    • 摘要: AIM:To investigate the effect of neuropathic factor ( NGF) on visual acuity and visual evoked potential ( VEP) in patients with optic nerve contusion.?METHODS:Totally 78 patients ( 78 eyes ) with optic nerve contusion were selected. From January 2013 to June 2016, 39 cases ( 39 eyes ) were divided into observation group and control group respectively according to the random number table method. Prednisone, vitamins and mecobalamin tablets treatment were given to both groups, based on that, the observation group was given NGF treatment, continuous treatment of 2 courses (21d for a course of treatment).?RESULTS: There was no significant difference in visual field defect and visual field sensitivity between the observation group and the control group before treatment (P>0.05). After treatment, the visual field defect degree of the observation group was smaller, the visual field sensitivity was better than that of the control group ( P0. 05). After treatment, the P100 wave latency of the observation group was significantly shorter than that of the control group ( P0.05);治疗后,观察组的视野缺损度范围小于对照组(P0.05);治疗后,观察组患者的P100波潜伏期小于对照组(P<0.05)、观察组的P100波振幅显著高于对照组(P<0.05);治疗后,观察组的视力显著优于对照组(P<0.05).结论:NGF治疗视神经挫伤能显著地改善患者视力、VEP指标,减小视野缺损、提升视野光敏感度.

    • 摘要: Objective To investigate the clinical effect and safety of mouse nerve growth injection on optic nerve contusion and its impact on visual evoked potential (VEP).Methods The subjects of this study were selected from 68 cases (68 eyes)of pa-tients with optic nerve contusion admitted in our hospital from June 2014 to December 2016,who were randomly divided into the observation group and the control group,each with 34 cases (34 eyes).The control group was treated with glucocorticoids and vi-tamins.The observation group was administered with mouse nerve growth factor 30 μg+2 mL sterile water injected into the mus-cle,in addition to what was given to the control group.It was given to the observation group once daily for 6 weeks.Patients'vi-sion,central vision field grayscale,latency and amplitude of VEP and adverse reactions before and after treatment between the two groups were compared.Results After treatment,the overall effective rate of the observation group was 91.18%,which had signif-icant difference from 70.59% of the control group (P 0.05).Conclusion In treatment of optic nerve contusion, mouse nerve growth injection has a significant curative effect.It's helpful for patients to restore their vision and VEP improve-ment,safe and worth clinical application.%目的 探讨注射用鼠神经生长因子治疗视神经挫伤的临床疗效及对视觉诱发电位(VEP)的影响.方法 选取68例(68眼)视神经挫伤患者为研究对象,随机分为观察组和对照组各34例(34眼).对照组给予糖皮质激素及维生素类药物治疗;观察组在对照组基础治疗上给予注射用鼠神经生长因子30μg+2 mL灭菌注射用水肌内注射,1次/d,疗程6周.对比2组患者治疗前后视力、中心视野灰度、VEP潜伏期和波幅、不良反应发生情况.结果 治疗后观察组总有效率91.18%明显高于对照组70.59%,差异具有统计学意义(P0.05).结论 注射用鼠神经生长因子对视神经挫伤临床治疗效果显著,有助于患者视力恢复及VEP的改善,安全性高,值得应用.

    • 摘要: 目的:观察外伤性视神经挫伤后应用鼠神经生长因子治疗疗效,评价该药的治疗效果.方法:选取2012-01/2016-01收治的外伤性视神经挫伤患者48例50眼进行回顾性研究,随机分为两组,治疗组24例24眼,对照组24例26眼;治疗组主要应用鼠神经生长因子30μg,每日1次,肌肉注射,用药6wk,同时给以高压氧舱、糖皮质激素类及维生素类药物;对照组除不加用鼠神经生长因子外,其余治疗同治疗组.所有患者均在用药后6wk复查视力、视觉诱发电位(visual evoked potential,VEP)、平均光敏感度(mean sensitivity,MS)、视野平均缺损(mean deviation,MD).结果:用药6wk后,治疗组与对照组最佳矫正视力比较,差异有统计学意义(P<0.05);治疗组VEP P100波潜伏期为98.76±6.93ms、振幅5.22±1.64μV,对照组分别为116.52±8.82ms、4.28±1.75μV,两组间差异均有统计学意义(P<0.05);治疗组与对照组MS、MD比较,差异均有统计学意义(P<0.05).结论:外伤性视神经挫伤应用鼠神经生长因子治疗后,有更为明显的疗效.%AIM:To observe the effect of nerve growth factor treatment on traumatic optic nerve contusion and evaluate its therapeutic effect.METHODS:From January 2012 to January 2016,48 cases (50 eyes) with optic nerve contusion were treated.Retrospective study was carried out and the patients were randomly divided into two groups,treatment group 24 cases of 24 eyes,26 eyes of 24 cases in the control group;the treatment group mainly used mouse nerve growth factor injection 30μg,1 times daily intramuscular injection of medication,6wk,with the application of high pressure hyperbaric oxygen therapy,glucocorticoid and vitamin treatment;the treatment of control group was the same as treatment group,but without the injection of mouse nerve growth factor.Visual acuity,visual evoked potential (VEP) and mean sensitivity (MS),and mean deviation (MD) were examined in all patients at 6wk after the treatment.RESULTS:After treated for 6wk,there were significant differences in the best corrected visual acuity (P< 0.05)between the two groups.VEP P100 latency of treatment group was 98.76±6.93ms,amplitude was 5.22±1.64μV,those of the control group were 116.52±8.82ms,4.28± 1.75μV,both significantly different between the two groups (P<0.05).The differences on MS and MD were significant (P<0.05).CONCLUSION:The treatment of traumatic optic nerve contusion with nerve growth factor injection has more obvious curative effect.

    • 摘要: 目的 分析视神经挫伤患者采用大剂量甲泼尼龙与多种药物联合实施综合治疗的临床效果.方法 抽取过去一段时期内在本院接受药物治疗的视神经挫伤疾病患者138例,将其采用数学随机列表方式分为对照组、研究1组、研究2组,每组46例.对照组在常规药物基础上,加用地塞米松进行治疗;研究1组在常规药物基础上,加用大剂量甲泼尼龙进行治疗;研究2组在研究1组的基础上加用鼠神经生长因子进行治疗.对比3组研究对象视神经损伤药物治疗总有效率、视力水平恢复正常时间和用药治疗总时间、药物原因导致的不良反应.结果 研究2组患者视神经损伤药物治疗总有效率达到91.3%,高于研究1组的76.1%和对照组的58.7%(P<0.05);视力水平恢复正常时间和用药治疗总时间短于研究1组和对照组(P<0.05);仅有2例药物原因导致的不良反应,少于研究1组的7例和对照组的12例(P<0.05).结论视神经挫伤患者采用大剂量甲泼尼龙与多种药物联合实施综合治疗,特别是加用鼠神经生长因子,可以使视力水平在短时间内得到显著改善,减少不良反应,提高治疗效果.

    • 摘要: 目的: 探讨鼠神经生长因子联合甲钴胺治疗视神经挫伤的临床效果和安全性.方法: 选取2015年10月-2017年3月在本院治疗的64例视神经挫伤患者,采用随机数字表法分为观察组和对照组,每组32例.对照组在常规治疗的基础上仅给予甲钴胺治疗,观察组在常规治疗的基础上给予鼠神经生长因子联合甲钴胺治疗,比较两组患者的治疗效果、视野缺损程度和不良事件发生情况.结果: 治疗1、2个疗程后,观察组的总有效率均显著高于对照组,视野缺损程度低于对照组,差异均有统计学意义(P<0.05),且两组患者均未出现严重的不良事件.结论: 鼠神经生长因子联合甲钴胺治疗视神经挫伤具有较好的临床效果,可显著改善患者的视力恢复情况,减轻视野缺损程度,且安全性较好,可考虑在临床推广应用.

    • 摘要: Objective To investigate the condition of eye injury,the causes,the correlation with different kinds of brain trauma of traumatic brain injury combined with ocular injury.Methods In neurosurgery department,1036 patients were retrospectively analyzed.These patients had traumatic brain injuries in their first visiting.Forty-six patients out of 1036 had craniocerebral trauma combined with complex ocular injuries.These patients were analyzed to find the injured part of the eye,the causes,the relation with traumatic brain injuries,the treatment of ocular injuries and the recovery of their visual function.Results According to the medical records,40 patients of these 46 patients were men,accounting for 86.96%.Six patients were female,accounting for 13.04%.And their ages were from 15 to 72 years old with an average of (40.11 ± 11.89) years old.These patients included 13 cases of orbital wall fracture accounting for 28.26%,11 cases of eyelid complex laceration accounting for 23.91%,10 cases (11 eyes) of eyeball rupture accounting for 21.74%,7 cases of optic nerve injury accounting for 15.22% and 5 cases of other ocular injuries accounting for 10.87%.The causes for the injuries were traffic accidents in 28 cases (60.87%),violence in 7 cases (15.22%),explosion in 5 cases (10.87%) and others in 6 cases (13.04%).The treatments were as follows.Enucleation was done for 8 eyes out of 11 eyes with open ocular trauma due to the serious damage.Screening optic canal decompression was carried out for 3 patients with clear optic canal fracture in emergent treatment,and the vision was improved after the operation.For the other 4 patients,glucocorticoid impact therapy was carried out and only 1 patient had vision improvement.It was relatively easy to diagnose traumatic brain injuries combined with orbital wall fracture by CT and most of this kind of damages can be repaired by the subsequent surgery.For 2 patients with external ophthalmoplegia,the function of ocular muscle was partially recovered while diplopia remained.Conclusion For patients with traumatic brain injuries combined with ocular injuries,the principle of treatment should be on the premise of saving lives,meanwhile,serious eyeball rupture,perforation and optic nerve contusion should be treated as soon as possible to save the visual function.%目的 分析颅脑外伤合并眼外伤患者的眼部损伤状况、致伤原因及与不同性质脑外伤的相关性.方法 回顾性分析神经外科收治首次就诊的脑外伤1036例,其中合并眼外伤者46例,分析其眼损伤部位、外伤原因、与不同性质脑损伤的相关性、治疗方法和对视功能的影响.结果 颅脑外伤合并眼外伤46例(47只眼).男40例,占86.96%;女6例,占13.04%.年龄15~72岁,平均(40.11±11.89)岁.合并的眼部损伤包括:眶壁骨折13例,占28.26%;眼睑复杂裂伤11例,占23.91%;眼球穿孔及破裂伤10例(11只眼),占21.74%;视神经损伤7例,占15.22%;其他5例,占10.87%.致伤原因是:交通伤28例(60.87%),暴力伤7例(15.22%),爆炸伤5例(10.87%),其他6例(13.04%).10例(11只眼)开放性眼球外伤中8例(8只眼)行眼球摘除术,2例(3只眼)保存了视功能.7例视神经挫伤,3例行经筛窦视神经管减压术,视力提高,4例无手术指征者给予药物治疗,1例视力提高.合并眶壁骨折经CT检查确诊,后期手术修复,对视功能影响较轻.2例合并眼肌麻痹者,诊断相对滞后,药物治疗观察期内复视未能恢复.结论 颅脑外伤合并眼外伤的患者在坚持抢救生命的前提下,尽早处理严重的眼球破裂及穿孔伤和视神经挫伤,力求挽救视功能.

    • 摘要: 目的:观察大鼠视神经挫伤后视网膜内质网应激(endoplasmic reticulum stress,ERS)变化.方法:复制大鼠视神经挫伤(optic nerve crush,ONC)模型.实验分正常对照组、假手术组和ONC组.用TUNEL方法检测细胞凋亡;用蛋白印迹技术检测凋亡相关蛋白表达和ERS相关蛋白表达;观察大鼠视网膜病理变化、检测神经节细胞(retinal ganglion cells,RGCs)数量和视网膜内侧部分厚度(retinal thickness of inner part,RTIP).结果:与正常对照组比较,假手术组大鼠视网膜RGCs数量、RTIP未见明显变化;与假手术组比较,ONC 7~21 d组大鼠RGCs数量、RTIP分别呈时间依赖性数量减少、厚度变薄,ONC 3 d组TUNEL阳性细胞数量明显增多、主要分布于RGCs,细胞凋亡相关蛋白Cleaved caspase-3、Cleaved PARP-1和ERS相关蛋白GRP78 、CHOP和caspase 12蛋白表达明显上调.结论:ONC能够诱导大鼠视网膜发生ERS,ERS可能是神经节细胞死亡的机制之一.

    • 摘要: 目的:临床观察并评价鼠神经生长因子治疗视神经挫伤的临床效果.方法:对在笔者所在医院接受治疗的37例视神经挫伤患者临床资料进行回顾性分析,并对其进行常规治疗作为此次研究对照组;另选取同期同症37例患者作为观察组,在常规治疗的基础上加用鼠神经生长因子治疗.观察两组临床治疗效果并进行比较.结果:治疗后两组患者视力情况较治疗前均有显著好转(P<0.05);治疗后两组患者中心视野灰度均下降,视觉诱发电位潜伏期均缩短,波幅均增加,其中观察组好转情况显著优于对照组,差异均有统计学意义(P<0.05);观察组治疗总有效率为91.89%,对照组治疗总有效率为70.27%,观察组明显高于对照组,差异有统计学意义(P<0.05).结论:临床使用鼠神经生长因子治疗视神经挫伤效果显著,可有效帮助患者视力恢复,推广价值较大.

    • 摘要: 目的:观察视觉诱发电位( VEP)在鼠神经生长因子治疗外伤性视神经挫伤后的变化,评价鼠神经生长因子的治疗效果。  方法:选择2013-10/2015-02我院收治的患者,共34例34眼,经临床诊断为外伤性视神经挫伤,随机分为2组。治疗组22例22眼,主要采用鼠神经生长因子30μg,每日1次,用2.0mL注射用水溶解,肌肉注射,用药42d,同时给予糖皮质激素类及维生素类药物;对照组12例12眼,除不加用鼠神经生长因子外,其他用药同治疗组。所有2组患者均在用药第21、42 d复查视力和VEP。  结果:用药42 d后,治疗组最佳矫正视力差异有统计学意义( P<0.05);治疗组 VEP P100波潜伏期为104.59依7.54ms,振幅5.12依1.48μV,对照组分别为113.25依9.21ms,4.69依1.89μV,两组之间的差异均有统计学意义( P<0.05);治疗组有效率82%,对照组42%,两组比较有统计学意义(P=0.026)。  结论:鼠神经生长因子对外伤性视神经挫伤有显著疗效, VEP变化能够客观准确的反映出临床效果,是评价临床疗效的重要依据。%AIM: To observethe changes of visual evoked potential ( VEP) of optic nerve contusion treated with mouse nerve growth factor and estimate the efficacy of mouse nerve growth factor. METHODS: Thirty-four cases (34 eyes) of optic nerve contusion enrolled from October 2013 to February 2015 were selected and randomly divided into two groups. The treatment group 22 patients ( 22 eyes ) was given the mouse nerve growth factor 30μg, with 2. 0mL dissolved in water for injection, intramuscular injection once daily, with an average treatment of 42d, while for the hormones and vitamins medication treatment. For the control group 12 cases ( 12 eyes ) , mouse nerve growth factor was not used, other drugs were the same as the treatment group. All patients were accepted a review of vision and VEP at 21, 42d after the treatment. RESULTS: After 42d treatment, the difference of best corrected visual acuity showed statistical significance ( P<0. 05 ) and VEP P100 latent period was 104. 59±7. 54ms, amplitude was 5. 12±1. 48μV in treatment group. While, latent period was 113. 25±9. 21ms, amplitude was 4. 69±1.89μV in control group. Also, VEP P100 latency of the two groups showed statistical significance ( t=-3. 132, P<0. 05) after 42d treatment, while VEP P100 amplitude had the same conclusion (t=2. 541, P<0. 05). The effective rate was 82% in treatment group, while 42% in control group with significant difference (P=0. 026). CONCLUSION: Mouse nerve growth factor has significant efficacy on traumatic optic nerve contusion, the changes of VEP can accurately reflect the conclusion, and VEP has proved to be an important basis on estimating the clinical efficacy.
