴定应用于重度癌痛患者中的临床疗效.方法:选取2013年1月至2015年12月我院肿瘤内科收治的24例重度癌痛患者作为此次研究的对象,并采用SY-1200号静脉输液泵实施吗啡持续泵入快速滴定,根据吗啡所需剂量以及患者疼痛情况,为患者制定个性化的镇痛方案.比较治疗前后患者的静息疼痛评分、运动疼痛评分、焦虑评分、抑郁评分、生活质量评分,观察患者镇痛过程中的不良反应发生情况.结果:所有患者均在24小时内完成吗啡快速滴定,完成时间为18~23小时,平均为(20.87 ± 2.09)小时.与治疗前相比,治疗后患者的静息疼痛评分、运动疼痛评分、焦虑评分、抑郁评分均明显降低(P<0.05),患者的生活质量评分均明显提高(P<0.05).镇痛过程中,有2例患者出现嗜睡状况,在将吗啡泵入剂量下调后,嗜睡状况消失.结论:采用吗啡持续静脉泵入法对肿瘤内科重度癌痛患者进行快速滴定,可快速、有效地缓解患者的癌因性疼痛,改善患者的心理状态和生活质量.不良反应具有可控性,可通过调整给药剂量,使其得以缓解.%Objective:To study the application and clinical effects of rapid dosage titration of continuous intrave-nous morphine infusion for the treatment of severe cancer pain.Methods:From January 2013 to December 2015,24 patients with severe pain selected within the period were admitted in oncology of our hospital and as the research ob-ject,which were treated with rapid dosage titration of continuous intravenous morphine infusion useing the SY-1200 intravenous infusion pump,according to the required dose of morphine or pain of patients,in order to customize a per-sonalize nalgesic solution.Compare resting pain score,movement pain score,anxiety score,depression score,quality of life score,adverse reactions occurred in analgesic process before and after treatment.Results:All patients were com-pleted within 24 hours for morphine rapid titration,which completion time is 18~23 hours,average(20.87 ± 2.09) hours.Compared with pre -treatment,the resting pain score,movement pain score,anxiety score,depression score were significantly lower(P<0.05),and the patientˊs quality of life score were significantly increased(P<0.05).In the process of analgesia,there were 2 cases appear sleepiness.After lowering the dose of morphine,the narcolepsy condition disappeared.Conclusion:Morphine continuous intravenous pumping method is used to rapid titration of on-cology patients with severe pain,can quickly and effectively alleviate the patientˊs cancer-related pain,improve the patientˊs psychological status and quality of life,and adverse reactions can be controllable.Adverse reactions can be adjusted by drug dose,allowing it to ease.
