    • 摘要: The aim of the study is to assess the factors that influence the sustainability of agricultural development. The study was carried out in the municipality of Bobonaro for about four months, from July to October 2021. Slovin’s method was used to determine the sample size, with a desired percentage of 10% of the total population. Thus, 154 producers were interviewed as respondents and key persons in the area of agriculture. The variables interviewed are the main factors of agricultural productivity and the factors that affect sustainable agricultural development. The results of the descriptive statistical analysis of the data showed that about 94.87% of the respondents’ productive area is in operation, with an average of 1.84 ha per respondent and the abandoned area around 0.11 ha per respondent. The average value of rice production is 2.38 ton per ha and corn 1.07 ton per ha. About 87% of producers raised the main types of animals such as cattle, pigs, goats with an average density of 2 to 5 animals per establishment. About 61% of producers have access to the market at a normal price for agricultural products. The factors that hinder less successful agricultural development, such as: those of nature with the greatest impact are climate changes, which influence changes in cultivation sessions and in the characteristics of productive soils, availability of water for irrigation and animal feed. On the socioeconomic side, it includes the willingness or interest of young people who want to work as farmers, less investment and the impact of the Covid-19 pandemic. It is concluded that natural and socioeconomic impacts can reduce agricultural productivity, so that this will be a major challenge for the development of the agricultural sector in the future.

    • 摘要: Background: Breast cancer has become an important disease burden harmful to women’s health. The treatment of breast cancer is mainly surgical treatment. In the context of enhanced recovery after surgery, the average length of hospital stay has been reduced, so there is increasing concern about postoperative patient discharge readiness. Objective: To identify factors influencing readiness for hospital discharge among patients having undergone breast cancer surgery and to explore the relationship between discharge readiness and the quality of discharge teaching. Methods: A total of 99 patients after breast cancer surgery at a regional tertiary medical tumor center in Guangzhou between June 2021 and December 2021 were collected using the convenience sampling method. A general data questionnaire, readiness for hospital discharge scale, and quality of discharge teaching scale were used to conduct a questionnaire survey on the subjects. Results: 83.8% of the patients feel ready for discharge, and the score of discharge readiness was 155.98 ± 28.87. The patients’ content received and the skills in teaching delivery were the influencing factors of discharge readiness. Discharged with a drainage tube was an independent factor of discharge readiness. Conclusions: Readiness for hospital discharge among patients undergoing breast cancer surgery was above medium level, and perfect intervention measures should be formulated for the influencing factors of discharge readiness. Especially, specific executable safeguard measures should be formulated for patients who are discharged early after surgery to promote the safety of home care.

    • 摘要: BACKGROUND The factors influencing the prognosis of patients with esophageal cancer vary among studies and are still poorly known.AIM To determine the factors associated with survival in patients with esophageal cancer.METHODS This retrospective study included patients with esophageal cancer admitted between January 2017 and March 2020 at Heping Hospital Affiliated to Changzhi Medical College. All patients were treated according to the available guidelines. Follow-up was censored in October 2020. Univariable and multivariable Cox regression analyses were used to determine the independent risk factors for overall survival(OS).RESULTS In total, 307 patients were included. Their median age was 64(range, 44-79) years, 63.5% were male, and the median disease course was 2(0.1-36) months. The median tumor size was 3(0-10) cm. Most patients were T3(29.6%), N0(70.0%). Most tumors were grade 2(48.2%), and 87.3% were squamous cell carcinoma. The in-hospital mortality was 16.9%, the 30-day mortality was 19.9%, and the 90-day mortality was 25.4%. The cumulative OS rates at the last follow-up were 82.1%(95%CI: 67.7%-96.5%) for stage 0/Ⅰ/Ⅱ and 47.4%(95%CI: 16.5-78.6%) for stage Ⅲ/ⅣA(P < 0.001). The multivariable analysis showed that creatinine levels(HR = 1.02, 95%CI: 1.00-1.03, P = 0.050), pTNM Ⅲ/ⅣA(HR = 4.19, 95%CI: 2.19-8.01, P < 0.001), adjuvant radiotherapy and/or chemotherapy(HR = 0.23, 95%CI: 0.11-0.49), and the Comprehensive Complication Index(CCI)(HR = 1.02, 95%CI: 1.004-1.03, P = 0.011) were independently associated with OS.CONCLUSION The survival of patients with esophageal cancer is poor, especially those with pTNM Ⅲ/ⅣA. pTNM stage Ⅲ/ⅣA, CCI, and adjuvant therapy(radiotherapy and/or chemotherapy) are independently associated with OS.
