我院住院患者1059份退药申请单原因分析

摘要

目的 分析住院患者退药的原因,以提高用药安全性.方法 收集我院2010年8～10月17个临床病区共1059份退药申请单,并与"军卫一号"系统中退药金额进行核查统计,分析汇总退药原因.结果 1059份退药原因从高到低依次为患者出院(71.67%)、调整医嘱(18.04%)、发生药物不良反应(3.02%)、患者转科(2.55%)、医嘱下达错误(2.55%)、患者不依从(1.70%)、患者死亡(0.94%)等.退药品种中,抗生素类(29.40%)、心血管系统用药(20.60%)、调节电解质平衡药(8.80%)位列前3名;退药科室中,妇产科、神经外科、心血管内科排在前三位.结论 临床存在着很多不必要的退药现象,医院应加强临床退药的规范化管理,减少用药安全隐患.%Objective To analyze the reasons of drug rejection of inpatient, and to improve medication safety.Methods A total of 1059 application lists for drug rejection during August 2010 and October 2010 were collected from 17 endemic areas.The data of amount of money about drug rejection in NO.1 Military Medical System were statistically analyzed, and the reasons of drug rejection were analyzed and summarized.Results The reasons for drug rejection were in the order of being discharged from hospital (71.67％), adjustment of medical advice (18.04％), adverse drug reactions (3.02％), transferring to another department ( 2.55％ ), mis-medical advice ( 2.55％ ), non-compliance of patients ( 1.70％ ), and mortality (0.94％).The top 3 categories of rejected drug included antibiotics(29.40％ ), cardiovascular drugs (20.60％) and moderators of electrolyte (8.80％), and Departments of Gynecology and Obstetrics, Neurosurgery, and Cardiology were among the top 3 departments of drug rejection.Conclusion Unnecessary drug rejections exist in hospitals.Standard management of drug rejection should be enhanced to reduce hidden dangers of medication safety.
