BACKGROUND Transcatheter aortic valve replacement(TAVR)provokes an early injury response,partially represented by dynamic changes in inflammatory markers.TAVR greatly benefits the elderly and we aimed to determine whether increased inflammatory markers post-TAVR in octagenerians were different than their younger counterparts and whether it was associated with adverse clinical outcomes.METHODS Patients with severe symptomatic aortic stenosis who underwent transfemoral TAVR from January 2010 to December 2021 were enrolled.Total white blood cells(WBC)count and subpopulation dynamics were evaluated.RESULTS Five-hundred and seven patients were finally included in the study,65%of these patients were 80 or more years old(54%female,median age 84[82-87])years,with severe symptomatic aortic stenosis.In patients aged above 80 years(patients≥80),we noticed significant kinetic changes in the WBC and their differential cellular subpopulations(P<0.0001)between admission and early days post-procedure.This was evident by a significant increase in total WBC(median 7.1 to 9.4)and absolute neutrophil count(median 4.7 to 7.4),neutrophil-lymphocyte(NL)ratio(median 2.82 to 7.21),and a meaningful decrease in absolute lymphocyte count(median 1.5 to 1.0).Implantation of self-expandable valves(SEVs)was associated with a more pronounced inflammatory response than balloon-expandable valves(BEVs).Higher WBC and neutrophil counts were associated with higher mortality and major vascular complications at 30 days,in addition,higher neutrophil counts and NL ratios were found to be correlated to arrhythmia at 30 days with P values of 0.04 and 0.028,respectively.CONCLUSION This is the first description of a differential age-related inflammatory response in patients after TAVR,which shows an association between inflammatory markers post procedure and clinical outcome.Nevertheless,survival rates were similar in the elderly population and in younger patients,despite the presence of comorbid conditions.

The striatum is the largest structure of the basal ganglia,receiving synaptic input from multiple regions including the neocortex,thalamus,external globus pallidus,and midbrain.One of its major roles is to integrate sensorimotor,associative and limbic information.The development and evolution of basal ganglia is one of the most fundamental and important questions in neuroscience.Here,we segmented the fetal striatum using 7.0T MR images from 46 specimens from 14 to 22 gestational weeks and revealed the developmental course of the fetal striatum using volume and shape analyses.The absolute volume of caudate nucleus(CN)was linearly expanded and moderate hemisphere effect was observed in the absolute volume of CN.While the relative volume had a nonlinear relationship with GW,and there was no obvious difference between hemispheres.Both absolute and relative volumes of the putamen showed linear expansion,and there was no obvious difference between hemispheres.

The orchestrated and well-coordinated attack on China, coming soon after China declared complete victory over absolute poverty that saw nearly 100 million lifted out of poverty in the past eight years, is not accidental.

Liver transplantation(LT)for hepatocellular carcinoma(HCC)was characterized in its early experience by high recurrence rates due to extensive tumor burden(1).Only after the adoption of Milan criteria(2),LT was recognized as a standard treatment for HCC(3).Besides HCC,other hepatic malignancies were proposed to be treated in the past with LT but due to the poor results observed(4),these indications were abandoned.Among them,colorectal liver metastases(CRLM)were considered an absolute contraindication until recently.

In this paper, the radial temperature distributions of the blown CO2 arcs in a model gas circuit breaker were investigated by optical emission spectroscopy methods. The CO2 flows with different flow rates(50, 100 and 150 1 min^-1) were created to axially blow the arcs burning in a polymethyl methacrylate(PMMA) nozzle. Discharges with different arc currents(200 and 400A) were conducted in the experiment. The absolute intensity method was applied for a carbon ionic line of 657.8 nm to obtain the radial temperature profiles of the arc columns at a cross-section 1 mm above the nozzle. The calibration for the intensity of the CⅡ 657.8 nm line was achieved by the Fowler–Milne method with the help of an oxygen atomic line of 777.2 nm.The highest temperature obtained in the arc center was up to 19 900 K when the arc current was 400 A and the CO2 flow rate was 50 1 min^-1, while the lowest temperature in the arc center was about 15 900 K when the arc current was 200 A and the CO2 flow rate was 150 1min^-1. The results indicate that as the arc current increases, the temperature in the arc center would also increase apparently, and a larger gas flow rate would lead to a lower central temperature in general. It can also be found that the influence of the CO2 flow rate on the arc temperature was much less than that of the arc current under the present experimental conditions. In addition,higher temperature in the arc center would cause a sharper temperature decrease from the central region towards the edge.

BACKGROUND Recent evidence shows that long non-coding RNAs(lncRNAs) are closely related to hepatogenesis and a few aggressive features of hepatocellular carcinoma(HCC). Increasing studies demonstrate that lncRNAs are potential prognostic factors for HCC. Moreover, several studies reported the combination of lncRNAs for predicting the overall survival(OS) of HCC, but the results varied. Thus,more effort including more accurate statistical approaches is needed for exploring the prognostic value of lncRNAs in HCC.AIM To develop a robust lncRNA signature associated with HCC recurrence to improve prognosis prediction of HCC.METHODS Univariate COX regression analysis was performed to screen the lncRNAs significantly associated with recurrence-free survival(RFS) of HCC in GSE76427 for the least absolute shrinkage and selection operator(LASSO) modelling. The established lncRNA signature was validated and developed in The Cancer Genome Atlas(TCGA) series using Kaplan-Meier curves. The expression values of the identified lncRNAs were compared between the tumor and non-tumor tissues. Pathway enrichment of these lncRNAs was conducted based on the significantly co-expressed genes. A prognostic nomogram combining the lncRNA signature and clinical characteristics was constructed.RESULTS The lncRNA signature consisted of six lncRNAs: MSC-AS1, POLR2 J4, EIF3 J-AS1,SERHL, RMST, and PVT1. This risk model was significantly associated with the RFS of HCC in the TCGA cohort with a hazard ratio(HR) being 1.807(95%CI[confidence interval]: 1.329-2.457) and log-rank P-value being less than 0.001. The best candidates of the six-lncRNA signature were younger male patients with HBV infection in relatively early tumor-stage and better physical condition but with higher preoperative alpha-fetoprotein. All the lncRNAs were significantly upregulated in tumor samples compared to non-tumor samples(P < 0.05). The most significantly enriched pathways of the lncRNAs were TGF-β signaling pathway, cellular apoptosis-associated pathways, etc. The nomogram showed great utility of the lncRNA signature in HCC recurrence risk stratification.CONCLUSION We have constructed a six-lncRNA signature for prognosis prediction of HCC.This risk model provides new clinical evidence for the accurate diagnosis and targeted treatment of HCC.
