    • 摘要: BACKGROUND Epidemiologic studies have revealed a decrease in the prevalence of Helicobacter pylori(H.pylori)infection in Western Europe.AIM To obtain data regarding the prevalence of H.pylori in Csongrád and Békés Counties in Hungary,evaluate the differences in its prevalence between urban and rural areas,and establish factors associated with positive seroprevalence.METHODS One-thousand and one healthy blood donors[male/female:501/500,mean age:40(19–65)years]were enrolled in this study.Subjects were tested for H.pylori IgG antibody positivity via enzyme-linked immunosorbent assay.Subgroup analysis by age,gender,smoking habits,alcohol consumption,and urban vs nonurban residence was also performed.RESULTS The overall seropositivity of H.pylori was 32%.It was higher in males(34.93%vs 29.2%,P=0.0521)and in rural areas(36.2%vs 27.94%,P=0.0051).Agricultural/industrial workers were more likely to be positive for infection than office workers(38.35%vs 30.11%,P=0.0095)and rural subjects in Békés County than those in Csongrád County(43.36%vs 33.33%,P=0.0015).CONCLUSION Although the prevalence of H.pylori infection decreased in recent decades in Southeast Hungary,it remains high in middle-aged rural populations.Generally accepted risk factors for H.pylori positivity appeared to be valid for the studied population.

    • 摘要: AIM: To assess serum cartilage oligomeric matrix protein(COMP) as a marker of cirrhosis and risk of progression to hepatocellular carcinoma(HCC). METHODS: A COMP enzyme-linked immunosorbentassay was used to test 187 patients with chronic liver diseases at the time point of first evaluation. The selected patients included 72 with chronic hepatitis B infection, 75 with chronic hepatitis C infection, 22 with primary biliary cirrhosis, 7 with autoimmune hepatitis type 1, and 11 with alcoholic liver disease. Demographic, biochemical, histological and clinical characteristics of the patients were recorded at the first evaluation. One hundred and forty-seven patients were followed for a median [interquartile range(IQR)] duration of 96.5(102) mo. The clinical, biochemical and histological data, as well as the development of cirrhosis, HCC according to internationally accepted criteria and in case of death, a liver-related cause during the follow-up period, were recorded at the electronic database of our clinic. COMP determination was also performed in 43 healthy individuals who served as the control study group.RESULTS: COMP positivity(> 15 U/L) was detected in 22%-36% among chronic liver disease groups. Strikingly, almost 83% of COMP-positive patients were cirrhotic at baseline, independently of cause of liver disease. Among the patients who developed HCC during follow-up, 73.7%(14/19) were COMP positive at baseline. COMP positivity was significantly associated with older age(P < 0.001), advanced fibrosis(P = 0.001) and necroinflammatory activity(P = 0.001), higher aspartate aminotransferase(P < 0.001), alanine aminotransferase(P < 0.02), γ-glutamyl transpeptidase(P = 0.003), alkaline phosphatase(P = 0.001), bilirubin(P < 0.05), international normalized ratio(P = 0.002) and alpha-fetoprotein levels(P < 0.02), and lower albumin(P < 0.001), and platelet count(P = 0.008). COMP levels [median(IQR)] were significantly higher in cirrhotics compared to non-cirrhotics [13.8(7.9) U/L vs 9.8(4.6) U/L, respectively; P < 0.001]. On multivariate logistic regression analysis, COMP-positivity was independently associated only with cirrhosis(OR = 4.40, 95%CI: 1.33-14.69, P = 0.015). Kaplan-Meier analysis showed that COMP positivity was significantly associated with HCC development(P = 0.007) and higher incidence of liver-related death(P < 0.001). CONCLUSION: Elevated COMP levels are strongly associated with cirrhosis and HCC progression. Serum COMP is a new promising non-invasive biomarker for HCC risk assessment in surveillance programs.

    • 摘要: Enzyme-linked immunosorbent assay (ELISA) is the most widely used method for measuring a single cytokine. Recent developments in cytokine quantification such as multiple arrays measure multiple cytokines simultaneously. Although good correlations between ELISA and multiplex methods have been observed, side by side comparisons are limited. In the present study we hypothesized that ELISA and Luminex techniques are comparable in detecting cytokines in culture medium when pulmonary artery smooth muscle cells (PASMC) are exposed to stress. Primary human PASMC were cultured in modular chambers and exposed to 21% FiO2 and peak inspiratory and positive end expiratory pressure of 24 and 8 cmH2O respectively, and 95% FiO2. At 24 hours, culture medium was collected and assayed for interleukin-6 (IL-6) and IL-8 by quantitative ELISA and by Human Cytokine 25-Plex Panel using a Luminex 200 analyzer. A comparative analysis of agreement between our ELISA and Luminex data was detailed for control and stress conditions using the Bland-Altman plot analysis. Each assay resulted in comparable increased (p < 0.001) levels of IL-6 and IL-8 as compared to control in response to oxidative and biophysical stress. The Bland-Altman analysis demonstrated that 95% of the differences between ELISA and Luminex values were within ±1.96 SD from the mean difference indicated by the 95% limits of agreement for the measurements of IL-6 and IL-8. There was no systematic bias as a function of inflammation level. We conclude that in this cell culture model, ELISA and Luminex are comparable in detecting the levels of IL-6 and IL-8 in the culture medium. If measurements of multiple cytokines are demanded and the amount of sample is limited, Luminex multi-analyte profiling technology is accurate and sensitive.

    • 摘要: The purpose of this study is to compare the levels of IL-6, 8 in tear fluids of people exposed to mustard gas in the war between Iraq and Iran who had the chronic dry-eye symptoms compared to the normal group. In this study, 25 of the patients who were exposed to mustard gas and had chronic dry eye symptoms were compared to 25 patients as control group, consisting of 25 people who had common chronic dry eye symptoms with blepharitis and 25 healthy people as normal group. The levels of IL-6 and IL-8 in tear fluid of people of these three groups were assessed by enzyme-linked immunosorbent assay (ELISA). The results for levels of IL-6 (P = 0.002) and IL-8 (P = 0.001) in tear fluid of patients in comparison with normal group show a significant increase. The differences were considered statistically significant at P < 0.05. The effect of exposure to mustard gas on eyes of chemical-injured veterans destroyed meibomian glands which paved the way for evaporative type of dry-eye. As a result, the cited ILs in the tear fluid of these patients increased and resulted in later eye-impairments.

    • 摘要: AIM:To investigate serum insulin-like growth factorbinding protein 5(IGFBP-5)levels and intestinal IGFBP-5expression in patients with Crohn’s disease(CD).METHODS:We analyzed the serum concentrations and intestinal expression of IGFBP-5 in 42 patients with CD,of whom 26 had endoscopically or radiologically proven stricture formation.Nine of the 42 patients had active disease,with a Crohn’s disease activity index>150.Serum IGFBP-5 levels were analyzed in 20 healthy controls matched by sex and age to the CD patients.Serum IGFBP-5 was measured using an enzyme-linked immunosorbent assay.Intestinal tissue was obtained from patients through endoscopic biopsies.IGFBP-5expression was detected using immunohistochemistry and was scored semiquantitatively.RESULTS:The median serum IGFBP-5 concentrations of CD patients were significantly lower compared with healthy controls[median 7.2(IQR:5.5-11.3)ng/mL vs 11.3(8.0-44.6)ng/mL,P<0.001].There was no significant difference between median serum IGFBP-5levels in CD patients with or without stricture formation[6.9(5.5-11.3)ng/mL vs 7.8(5.3-10.1)ng/mL,P=0.815].The serum IGFBP-5 levels were not significantly different between patients with active disease and inactive disease[7.2(6.5-7.6)ng/mL vs 7.2(5.5-11.3)ng/mL,P=0.890].However,a significant correlation was observed between serum IGFBP-5 levels and platelet count(PLT)(r=0.319,P=0.0395).No significant correlation was found between tissue IGFBP-5 immunohistochemical staining intensity scores and serum IGFBP-5 levels.No significant difference was found when comparing the serum IGFBP-5 levels among the patients with different tissue IGFBP-5 staining scores(absent/very weak,weak,moderate or strong).There was a significant correlation between tissue IGFBP-5staining scores and white blood cell count(r=0.391,P=0.01)and PLT(r=0.356,P=0.021).CONCLUSION:Our results indicate that serum IGFBP-5 concentrations were lower in CD patients compared to healthy controls regardless of disease activity or the presence of stricture formation.

    • 摘要: In this study, we describe the advantages and disadvantages of direct competitive enzyme-linked immunosorbent assay (dc-ELISA) and indirect competitive ELISA (idc-ELISA) and ELISA methods for the detection of neomycin. Antineomycin polyclonal antibodies were prepared and used to detect neomycin by dc-ELISA and idc-ELISA. The cross-reaction rates of prepared anti-neomycin polyclonal antibodies with gentamincin and kanamycin were 2.04% and 0.02%, respectively, and with ampicillin, erythromycin and tetracycline all less than 0.01%. The accuracy and recovery of idc-ELISA were tested with an intra-plate error of less than 4%, an inter-plate error of less than 11% and a recovery between 135.5% and 191.3%. The detection limits of dc-ELISA and idc-ELISA were 28.58 ng/mL and 51.74 ng/mL, respectively, both of which were below the national maximum residue limit (MRL) of 500 pg/kg. Therefore, a dc-ELISA method and an idc-ELISA method to detect neomycin have successfully established.     • 摘要: Colorectal cancer is characterized by a low survival rate even though the basis for colon cancer development,which involves the evolution of adenomas to carcinoma,is known.Moreover,the mortality rates continue to rise in economically transitioning countries although there is the opportunity to intervene in the natural history of the adenoma–cancer sequence through risk factors,screening,and treatment.Screening in particular accounted for most of the decline in colorectal cancer mortality achieved in the USA during the period 1975-2000.Patients show a better prognosis when the neoplasm is diagnosed early.Among the variety of screening strategies,the methods range from invasive and costly procedures such as colonoscopy to more low-cost and non-invasive tests such as the fecal occult blood test(guaiac and immunochemical).As a non-invasive biological serum marker would be of great benefit because of the performance of the test,several biomarkers,including cytologic assays,DNA and mRNA,and soluble proteins,have been studied.We found that the soluble CD26(sCD26)concentration is diminished in serum of colorectal cancer patients compared to healthy donors,suggesting the potential utility of a sCD26 immunochemical detection test for early diagnosis.sCD26 originates from plasma membrane CD26 lacking its transmembrane and cytoplasmic domains.Some 90%–95%of sCD26 has been associated with serum dipeptidyl peptidase IV(DPPIV)activity.DPP-IV,assigned to the CD26 cluster,is a pleiotropic enzyme expressed mainly on epithelial cells and lymphocytes.Our studies intended to validate this test for population screening to detect colorectal cancer and advanced adenomas are reviewed here.
