    • 摘要: 肾移植受者长期使用免疫抑制剂,导致其免疫功能低下,容易伴发各种病原体感染。近年来随着人类微小病毒B19(HPV-B19)感染的检测技术发展和肾移植手术的增多,肾移植术后HPV-B19的感染率呈逐年上升的趋势,是导致术后纯红细胞再生障碍性贫血(PRCA)的重要原因之一,影响移植肾功能恢复,甚至导致移植肾损伤或预后不良。为了进一步规范肾移植受者HPV-B19感染的诊断和治疗,中华医学会器官移植学分会和国家肾脏移植质控中心组织专家,从HPV-B19病原学、流行病学特点、临床表现、诊断、预防、治疗、存在的问题及展望等方面,制订肾移植术后HPV-B19感染的临床诊疗规范,以期为我国肾移植术后HPV-B19感染的规范化防治提供指导。

    • 摘要: [目的]了解成年牦牛(Bos grunniens)菱脑中促红细胞生成素(erythropoietin,EPO)及其受体(erythro-poietin receptor,EPOR)的表达分布特征,对其与低氧适应性之间的关系进行探讨.[方法]利用qRT-PCR及免疫组织化学技术,对EPO及EPOR在牦牛菱脑中的表达与分布进行研究.[结果]EPO和EPOR mRNA在成年牦牛小脑蚓部的相对表达量最高,EPO及EPOR蛋白主要表达于小脑皮质浦肯野氏细胞胞质.[结论]在低氧适应过程中,EPO及EPOR在牦牛菱脑蚓前叶的表达量最高,因此牦牛在低氧状况下其小脑蚓前叶可能发挥更重要的功能作用.

    • 摘要: 目的 观察振幅整合脑电图(aEEG)在评估促红细胞生成素(EPO)治疗早产儿脑损伤的应用分析.方法 选择2019年1月至2020年6月入住佛山市南海区妇幼保健院NICU病房的早产儿,选择胎龄28~34周、体重0.05).而治疗后较治疗前血细胞计数、转氨酶、血肌酐指数降低,且治疗组患儿血细胞计数、转氨酶、血肌酐、输血次数均少于对照组,而窒息史、肺表面活性物质应用、近期并发症率则低于对照组.治疗组监测aEEG平均评分中均高于对照组(P<0.05).结论 以aEEG评估EPO治疗早产儿脑损伤的治疗,可以改善患儿的血液指标,减少输血次数、窒息与近期并发症发生率,并降低肺表面活性物质的应用,效果理想.

    • 摘要: 目的:检测急性前葡萄膜炎(AAU)患者血清和房水中促红细胞生成素(EPO)、缺氧诱导因子-1α(HIF-1α)表达水平,探究二者的临床意义.方法:前瞻性研究.选取2018-01/2020-12本院收治的60例AAU患者为研究对象,同期增殖性玻璃体视网膜病变患者60例为对照组,收集两组患者血清及房水,使用酶联免疫吸附试验(ELISA)检测血清和房水中EPO、HIF-1α水平;并对AAU患者进行焦虑自评量表(SAS)和抑郁自评量表(SDS)评价,疾病严重程度评分;Pearson法分析AAU患者SAS评分、SDS评分与血清和房水中EPO、HIF-1α水平的相关性;Spearman分析AAU患者疾病严重程度评分与血清和房水中EPO、HIF-1α水平的相关性.结果:与对照组相比,AAU患者血清和房水中EPO、HIF-1α水平均升高(P0.05),血清和房水中EPO与HIF-1α水平均呈正相关(P<0.05).Spearman分析AAU患者疾病严重程度评分与血清和房水中EPO、HIF-1α水平均呈正相关(P<0.05).结论:AAU患者血清和房水中EPO、HIF-1α均高表达,且两者与疾病严重程度评分关系密切.

    • 摘要: Objective To investigate the role of bone marrow mesenchymal stem cells (BMSCs) pretreated with erythropoietin (EPO) in the prevention of acute rejection after renal transplantation in rats. Methods BMSCs were divided into five groups: control group (without EPO), group A (pretreated with EPO at a final concentration of 10 IU/mL), group B (pretreated with EPO at a final concentration of 100 IU/mL), group C (pretreated with EPO at a final concentration of 500 IU/mL) and group D (pretreated with EPO at a final concentration of 1 000 IU/mL). In each group, the BMSCs were cultured for 24 h and 48 h. The proliferation rate of the BMSCs was determined by 3-(4,5-dimethyl-2-thiazolyl)-2,5-diphenyl-2-H-tetrazolium bromide (MTT) assay. The BMSCs were divided into two groups: BMSC group (without EPO) and EPO-BMSC group (pretreated with EPO at a final concentration of 500 IU/mL). After 48 h culture, Western blot was adopted to measure the expression level of CXC chemokine receptor (CXCR) 4 protein in BMSCs. Wistar rats were used as the donors, and SD rats were utilized as the recipients to establish the rat models with acute rejection after renal transplantation. The recipient rats were randomly divided into four groups (n=6 in each group) including the control group (without any intervention), EPO group (injection of 1 mL of solution containing 500 IU EPO via tail vein immediately after surgery), BMSC group (injection of 1 mL of solution containing 1×106/mL BMSCs via tail vein immediately after surgery) and EPO-BMSC group (injection of 1 mL of solution containing 1×106/mL BMSCs cultured in vitro with 500 IU/mL EPO via tail vein). The level of serum creatinine (Scr) level was determined by Scr detection kit. Western blot was used to detect the expression levels of interferon (IFN)-γ and interleukin (IL)-4 proteins. Results After 24 h culture, the proliferation rate of BMSCs did not significantly differ among all groups (all P>0.05). After 48 h culture, the proliferation rate of BMSCs in group C (pretreated with EPO at a final concentration of 500 IU/mL) was significantly higher than that in the control group (P0.05). At 5 d after operation, the levels of Scr in the EPO, BMSC and EPO-BMSC groups were significantly lower than that in the control group (all P0.05). At 1 d after surgery, compared with control group, the expression levels of IFN-γ protein and IFN-γ/IL-4 ratio in the renal tissues in the EPO, BMSC and EPO-BMSC groups were significantly decreased to varying extents (all P0.05).培养48 h,与对照组比较,C组(EPO终浓度500 IU/mL)的BMSC增殖率较高,差异有统计学意义(P0.05);术后5 d,与对照组比较,EPO组、BMSC组和EPO-BMSC组受体鼠的Scr水平均明显降低,差异均有统计学意义(均为P0.05).术后1 d,与对照组比较,EPO组、BMSC组及EPO-BMSC组受体鼠移植肾组织中IFN-γ蛋白表达水平和IFN-γ/IL-4均有不同程度降低,差异均有统计学意义(均为P<0.05).术后5 d,与对照组比较,EPO组、BMSC组和EPO-BMSC组中IFN-γ蛋白表达水平和IFN-γ/IL-4均有不同程度降低,差异均有统计学意义(均为P<0.05),其中EPO-BMSC组中IFN-γ蛋白表达水平和IFN-γ/IL-4均低于EPO组和BMSC组,差异均有统计学意义(均为P<0.05).结论 EPO预处理BMSC可预防肾移植术后急性排斥反应,保护移植肾功能.

    • 摘要: 促红细胞生成素(erythropoietin,EPO)是一种含唾液酸的酸性糖蛋白,是人体内源性的一种生理活性物质,传统观点认为,EPO促进晚期红系祖细胞的繁殖、分化和成熟.近年来越来越多的研究表明,EPO具有神经保护作用,其机制可能包括抑制炎症反应和水肿、抗凋亡、抑制兴奋性氨基酸的毒性作用、营养神经等.根据来源可分为内源性和外源性EPO.有动物实验已证实,外源性EPO可通过血脑屏障(blood-brain barrier,BBB)进而在脑内发挥神经保护作用.本文主要就EPO对脑出血(intracerebral hemorrhage,ICH)后发挥脑保护作用机制的研究进展作一综述.

    • 摘要: 多发性骨髓瘤(MM)是一种常见的浆细胞恶性肿瘤,好发于老年人,约2/3的MM患者易并发贫血.MM贫血会随着MM病情的进展而加重,严重影响患者的生活质量.MM贫血的发生机制主要包括细胞因子(如白介素-6、肿瘤坏死因子-α和白介素-1等)抑制红系造血、促红细胞生成素(EPO)减少、反复感染及血黏度升高等.目前针对MM贫血的治疗主要包括原发病治疗、输血治疗及EPO治疗等.本文简要综述MM贫血的发病机制及目前的治疗方法,旨在为MM的诊治提供理论依据.

    • 摘要: 目的 检测促红细胞生成素(erythropoietin,EPO)、CD105在髓母细胞瘤(MB)中的表达情况,旨在对MB预后判断及寻求新的有效治疗方法提供依据.方法 采用免疫组织化学S-P法检测68例髓母细胞瘤组织中EPO蛋白、CD105的表达,并检测微血管密度(MVD),分析其与EPO的关系,并绘制生存曲线图.结果 ①髓母细胞瘤EPO中蛋白阳性表达率为73.53%(50/68例),明显高于正常脑组织,二者表达差异有统计学意义(P<0.01);CD105蛋白标记的MVD在髓母细胞瘤和正常脑组织中的计数分别为(24.94士8.85)、(0.96士0.68)个,二者间差异亦有统计学意义(P<0.01).②相关性分析发现,肿瘤的EPO强弱与微血管密度呈明显正相关;COX生存分析发现,高危组、EPO表达阳性是影响患者预后的危险因素.结论 EPO在髓母细胞瘤中有较高的阳性表达率,CD105的表达在髓母细胞瘤的发生、发展中具有重要作用,可能受EPO调节.EPO表达强弱是影响患者预后的危险因素.

    • 摘要: 目的:分析急性上消化道出血后贫血患者的促红细胞生成素(EPO)随时间变化趋势及其与贫血程度之间的关系。方法将46例急性上消化道出血后贫血患者纳入研究(观察组),取同期46例健康体检者作为对照组;采取观察组患者出血后第1d、第4d、第7d的空腹外周静脉血,对照组受检者则采集1次即可,通过对受检者EPO浓度的检测,对比分析观察组患者各个时间段与对照组受检者的 EPO水平情况。结果(1)急性上消化道出血后贫血患者的EPO水平随时间的变化呈先上升后下降的趋势;观察组患者第4d的EPO水平与对照组相比,明显要高(P <0.05)。(2)在EPO水平方面,重度贫血患者显著高于中、轻度贫血者,组间数据差异有统计学意义(P <0.05)。结论急性上消化道出血后贫血患者的EPO水平随时间的变化呈先上升后下降的趋势,EPO水平最高的是重度贫血患者。%Objective to analyze time tendency of erythropoietin of anemia patient with acute upper gastrointestinal hemorrhage and its relationship between anemia degree.Methods 46 cases anemia patients with acute upper gastrointestinal hemorrhage were included in observation group, and 46 cases healthy persons at same period were included in control group. Observation group patients were detected with EPO concentration of fasting peripheral venous blood on the 1st, 4th, 7th day of hemorrhage, and control group with fasting peripheral venous blood. Compare and analyze EPO levels between observation group and control group subjects.Results (1) EPO level of anemia patient with acute upper gastrointestinal hemorrhage showed increase first and then decrease with time changing, that of observation group on the 4th day is significantly higher than control group (P<0.05). (2) EPO level of severe anemia patients was significantly higher than with light and mild anemia patients, difference between groups showed statistical significance(P<0.05). Conclusion EPO level of anemia patient with acute upper gastrointestinal hemorrhage showed increase first and then decrease with time changing, severe anemia patients showed the highest EPO level.
