    • 摘要: Background: This is still a public problem that needs to be solved urgently: chronic and refractory wound healing with long course and complex pathological mechanism. At present, there is still a lack of effective clinical treatment. This study, therefore, aims at exploring moist exposed burn therapy/moist exposed burn ointment (MEBT/MEBO) combined with Zhuang medicine detoxification in the treatment of chronic refractory wound healing. Methodology: 100 SPF Wistar rats were randomly divided into blank control group, model control group, MEBO group, Zhuang medicine group and Combined group, with 20 rats in each group. Open wound model was established in blank control group, and chronic refractory wound model was established in other groups. Black control group and model control group were given food and water freely, MEBO group was given dressing change once a day, Zhuang medicine group was given intragastric administration once a day, and combined group was given dressing change and intragastric administration once a day. The effective rate of wound healing was observed after 12 days of continuous intervention. Conclusion: Skin regeneration medical technique combined with Zhuang medicine poison theory can effectively reduce the symptoms of bleeding and exudation, reduce the area of wound, shorten the healing time of wound, and achieve physiological healing of wound. It has a good effect on chronic refractory wound.

    • 摘要: 目的:观察MEBT/MEBO(皮肤再生医疗技术)治疗糖尿病足溃疡的临床疗效.方法:选取我院2016年1月至2017年12月收治的糖尿病足溃疡患者60例,将其随机分为2组,各30例.采用贝复济凝胶治疗对照组,采用MEBT/MEBO治疗观察组.比较两组治疗后疗效及各时段创面面积变化.结果:在治疗有效率上,观察组为93.33%较对照组76.67%明显较高,且P<0.05.在治疗后各时段创面面积上,观察组较对照组均明显缩小,且P<0.05.结论:MEBT/MEBO治疗糖尿病足溃疡的疗效显著,即可有效促进患者创面愈合,因此值得临床应用推广.

    • 摘要: 目的:观察皮肤再生医疗技术(MEBT/MEBO)结合中医辨证治疗糖尿病足溃疡的临床效果.方法:将确诊为糖尿病足溃疡的患者,按随机数字表法分为观察组和对照组,在基础治疗的基础上,观察组予MEBT/MEBO结合中医辨证治疗,对照单纯组予外用重组牛碱性成纤维细胞生长因子(bFGF)治疗,治疗2个疗程后,比较两组患者治疗前后临床症状评分与临床疗效.结果:治疗后,观察组的临床症候积分(15.13±2.47)分,高于对照组的(13.87±2.29)分,差异有统计学意义(t=24.8928,P<0.05).治疗后,观察组总有效率90%,高于对照组的70%,差异有统计学意义(X2=5.00,P<0.05).结论:MEBT/MEBO联合中医辨证治疗糖尿病足溃疡的效果确切,优于单纯使用外用重组牛碱性成纤维细胞生长因子治疗.

    • 摘要: 目的 观察烧伤湿性医疗技术(Moist Exposed Burn Therapy,MEBT)/湿润烧伤膏(Moist Exposed Burn Ointment,MEBO) (MEBT/MEBO)对Wagner Ⅱ级糖尿病足溃疡病人治疗28 d时创面愈合率及周围神经病变的影响.方法 选取右江民族医学院附属医院2013年10月-2016年6月间治疗的Wagner Ⅱ级糖尿病足伴有2型糖尿病的患者52例,随机分为美宝组(26例)和贝复新组(26例),前者予外用美宝湿润烧伤膏治疗,后者予外用重组牛碱性成纤维细胞生长因子凝胶治疗,对比两组患者治疗28 d时的创面愈合率,以及对比入院时及治疗14 d、28 d的密歇根神经病变筛查量表(MNSI)评分.结果 美宝组与贝复新组28 d的创面愈合率差异无统计学意义(P>0.05);两组在入院时、治疗14d和28 d的MNSI评分逐渐降低,组内三个时间点MNSI评分差异有统计学意义(P<0.05),组间差异无统计学意义(P>0.05).结论 MEBT/MEBO能促进Wagner Ⅱ级糖尿病足溃疡创面的愈合及改善糖尿病周围神经病变的症状.

    • 摘要: 目的 应用皮肤再生医疗技术(MEBT/MEBO)治疗WagnerⅠ~Ⅲ级的糖尿病足患者,探讨该技术治疗糖尿病足的疗效和作用机制.方法 选择WagnerⅠ~Ⅲ级糖尿病足患者共60例,随机分为治疗组和对照组2组,每组30例.治疗组用MEBT/MEBO,对照组用贝复济,连续治疗28 d,观察治疗效果,用ELISA检测创面组织中血管内皮生长因子(VEGF)、表皮生长因子(EGF)的表达量变化.结果 经28 d治疗后,治疗组创面愈合有效率(93.33%)高于对照组(76.67%);在治疗第14 d VEGF含量高于对照组,在治疗第14 d和第28 d EGF含量高于对照组,差异均具有统计学意义(P<0.05或P<0.001).结论 MEBT/MEBO能通过增加创面中VEGF、EGF的含量,有效减小创面范围,促进糖尿病足溃疡创面修复,适合临床推广使用MEBT/MEBO治疗糖尿病足.

    • 摘要: 目的 观察原位再生医疗技术(Moist Exposed Burn Therapy/Moist Exposed Burn Ointment,MEBT/MEBO)治疗股动脉假性血管瘤破裂人工血管植入术后创面感染的临床疗效.方法 对2010年10月福安市医院外科收治的1例因吸毒而导致一侧股动脉假性血管瘤破裂,Ⅰ期采用人工血管植入术治疗后创面感染的患者,采用MEBT/MEBO规范治疗,无损伤地清除创面坏死组织后均匀涂抹湿润烧伤膏,并用湿润烧伤膏药纱覆盖,治疗过程中严格保护裸露的人工血管,严密观察肢端的血液循环变化及创面愈合情况.结果 治疗70 d后创面愈合,患者出院.出院后3个月随访,创面愈合良好,无明显瘢痕增生,剧烈活动时会出现轻微的患肢无力、疼痛等症状,但不影响日常生活,正常活动时无不适感.结论 MEBT/MEBO治疗股动脉假性血管瘤破裂人工血管植入术后感染创面的疗效显著,能够改善创面微循环,控制创面感染,促进创面愈合及微血管再生.

    • 摘要: Objective This article aims to dynamically observe extracellular signal-regulated kinase 1/2 (ERK1/2),activated transcription factor 2 (ATF2)protein expression changes in rats’hard-to-heal wound tissues,and to elucidate the molecular mechanism of skin regenerative medicine and therapy (Moist exposed burn therapy/Moist exposed burn ointment,MEBT/MEBO)in enhancing hard-to-heal ulcer repair from the crosstalk of phosphorylated ERK1/2 activating ATF2. Methods Ninety-six SD rats were randomly divided into 4 groups:MEBO group,rb-bFGF group,model group,and blank control group,24 rats in each group. The superficial skin ulcer models were developed.The healing of wounds was observed.On days 3,7 and 14 after the models were developed,the same location tissues in every rats were collected,then the phosphoryla-ted ERK1/2,ATF2 protein expressions were measured by using Western Blotting. Results The newly pink epitheliums grew from periphery of wounds towards center.The recovery of wounds was fine.Comparison of wound healing time between MEBO group and rb-bFGF group showed there was no statistically significant difference (P >0.05),and both of MEBO group and rb-bFGF group had shorter wound healing time than that of model group (P 0.05 ),but comparing to model group yielded statistical difference (P 0.05),均小于模型组(P 0.05),但与模型组相比差异有统计学意义(P <0.01)。结论 MEBT/MEBO 能加快大鼠慢性难愈合创面的愈合速度,提高修复质量;MEBT/MEBO 可能通过激活 ERK1/2信号通路,调控磷酸化 ERK1/2、ATF2表达水平,使 ERK1/2激活 p38信号通路的下游底物 ATF2产生交互作用,共同促进慢性难愈合创面的修复。

    • 摘要: 目的:观察 MEBT/MEBO 病证结合治疗糖尿病足溃疡患者的临床疗效,以探讨 MEBT/MEBO 的治疗优势。方法选择 Wagner Ⅱ~Ⅲ级糖尿病足溃疡并具有中医湿热毒蕴、筋腐肉烂证的患者共48例,随机分为观察组(n =25)和对照组(n =23),对比两组患者的临床疗效,并选取典型案例进行分析。结果观察组患者疗效优于对照组,差异具有统计学意义(P <0.01)。经过8周治疗后,典型案例中的患者症状明显缓解,溃疡部位达到临床愈合标准。该患者湿热毒蕴、筋腐肉烂证的6项主症完全消失,5项次症除了形体消瘦短时间内无法改变,其余4项均有不同程度缓解,舌质由红绛转为舌淡红,苔黄腻转为苔白,寸口数脉消失,滑脉仍存在,趺阳脉较入院前搏动增强。结论 MEBT/MEBO 能够有效地促进溃疡创面(病)愈合,同时也能改善糖尿病足湿热毒蕴、筋腐肉烂证,对病证结合治疗糖尿病足溃疡具有极大的临床意义。

    • 摘要: 目的:用免疫组化法及超微病理技术探讨烧伤湿性医疗技术( Moist Exposed Burn Therapy, MEBT)及其配套产品湿润烧伤膏( Moist Exposed Burn Ointment, MEBO)促进大鼠糖尿病足溃疡创面愈合的机制。方法将100只SPF级雄性SD大鼠随机分为空白对照组(20只)与糖尿病造模组(80只)。采用腹腔注射链脲佐菌素( STZ)制备大鼠糖尿病模型,选取60只造模成功的大鼠并随机分为MEBO组、贝复济组、模型组(每组20只)。4组大鼠均建立足溃疡模型,空白对照组、模型组外敷生理盐水, MEBO组外敷湿润烧伤膏,贝复济组外敷贝复济。治疗第5天和第14天,各组分别处死10只大鼠,利用电镜观察溃疡创面肉芽组织细胞的超微结构,并用免疫组化技术测定TGF-β1、 Smad3蛋白的表达量,对比各组观察及测定结果。结果治疗第5天和第14天, MEBO组与贝复济组溃疡创面肉芽组织细胞的超微结构逐步改善,细胞内各细胞器的形态结构逐渐恢复, MEBO组、贝复济组肉芽组织细胞的超微结构情况明显优于模型组。连续换药第5天, MEBO组TGF-β1、 Smad3蛋白的表达量均明显高于模型组( P<0.001),且MEBO组、空白对照组、贝复济组3组相比, TGF-β1、 Smad3蛋白的表达量无明显差异;第14天, MEBO组TGF-β1、 Smad3蛋白的表达量均明显低于空白对照组(P<0.001),且MEBO组、贝复济组、模型组3组相比, TGF-β1、 Smad3蛋白的表达量无明显差异。结论 MEBT/MEBO能改善细胞的超微结构,并通过动态调节TGF-β1、 Smad3蛋白的表达量,促进糖尿病足溃疡创面的愈合,同时可在一定程度上减少瘢痕形成。%Objective To explore the mechanism of Moist Exposed Burn Therapy ( MEBT ) and its supportive product Moist Exposed Burn Ointment ( MEBO) in promoting diabetic foot ulcer wound healing in rats by using the immuno-histochemical method and ultrastructural pathological techniques.Methods One hundred SPF male SD rats were randomly divided into a blank control group (20 rats) and a diabetes model group (80 rats).Diabetic rat models were prepared in the diabetes model group by intraperitoneal injection of Streptozotocin ( STZ) and finally 60 diabetic rats were selected to be ran-domly divided into MEBO group, the bovine basic fibroblast growth factors( bFGF) group and model group (20 rats in each) .After foot ulcer models were established in rats of all the four groups, the blank control group and the model group were dressed with normal saline, while MEBO group was dressed with MEBO and bFGF group was dressed with the bovine basic fibroblast growth factors.Five and fourteen days later, 10 rats were put to death in each group respectively.Ultra-structure of granulation tissues of ulcer wounds was observed by using TEM microscope and the expression levels of Protein TGF-β1 & Smad3 were detected using immunohistochemical techniques, of which the results were compared between groups.Results On the day 5 and 14 of treatment, the cellular ultra-structures of granulation tissues in MEBO group and bFGF group were gradually improved, with gradual recovery of organelles in terms of morphology and structure, which was obviously superior to that in the model group.After 5 days of continuous dressing, the expression levels of Protein TGF-β1&Smad3 in MEBO group were obviously higher compared with the model group (P<0.001), while the expression levels of Protein TGF-β1 &Smad3 in MEBO group, blank control group and bFGF group didn’t differ significantly.On the day 14, the expressions of protein TGF-β1 & Smad3 in MEBO group were both significantly lower than that in the blank control group ( P<0.001 ) , while there were no significant difference between the expressions of protein TGF-β1 & Smad3 in MEBO group, bFGF group and model group as compared with each other.Conclusion MEBT/MEBO can improve the cellular ultra-structure and promote diabetic foot ulcer wound healing by dynamically adjusting the expression of protein TGF-β1 &Smad3, as well as reduce scarring to some extent.
