    • 摘要: 目的探讨血清胱抑素C(Cys-C)、内皮细胞生长因子(VEGF)、糖类抗原153(CA153)在乳腺癌诊断中的价值。方法选取2018年6月至2020年11月该院74例乳腺癌患者作为观察组,另选取同期74例体检健康者作为对照组。比较两组血清Cys-C、VEGF、CA153水平,不同TNM分期乳腺癌患者血清Cys-C、VEGF、CA153水平,比较血清Cys-C、VEGF、CA153单独检测与联合检测的检出率,分析血清Cys-C、VEGF、CA153水平和TNM分期的相关性。结果观察组血清Cys-C、VEGF、CA153水平均高于对照组(P<0.05);TNMⅠ~Ⅱ期患者血清Cys-C、VEGF、CA153水平较TNMⅢ~Ⅳ期患者低(P<0.05);3项指标联合测诊的检出率为90.54%,均高于各项单独检测(P<0.05);Pearson相关分析结果显示,血清Cys-C(r=0.598)、VEGF(r=0.623)、CA153(r=0.619)和乳腺癌TNM分期呈正相关(P<0.05)。结论乳腺癌患者血清Cys-C、VEGF、CA153水平较健康女性高,并和TNM分期有关,而3项联合检测的乳腺癌检出率较高,可为临床乳腺癌筛查提供参考依据。

    • 摘要: 目的分析苦劳汤坐浴辅助肛周脓肿内口封闭联合负压封闭引流技术治疗对高位肛周脓肿患者的临床价值。方法选取2011年1月—2018年12月收治的高位肛周脓肿138例,根据治疗方法分为研究组和对照组,每组69例。对照组行负压封闭引流技术,研究组在对照组基础上联合苦劳汤坐浴辅助肛周脓肿内口封闭术治疗。比较2组肛门括约肌功能、术后疼痛、创面愈合时间以及术后复发情况;治疗前后C反应蛋白(CRP)、白介素-6(IL-6)及肿瘤坏死因子-α(TNF-α)水平;治疗前、治疗后1 d及治疗后1周血清血管内皮细胞生长因子(VEGF)、血小板衍生内皮细胞生长因子(PD-ECGF)条带灰度值。观察2组临床疗效。结果研究组Wexner评分、视觉模拟评分法(VAS)评分低于对照组,创面愈合时间短于对照组(P<0.01)。治疗后2组CRP、IL-6及TNF-α水平低于治疗前,且研究组低于对照组(P<0.05,P<0.01)。治疗后1周研究组VEGF与PD-ECGF条带灰度值高于对照组(P<0.01)。研究组临床总有效率高于对照组(P<0.01)。结论苦劳汤坐浴辅助肛周脓肿内口封闭联合负压封闭引流技术治疗高位肛周脓肿可抑制炎症反应、促进VEGF、PD-ECGF表达,在减轻疼痛、缩短愈合时间、增强括约肌功能、临床疗效方面优于单一负压封闭引流术。

    • 摘要: 目的 探讨苏氏接骨胶囊(Su)对大鼠胫骨干骨折早期愈合的影响及其作用机制.方法 雄性SD大鼠36只,行胫骨干开放骨折髓内钉固定造模术后,随机分为3组,分别为假干预组(Sham组),1倍剂量组(Su-140组)和2倍剂量组(Su-280组),次日给予Su进行药物干预10d后,取患肢行微计算机断层扫描技术(Micro-CT),分析免疫组化,并检测外周血中内皮细胞生长因子(VEGF)、血小板衍生生长因子(PDGF)、血小板-内皮细胞黏附分子(PECAM-1/CD31)等因子mRNA水平的表达情况,并计数骨痂内血管数.结果 Su-140组、Su-280组骨痂的骨体积分数(Tb.BV/TV):(0.272 ±0.025、0.410 ±0.065)高于Sham组(0.215±0.026,P=0.037、0.001),且Su-280组明显高于Su-140组(P =0.001);Su-280组的骨小梁厚度(Tb.Th)[(0.087±0.016) mm]高于Sham组和Su-140组[(0.057±0.010)、(0.063 ±0.010) mm,p=0.001、0.006],Sham组和Su-140组间比较差异无统计学意义(P=0.376);Su-280组的骨小梁数量(Tb.N)[(3.948±0.472) 1/mm]高于Sham组[(3.138±0.263) 1/mm]和Su-140组[(3.430±0.301) 1/mm,P=0.001、0.019],Sham组和Su-140组间差异无统计学意义(P=0.161);Real-time PCR结果显示VEGF、PDGF、CD31各因子mRNA水平表达量,Su-280组各因子相对表达量(1 335.852±208.668、5 599.593±315.075、1 683.905±420.355)均高于Sham组(931.667±101.872、5085.952±151.719、1 186.030±241.147;P=0.001、0.011、0.014);Su-140组VEGF的表达量(1 122.848±125.963)高于Sham组(P =0.046),PDGF、CD31(5 198.763 ±395.535、1 277.982±223.717)同Sham组间差异无统计学意义(P =0.531、0.613);Su-280组各因子相对表达量均高于Su-140组(P=0.029、0.038、0.038);骨痂内血管数计数结果显示,Su-140组(41.251±3.862)高于Sham组(35.011±3.795) (P =0.041),Su-280组(43.833±2.317)高于Su-140组和Sham组,且差异有统计学意义(P=0.034、0.001).结论 苏氏接骨胶囊在大鼠胫骨干骨折早期促进了VEGF、PDGF、CD31各因子的分泌,从而促进骨痂内血管的大量增加及骨折的愈合.%Objective To observe the effect of Su's bone fracture capsule (Su) on early healing of tibial shaft fracture in rats and explore its mechanism.Methods 36 male SD rats were randomly divided into 3 groups (n =12),for closed tibial shaft fracture with intramedullary nail fixation model operation and the second day after operation were given interventionwith Su's bone fracture capsule.There were three groups,Sham intervention group (Sham group),1 times the dose group (Su-140 group) and 2 times dosage group (group Su-280).After 10 days the rats were killed and the fracture healing of operated limbs were observed and quantitative analyzed based on Micro-CT;The mRNA of vascular endothelial growth factor (VEGF),platelet derived growth factor (PDGF),platelet endothelial cell adhesion molecule-1 (PECAM-1)/CD31 in the peripheral blood were independently detected by Real-time quantitative polymerase chain reaction (Real-time PCR);Callus tissue pathological observation and immunohistochemical staining,and counting the number of vessels in the callus were done.Results It showed that by Micro-CT,compared with the Sham,the Tb.BV/TV ofgroup Su-140 and group Su-280 (0.272-0.025,0.410±0.065) were higher than Sham (0.215 ±0.026) (P=0.037,P=0.001),and group Su-280 was significantly higher than Su-140 (P =0.001);trabecular thickness of the gToup Su-280 (Tb.Th) (0.087 ± 0.016) mm were higher than that of Sham and group Su-140 [(0.057 ± 0.010) mm,(0.063 ±0.010) mm,P =0.001,0.006],but there was no significant difference between the Sham and group Su-140 (P =0.376);the number of trabecular bone in group Su-280 (Tb.N) (3.948 ±0.472) l/mm were higher than Sham [(3.138 ± 0.263) l/mm] andgroup Su-140 [(3.430 ±0.301) 1/mm,P =0.001,0.019],but there was no significant difference between the Sham and group Su-140 (P =0.161);Real-time PCR results showed that the expression level of VEGF,PDGF and CD31 of group Su-280 were relatively (1 335.852 ± 208.668,5 599.593 ± 315.075,1 683.905±420.355) higher than Sham (931.667 ± 101.872,5085.952-± 151.719,1 186.030± 241.147;VEGF:P=0.0O1,PDGF:P=0.011,CD31:P=0.014);The level of VEGF ingroup Su-140 (1 122.848 ± 125.963) was higher than Sham,and there were no significant differences in the levels of PDGF and CD31 (5 198.763 ± 395.535,1 277.982 ± 223.717) between the Sham and group Su-140 (P=0.531,0.613);Su-280 group relative expression of each factor was higher than that in group Su-140 (VEGF:P =0.029,PDGF:P =0.038,CD31:P =0.038);Thecounting resultsshowed that the number of blood vessels in bone callus of the group Su-140 (41.251 ± 3.862) was higher than Sham (35.011 ±3.795) (P=0.041),group Su-280 (43.833 ±2.317) was higher than group Su-140 and Sham group,and the difference was significant (P =0.034,P =0.001).Conclusion The Su's bone fracture capsule in the process of rats with tibial shaft fracture has a positive role in promoting the secretion of VEGF,PDGF,CD31 and a substantial increase in bone callus vascular,and then accelerate the early calcification and bone fracture callus healing.

    • 摘要: 目的 观察通心络联合外周血干细胞移植治疗对糖尿病足溃疡大鼠促血管新生的影响.方法 2012年10月—2013年8月于河北以岭医药研究院药理实验室进行实验.高脂喂养联合低剂量链脲佐菌素(STZ)建立糖尿病大鼠模型,通过冰块降温和液氮冷冻,诱导大鼠糖尿病足溃疡模型.将造模成功的50只大鼠分为正常对照组、模型组、通心络组、干细胞组、通心络+干细胞组5组,每组10只,正常对照组和模型组大鼠以等体积的生理盐水灌胃;通心络组给予通心络0.8 g/kg灌胃,1次/d;干细胞组给予干细胞移植治疗;通心络+干细胞组给予干细胞移植同时给予通心络0.8 g/kg灌胃,1次/d,于给药7 d后处死动物取材进行指标检测.ELISA法测定血清血管内皮生长因子(VEGF)、晚期糖基化终末产物(AGE)、晚期糖基化终末产物受体(RAGE),RT-PCR法测定组织局部VEGF mRNA、RAGE mRNA表达水平.结果 各组间比较血清VEGF、AGE、RAGE有显著差异(F=28.675、18.018、35.946,P均=0.000),与模型组比较,通心络组、干细胞组及通心络+干细胞组血清VEGF显著增加(P0.05);与干细胞组比较,通心络组显著增加溃疡局部VEGF mRNA表达(P<0.01),降低RAGE mRNA表达(P<0.05),通心络+干细胞组显著增加溃疡局部VEGF mRNA表达(P=0.002),显著降低RAGE mRNA表达(P<0.01);与通心络组比较,通心络+干细胞组增加溃疡局部VEGF mRNA表达(P<0.05),降低RAGE mRNA表达(P<0.05).结论 通心络联合外周血干细胞移植能够提高糖尿病足溃疡大鼠血清和溃疡局部组织中VEGF水平,降低AGE、RAGE水平,促进血管新生.%Objective To discuss promoting angiogenesis by observing the levels of vascular endothelial growth factor (VEGF),advanced glycation end products(AGE),advanced glycation end products receptor(RAGE) in the diabetic foot ul-cers rats treated by Tongxinluo capsule combined with peripheral blood stem cell transplantation. Methods From October 2012 to August 2013,the research was conducted in Hebei Yiling Medicine Pharmacology Laboratory. The diabetic rat model was es-tablished. Diabetic foot ulcers rats were randomly divided into five groups(n=10):normal control,model,TXL,stem cell, TXL + stem cells,the serum level of VEGF,AGE,RAGE were determined by enzyme linked immunosorbent assay,the local organizations levels of VEGF mRNA,RAGE mRNA expression were examined by RT PCR method. Results The comparison between groups of serum VEGF,AGE and RAGE have significant difference (F=27.675,F=18.018,F=35.946,P=0. 000),compared with model group, Tongxinluo capsule group and Tongxinluo capsule + stem cell group serum VEGF in-creased significantly (P=0.003, P=0.000), AGE, RAGE decreased significantly (P=0.006, P=0.000), stem cell group increased(P=0.015),AGE and RAGE decreased(P=0.036,P=0.021);Tongxinluo capsule group showed signif-icantly increased serum VEGF (P=0.006), AGE and RAGE decreased (P=0.017, P=0.030), Tongxinluo capsule +stem cell group's serum VEGF increased significantly (P=0.000),AGE,RAGE significantly decreased (P=0.002, P=0.000);compared with Tongxinluo capsule group,Tongxinluo capsule + stem cell group showed increased of serum VEGF (P=0.041), AGE and RAGE decreased (P =0.035, P =0.013); Among the groups, ulcer tissue VEGF mRNA and RAGE mRNA expression have significant difference (F=8.152,F=7.650, P=0.000), compared with the model group, Tongxinluo capsule group increased the expression of VEGF mRNA and decreased the expression of RAGE mRNA (P=0. 014,P=0.023), Tongxinluo capsule + stem cells group significantly increased the expression of VEGF mRNA and de-creased the expression of RAGE mRNA (P=0.003, P=0.000); in stem cell group, the expression of RAGE mRNA and VEGF mRNA in ulcer tissue showed no significant difference compared with the model group(P=0.235,P=0.470);com-pared with the stem cell group,Tongxinluo capsule group significantly increased the expression of VEGF mRNA in local ulcer (P=0.008) and decreased the expression of RAGE mRNA (P=0.039), Tongxinluo capsule + stem cells group signifi-cantly with increased local VEGF mRNA expression(P=0.002),significantly reduced the expression of RAGE mRNA(P=0.001); compared with Tongxinluo capsule group, Tongxinluo capsule + stem cell group increased ulcer's expression of VEGF mRNA (P=0.032),reduced the expression of RAGE mRNA (P=0.027). Conclusion Tongxinluo capsule com-bined peripheral blood stem cell transplantation can improve the VEGF levels in serum and local organizations of diabetic foot ulcers rats,and reduce AGE,RAGE levels,promoting angiogenesis.

    • 摘要: 目的 探讨复方化浊解毒汤联合DCF化疗方案对中晚期胃癌患者血清内皮细胞生长因子(VEGF)及环氧化酶-2(COX-2)水平的影响.方法 选取74例中晚期胃癌患者,随机将其分为观察组和对照组,每组37例.2组患者均给予DCF化疗方案,观察组在DCF化疗方案基础上同时给予复方化浊解毒汤进行治疗.以28 d为1个治疗周期,化疗2个周期.观察并记录患者疗效、毒副反应以及生活质量改善情况,检测患者治疗前、治疗1个周期后、治疗2个周期后血清中VEGF及COX-2的表达水平.结果 治疗2个周期后观察组有效率明显高于对照组(P<0.05);治疗2个周期后2组生活质量评分(KPS)均明显优于治疗前(P均<0.05),且观察组明显优于对照组(P<0.05);治疗1,2个周期后2组血清VEGF及COX-2表达水平均明显低于治疗前(P均<0.05),治疗2个周期后观察组VEGF及COX-2表达水平均明显低于对照组(P均<0.05).结论 复方化浊解毒汤联合DCF化疗方案可明显降低中晚期胃癌患者血清中VEGF以及COX-2的表达水平,对患者生活质量具有明显提高的作用,值得临床推广.

    • 摘要: 目的本研究以根尖周囊肿为疾病模型,根据其临床特点确定骨破坏速率,并利用免疫组织化学方法观察间质中微血管密度;同时原代培养囊壁成纤维细胞,检测其中各种VEGF异构体对微血管增生及破骨细胞诱导效率的影响,探讨VEGF可变剪接在牙源性囊肿骨破坏过程中的作用。方法选取19例根尖周囊肿,以CD34抗体免疫组化染色凸显血管内皮细胞,并计算血管腔面积,曲面断层影像进行病变面积的测量,并选取其中13例新鲜手术标本进行根尖周囊肿囊壁成纤维细胞培养,制备条件培养基和诱导破骨细胞,并进行RNA的提取及realtime PCR检测VEGF异构体的相对表达量。χ~2检验用于评估CD34阳性的血管腔面积和临床特点。血管腔面积、各种异构体相对表达量、囊肿进展速率和破骨细胞诱导效率的关系由线性回归及Spearman’s等级相关评估。结果 VEGF-121a/总VEGF和VEGF-165a/总VEGF正相关于微血管管腔面积,后者则与囊肿骨破坏速率正相关;而VEGF和VEGF-189a的相对表达量正相关于破骨细胞诱导效率,VEGF-165b/总VEGF和VEGF-189b/总VEGF则与破骨细胞诱导效率成负相关。结论提示VEGF-xxxa型可能主要促进囊肿的骨坏过程,而VEGF-xxxb型则可能有抑制效果。

    • 摘要: Objective To explore the preparation and quality control of As2 O3 nanoparticle .Methods PEG‐PLA was used as the vector material to prepare As2 O3 nanoparticle with ultrasonic emulsification method ,and the VEGFR‐2 was coupled to obtain VEGFR‐2/As2 O3‐PEG‐PLA nanoparticle .The particle size distribution ,Zata potential ,loading efficiency (LE) ,encapsulation effi‐ciency(EE) ,drug release in vitro and stability was determined ,and morphological characteristics was observed by transmission elec‐tron microscope(TEM) .Tweety‐four hepatocellular carcinoma nude mices were randomly divided into VEGFR‐2/As2O3‐PEG‐PLA nanoparticles group and As2 O3‐PEG‐PLA nanoparticles group ,by tail vein injection of nanoparticles .High performance liquid chro‐matography was used to determine content of As2 O3 .After 21 d ,six nude mices in each group were killed ,and the immunohisto‐chemistry and western blot method was used to detect the expression of VEGFR‐2 .Results The particle size of VEGFR‐2/As2 O3‐PEG‐PLA was determined to be (141 .9 ± 13 .2)nm ,Zata potential was (10 .2 ± 1 .1)mV .It was found to spherical or oval shape , with uniform size and dispersibility under TEM .LE and EE was (5 .51 ± 1 .83)% and (62 .12 ± 5 .98)% ,respectively .Drug release in vitro showed that VEGFR‐2/As2 O3‐PEG‐PLA exhibited controlled release effect ,with half of the release time as 10 h .Besides , VEGFR‐2/As2 O3‐PEG‐PLA showed a good stability in 3 days .Compared with As2 O3‐PEG‐PLA nanoparticles group ,the concen‐tration of As2 O3 in tumor and liver tissue was high ,the concentration of As2 O3 in blood ,heart ,kidney tissue was low ,the expression of VEGFR‐2 in tumor tissue was low in VEGFR‐2/As2O3‐PEG‐PLA nanoparticles group(P< 0 .05) .Conclusion The prepared As2 O3 nanoparticle using PEG‐PLA as vector and VEGFR‐2 as target showed uniform size ,high EE and LE ,good stability .And it preliminarily proved that VEGFR‐2 could be targeted in nude mice .%目的:探讨三氧化二砷(As2O3)纳米新剂型的制备和质量控制。方法以聚乙醇化聚乳酸(PEG‐PLA)为载体材料,W/O/W型超声乳化制备As2O3纳米粒,同时偶联具有肝癌靶向作用的VEGFR‐2,最终得到人源抗VEGFR‐2/As2O3‐PEG‐PLA纳米粒。对其粒径分布、Zata电位、载药量和包封率进行测定,通过透射电镜(TEM)观察其表观形态,同时考察其体外释药和稳定性。选择24只肝癌裸鼠,随机分为VEGFR‐2/As2O3‐PEG‐PLA组及As2O3‐PEG‐PLA组,通过尾静脉注射纳米粒,高效液相色谱法测定As2O3在两组裸鼠体内的分布;免疫组化及蛋白质印迹法(Westernblot)检测血管内皮细胞生长因子(VEGF)表达量。结果本实验制得的As2O3纳米制剂VEGFR‐2/As2O3‐PEG‐PLA粒径为(141.9±13.2)nm,Zata电位为(10.2±1.1)mV;经TEM观察呈圆形或椭圆形颗,粒状、大小较一致,分散性较好;载药量为(5.51±1.83)%,包封率为(62.12±5.98)%。体外释放发现其具有缓释效果,半数释放时间t1/2分别为10h;初步稳定性考察结果发现该制剂稳定性良好。与As2O3‐PEG‐PLA组比较,VEGFR‐2/As2O3‐PEG‐PLA组中肿瘤、肝组织的As2O3浓度较高,心、血液、肾组织的As2O3浓度较低(P<0.05),且肿瘤组织中VEGFR‐2阳性率及蛋白表达较低。结论以PEG‐PLA为载体材料制备得到As2O3纳米制剂,且偶联具有肝癌靶向作用的VEGFR‐2,最终得到粒径分布均匀,包封率和载药量高,稳定性良好的纳米制剂,且初步证实在肝癌裸鼠体内具有良好的靶向作用。
