    • 摘要: 目的评价早期颅骨修补术应用于脑外伤患者的临床疗效,以及对患者美国国立卫生研究院脑卒中量表(NIHSS)评分、简易智力状态评定量表(MMSE)评分的影响。方法选取2019年11月至2020年11月我院收治的126例脑外伤患者为研究对象,所有患者均行去骨瓣减压术和血肿清除术治疗,患者随机均分为两组,每组为63例。其中一组患者于去骨瓣减压术3~6个月行颅骨修补术(常规组),另一组于术后3个月内行颅骨修补术(早期组)。比较两组患者的临床疗效、NIHSS评分、MMSE评分及并发症发生率。结果早期组患者的恢复良好率显著高于常规组;两组患者的NIHSS评分均显著降低,且早期组NIHSS评分低于常规组;两组患者的MMSE评分均升高,且早期组MMSE评分显著高于常规组;此外,早期组患者的并发症发生率显著低于常规组,差异均有统计学意义(P<0.05)。结论对脑外伤患者行早期颅骨修补手术临床效果显著,改善患者的神经功能和认知功能,有效降低并发症发生率,有利于患者的恢复,值得临床推广应用。

    • 摘要: 为减少噪声污染,改善语音质量,语音增强是解决噪声污染的一种有效方法。针对传统语音增强在语音质量和语音可懂度方面的不足,对传统算法进行改进是非常有必要的。研究了将带噪声音基于自适应的完备经验模态分解(Complete Ensemble Empirical Mode Decomposition with Adaptive Noise,CEEMDAN)处理,将带噪语音转化为多个模态分量进行第二阶处理。当带噪语音为低信噪比时,语音采用快速独立成分分析(Fast Independent Component Analysis,Fast ICA)算法进行噪声分离,再通过对数最小均方误差(log Minimum Mean Square Error Estimation,log-MMSE)算法对分离后的带噪分量进一步处理,最后合成语音。当带噪语音处于高信噪比时,不需要对噪声进行剥离,直接采用最小均方误差(Minimum Mean Square Error Estimation,MMSE)算法对带噪分量进行处理,使输出较为清晰的语音。结果表明,改进算法对语音的增强效果明显。

    • 摘要: MIMO系统中,编码与空间分集的结合可以在无线通信链路中实现可靠高速率。针对V-BLAST结构提出了检测算法的研究与分析。通过MATLAB进行无干扰消除、理想干扰消除及非理想干扰消除不同情况的参数设置,分别对ZF与MMSE算法进行性能对比,由仿真结果可知:在ZF与MMSE算法中,干扰消除均可以提高输出信号的信噪比,而MMSE检测算法性能更优。

    • 摘要: 目的观察七氟烷吸入麻醉对老年腹腔镜患者术后认知功能恢复及体内炎性因子的影响,以分析七氟烷吸入麻醉在老年腹腔镜手术中的应用价值,为该类患者麻醉方法的选择提供依据。方法选取2018年11月至2020年11月收治的老年行腹腔镜手术患者106例,按照麻醉药物不同将其为试验组和对照组,每组53例。对照组注射丙泊酚麻醉,试验组采取七氟烷吸入麻醉,观察2组患者MMSE评分、认知功能障碍发生率、体内炎性因子水平。结果2组患者术后1d MMSE评分的差异无统计学意义(F=2.253,P=0.136);试验组患者术后2d、3 d MMSE评分均高于对照组,差异有统计学意义(P<0.05);2组患者术后2d、3d MMSE评分均显著高于术后1d,差异有统计学意义(P<0.05)。试验组术后7d的认知功能障碍发生率为17.0%(9/53),显著低于对照组的37.7%(20/53),差异有统计学意义(P<0.05);术后认知功能障碍患者中TNF-α和IL-6表达高于术后无认知功能障碍的患者,差异有统计学意义(P<0.01)。结论使用七氟烷吸入麻醉可减轻对老年腹腔镜手术患者认知功能的损害,并且能通过抑制体内IL-6和TNF-α水平表达而促进术后神经功能恢复。

    • 摘要: Background: Gastric cancer is one of the most common malignant tumors in the world. In recent years, studies on chemotherapy-induced cognitive impairment have been reported mainly on young women with breast cancer. Only few studies on chemotherapy-related cognitive impairment in patients with gastric cancer. Objective: To investigate the effect of chemotherapy on cognitive function in patients with advanced gastric cancer. Methods: Sixty patients with stage IV gastric cancer who received fluorouracil based chemotherapy were selected as gastric cancer group, including 25 patients with S-1 (S-1 group) and 35 patients with capecitabine (capecitabine group). 60 healthy controls matched with age and education level were recruited as control group. Mini Mental State Examination (MMSE), Montreal Cognitive Assessment (MoCA) and Auditory Verbal Learning Test (AVLT) were used to test. Results: There was no significant difference in MMSE, MoCA and AVLT between healthy control group and gastric cancer group before chemotherapy. MMSE, MoCA, immediate memory and delayed memory decreased in gastric cancer patients after chemotherapy (t = 2.206, t = 2.399, t = 2.206, t = 2.654, P Conclusion: Patients with gastric cancer will have chemotherapy-related cognitive dysfunction after chemotherapy. It is necessary to evaluate and guide or treat patients with cognitive function, so as to improve the quality of life of patients.

    • 摘要: 目的分析上肢机器人训练对血管性认知障碍患者蒙特利尔认知评估量表(MOCA)、精神状态评估表(MMSE)及肢体功能的影响。方法选取2020年5月至2021年10月该院收治的血管性认知障碍患者70例作为研究对象,采用随机数字表法分为研究组和对照组,每组35例。对照组采取常规康复训练,研究组在常规康复训练基础上采取上肢机器人训练。对比两组患者康复训练前后MOCA、MMSE、运动功能评价量表(FMA)上肢运动功能评分等。结果两组患者康复训练前MOCA、MMSE、FMA上肢运动功能评分比较,差异均无统计学意义(P>0.05);两组患者康复训练后MOCA、MMSE、FMA上肢运动功能评分均明显升高,且研究组患者康复训练后MOCA、MMSE、FMA上肢运动功能评分均明显高于对照组,差异均有统计学意义(P<0.05)。结论在血管性认知障碍患者的康复训练中采取上肢机器人训练能有效增强患者认知功能和上肢运动功能,对其病情康复具有积极作用。

    • 摘要: Optical Wireless Communication(OWC)is a new trend in communication systems to achieve large bandwidth,high bit rate,high security,fast deployment,and low cost.The basic idea of the OWC is to transmit data on unguided media with light.This system requires multi-carrier modulation such as Orthogonal Frequency Division Multiplexing(OFDM).This paper studies optical OFDM performance based on Intensity Modulation with Direct Detection(IM/DD)system.This system requires a non-negativity constraint.The paper presents a framework for wireless optical OFDM system that comprises(IM/DD)with different forms,Direct Current biased Optical OFDM(DCO-OFDM),Asymmetrically Clipped Optical OFDM(ACO-OFDM),Asymmetrically DC-biased Optical OFDM(ADO-OFDM),and Flip-OFDM.It also considers channel coding as a tool for error control,channel equalization for reducing deterioration due to channel effects,and investigation of the turbulence effects.The evaluation results of the proposed framework reveal enhancement of performance.The performance of the IM/DD-OFDM system is investigated over different channel models such as AWGN,log-normal turbulence channel model,and ceiling bounce channel model.The simulation results show that the BER performance of the IM/DD-OFDM communication system is enhanced while the fading strength is decreased.The results reveal also that Hamming codes,BCH codes,and convolutional codes achieve better BER performance.Also,two algorithms of channel estimation and equalization are considered and compared.These algorithms include the Least Squares(LS)and the Minimum Mean Square Error(MMSE).The simulation results show that the MMSE algorithm gives better BER performance than the LS algorithm.

    • 摘要: Communication and positioning,the two pillars of mobile communication systems,are currently being integrated together.The development of communication technologies is the driving force of the positioning progress.In turn,the location information provided by positioning improves the communication performance in various ways.However,the competition of these two functions in terms of resource allocation is a critical issue hindering their integration.In this article,we investigate the trade-off for the integrated communication and data-assisted positioning in multiple-input multiple-output orthogonal frequency division multiplexing systems.A data-assisted positioning method is designed first,which uses both positioning reference signals(PRSs)and data signals for positioning.The positioning and communication performance are theoretically evaluated respectively,then combined to obtain an integrated performance metric.The trade-off is analyzed and the integrated performance is optimized considering the priority of different functions.Numerical simulations show that the data-assisted positioning can not only improve the positioning accuracy,but also reduce the PRS overhead.And the established integrated performance metric can identify the optimal performance and the corresponding resource allocation schemes.

    • 摘要: 为了克服多径信道下码间串扰带来的影响,本文对SC-FDE系统中的信道估计和均衡算法进行研究.首先分析了DFT插值算法原理,提出采用格雷互补序列估计信道频率响应的方法,然后对MMSE均衡估计参数进行了详细推导,最后通过Matlab对均衡算法中时域滤波进行仿真验证,结果表明该方法能够有效滤除噪声,提升多径信道下系统接收性能,具有一定的应用价值.

    • 摘要: 目的:基于黔北地区中老年2型糖尿病(Type 2 Diabetes Mellitus,T2DM)患者探讨MMSE和MoCA量表在认知障碍患者的筛查作用.方法:选择2018年2月-2019年2月在遵义医科大学第三附属医院住院的中老年T2DM患者31例,分别采用简易智力状态检查量表(MMSE)和蒙特利尔认知评估量表(Montreal Cognitive Assessment,MoCA)对其认知功能进行评估和两量表得分进行比较,并采用一致性检验分析两者一致性.结果:认知功能障碍组与正常组MMSE与MoCA量表评分比较,正常组MMSE得分[28.00(27.25,29.00)]高于认知功能障碍组[24.00(21.00,26.00)],差异有统计学意义;正常组MoCA得分[27.00(26.50,29.00)]高于认知功能障碍组[19.00(16.00,22.25)],差异有统计学意义;MMSE与MoCA量表一致性检验的Kappa=0.314,一般一致性,MoCA诊断的阳性率(83.87%)高于MMSE(61.29%);MoCA+MMSE与MoCA量表一致性检验的Kappa=0.421,中等一致性;MoCA+MMSEMMSE量表一致性检验的Kappa=0.933,高度一致性.结论:联合应用MMSE与MoCA量表进行认知功能障碍患者进行筛查,有望提高在临床工作中对认知障碍患者的诊断效力,更适合基层开展.
