    • 摘要: 目的:观察布地奈德、特布他林压缩雾化与细辛脑静脉滴注联合治疗慢性阻塞性肺疾病急性加重(AECOPD)的疗效.方法:2017年8月-2019年8月收治AECOPD患者60例,随机分为两组,各30例.对照组采用常规治疗;观察组在常规治疗的基础上给予布地奈德和特布他林压缩雾化与细辛脑静脉滴注联合治疗.观察治疗前后第1秒用力呼吸容积(FEV1)占预计值百分比和第1秒用力呼吸容量与用力肺活量比值(FEV1/FVC).结果:观察组FVE1占预计比值百分比与FEV1/FVC均优于对照组,差异有统计学意义(P<0.05).结论:布地奈德、特布他林压缩雾化与细辛脑静脉滴注联合治疗AECOPD可更好地改善患者肺通气功能指标.

    • 摘要: 细辛脑本品主要成分为α-细辛脑,是中药石菖蒲的主要有效成分,具有平喘、止咳、祛痰、镇静、解痉、抗惊厥等作用,可治疗各类呼吸系统疾病引起的咳嗽、咳痰、喘息等.但在治疗过程中,少数成年人可产生轻微副反应,如口干、头晕、恶心、胃部不适、心慌及便秘等,罕见休克.据统计6岁以下儿童在应用细辛脑注射剂时发生严重不良反应的病例较其他年龄收的比例明显增高,主要不良反应表现为过敏性休克、过敏反应、呼吸困难等,故6岁以下儿童慎用细辛脑注射剂.现报道成年人因静滴细辛脑注射液致呼吸困难一例.

    • 摘要: 目的:探讨用细辛脑与沐舒坦对急性呼吸窘迫综合征(ARDS)患者进行治疗对其血管内皮生长因子(VEGF)与降钙素原(PCT)水平的影响.方法:将青县人民医院收治的120例ARDS患者随机分为对照组、细辛脑组、沐舒坦组与联合组,每组各有30例患者.对四组患者均进行常规治疗.在此基础上,加用细辛脑细辛脑组患者进行治疗,加用沐舒坦对沐舒坦组患者进行治疗,加用细辛脑与沐舒坦对联合组患者进行治疗,然后比较四组患者VEGF和PCT的水平.结果:治疗后,联合组患者VEGF和PCT的水平均低于其他三组患者,P<0.05.治疗后,细辛脑组患者与沐舒坦组患者VEGF和PCT的水平均低于对照组患者,P<0.05.结论:在对ARDS患者进行常规治疗的同时,加用细辛脑与沐舒坦对其进行治疗可显改善其VEGF与PCT的水平,缓解其病情.

    • 摘要: 目的 观察细辛脑对老年慢性阻塞性肺疾病急性加重期(AECOPD)的疗效,以明确细辛脑的临床应用价值.方法 将80例老年AECOPD病人随机分为观察组和对照组,每组40例.对照组采用西医常规治疗,观察组在对照组治疗的基础上加用细辛脑注射液静滴治疗.评估和比较2组的临床疗效和肺功能相关指标变化情况,包括第1秒用力呼气容积(FEV1)、FEV1占预计值百分比(FEV1%)和FEV1同用力肺活量(FVC)之间的比值(FEV1/FVC).结果 观察组的总有效率显著高于对照组(P0.05),治疗后观察组FEV1、FEV1%和FEV1/FVC均显著高于对照组(P<0.05).结论 细辛脑治疗老年AECOPD的效果明显,具有一定的辅助治疗作用和临床应用价值.

    • 摘要: 目的 初步探索细辛脑对哮喘小鼠转化生长因子-β1(TGF-β1)、血管内皮生长因子(VEGF)表达方面干预的影响,为细辛脑临床治疗哮喘提供理论指导.方法 将30只BALB/C雌性小鼠按随机数字表法分为哮喘组、细辛脑组和对照组,每组各10只.哮喘组和细辛脑组采用鸡卵白蛋白(OVA)诱导建立哮喘模型,对照组采用生理盐水替代,细辛脑组额外给予细辛脑雾化吸入干预.苏木素-伊红染色观察3组小鼠的肺组织病理变化,ELISA法检测3组小鼠肺泡灌洗液(BALF)中TGF-β1及VEGF水平,显微镜下对BALF沉淀物进行细胞计数和分类.结果 与对照组小鼠相比,哮喘组小鼠肺组织有明显的气道周围炎症细胞浸润,气管管壁增厚和狭窄,细辛脑组小鼠也有前述哮喘组的病理特点,但程度轻于哮喘组.细辛脑组BALF中TGF-β1和VEGF水平均高于对照组、低于哮喘组(P均<0.01).哮喘组的细胞总数及中性粒细胞、嗜酸性粒细胞、淋巴细胞、巨噬细胞计数均多于细辛脑组和对照组(P均<0.05),细辛脑组的中性粒细胞、嗜酸性粒细胞计数均多于对照组(P均<0.01).结论 TGF-β1和VEGF在哮喘小鼠BALF中表达,并可能参与哮喘早期气道重塑,细辛脑可降低哮喘小鼠TGF-β1和VEGF的水平,可能是其改善哮喘气道炎症和缓解气道重塑的机制之一.%Objective To primarily investigate the intervention effect of asarone upon the expression levels of transforming growth factor-β1 (TGF-β1) and vascular endothelial growth factor (VEGF) in asthmatic mice,aiming to provide theoretical guidance for asarone in the treatment of asthma in clinical practice.Methods Thirty female BALB/C mice were randomly divided into three groups using the random number table:the asthma,asarone and control groups (n =10 for each group).In the asthma and asarone groups,the mouse models with asthma were established by ovalbumin (OVA) induction,and physiological saline was utilized in the control group.The mouse in asarone group additionally received aerosol inhalation of asarone.The pathological changes of the lung tissues were observed by H.E.staining.The expression levels of TGF-β1 and VEGF in the bronchoalveolar lavage fluid (BALF) were quantitatively measured by ELISA.The cell counting and classification in the BALF deposit were performed under microscope.Results Compared with the control group,more evident inflammatory cellular infiltration surrounding the airway,bronchial wall thickening and stenosis were noted in the asthma and asarone groups.The degree of these symptoms in the asthma group was more severe than that in the asarone group.In the asarone group,the expression levels of TGF-β1 and VEGF in the BALF were significantly down-regulated than those in the asthma group,whereas considerably up-regulated compared with those in the control group (all P < 0.01).In the asthma group,the total cell count and the quantity of neutrophils,eosinophils,lymphocytes and macrophages were significantly higher compared with those in the control and asarone groups (all P < 0.05).The quantity of neutrophils and eosinophils in the asarone group was evidently higher than that in the control group (both P < 0.01).Conclusions Both TGF-β1 and VEGF are over-expressed in the BALF of mouse models with asthma mice and they probably participate in the airway remodeling during the early stage of asthma.Asarone can down-regulate the expression levels of TGF-β1 and VEGF in asthmatic mice,which possibly acts as one of the mechanism underlying mitigating airway inflammation and easing airway remodeling.

    • 摘要: 目的:观察及分析对于儿童支气管哮喘急性发作实施细辛脑联合多索茶碱静脉注射治疗的效果情况.方法:选择在我院接受治疗的90例支气管哮喘急性发作儿童作为研究对象,以随机分组的方式分成各具备45例的研究组和参照组.其中参照组患儿实施单纯细辛脑注射液治疗,而研究组的患儿实施细辛脑注射液联合多索茶碱静脉注射方式治疗,对比两组患儿的治疗效果情况.结果:通过观察两组患儿的总体治疗有效率,结果显示,相较于参照组而言,研究组患儿的总有效率更高,P<0.05;而且研究组患儿的临床症状消失时间、住院时间均显著的短于参照组,P<0.05.结论:对于支气管哮喘急性发作儿童实施细辛脑联合多索茶碱静脉注射治疗举措,可以得到更好的治疗成效.

    • 摘要: Objective(1) To investigate the changes of vascular endothelial growth factor (VEGF) in asthma mice. (2) To study the regulation role of asarone on expression of VEGF.Methods Twenty- four female BALB/c mice were randomly divided into three groups on average, the control group, the asthma model group, the asarone group. The mice model of asthma was established by the ovalbumin (OVA) challenge methods. VEGF levels in bronchoalveolar lavage fluid (BALF) and serum were measured by enzyme-linked immunosorbent assay (ELISA), the expression of VEGF in the lung was detected by Western blot, the immunohistochemisty was used to evaluate the pathological changes of bronchial and lung tissue and to analyze the expression of VEGF in the lung.Results The ELISA results showed that compared with the group of control there were statistic differences in VEGF content in BLAF and serum level of asthma group, and it was lower in the asarone group than that in the asthma group (P<0.05). The expression of VEGF in the asthma group was higher than that in the control group (P<0.05), the expression of VEGF in the asarone group was lower than that in the asthma group (P<0.05). Immunohistochemical results showed that the VEGF were positive expression in the majority of the alveolar epithelial cells, bronchial smooth muscle and around blood vessels in group B. There was a certain mitigation in group C but none in group A.Conclusions VEGF was highly expressed in the tissue of asthmatic mice and maybe participated in the process of airway remodeling. The asarone can improve the airway remodeling to some extent.%目的 (1)观察哮喘小鼠肺组织中血管内皮生长因子(VEGF)的表达变化;(2)研究细辛脑干预对VEGF表达的影响.方法 24只BALB/c雌性小鼠随机分为对照组(A组)、哮喘模型组(B组)、细辛脑干预组(C组),每组各8只.建立小鼠哮喘模型,酶联免疫吸附测定法(ELISA)对各组小鼠支气管肺泡灌洗液(BALF)和血清中VEGF进行定量分析;Western blot检测VEGF表达;免疫组化法检测小鼠肺组织中VEGF的表达.结果 ELISA法测定结果显示:与对照相比,哮喘组的VEGF在血清和BALF中表达量明显升高,差异具有统计学意义(P<0.05);细辛脑干预组的VEGF含量比哮喘组降低(P<0.05).Western blot检测哮喘组VEGF表达量相比于对照组升高(P<0.05),细辛脑组小鼠VEGF表达量相对于哮喘组降低(P<0.05).免疫组化结果显示哮喘组大部分肺泡上皮细胞、支气管平滑肌和血管周围VEGF均呈阳性表达,细辛脑组有一定程度的减轻,对照组几乎没有.结论 VEGF在哮喘小鼠的肺组织内高度表达,可能参与气道重塑过程,细辛脑在一定程度上可改善气道重塑的病理生理过程.

    • 摘要: 目的 制备细辛脑干粉吸入剂以提高细辛脑的稳定性,解决现有细辛脑制剂的生物利用度低及不良反应多等问题.方法 采用易于工业化生产的气流粉碎法,通过马尔文激光粒径仪和新一代药用粒子撞击器深入考察工艺因素、不同药物-辅料比及药物-载体比对其吸入性能的影响(如细颗粒比例、体外沉积率和排空率等).结果 细辛脑与1%的硬脂酸镁混合均匀后气流粉碎,得到的微粉再与大粒径乳糖载体以1∶5(w/w)混合后制得的细辛脑干粉吸入剂肺部有效沉积率可达25.24%,排空率达90%以上,且无晶型变化.结论 细辛脑干粉吸入剂制备工艺简单,操作可控,易于工业化生产,吸入性能优良,解决了上市制剂稳定性差的问题.

    • 摘要: 目的:探讨细辛脑注射液治疗支气管哮喘的临床治疗效果.方法:选取2015年4月—2016年4月我院收治的支气管哮喘患者76例作为观察对象,根据治疗方案的不同随机将患者分为对照组(n=38例)和观察组(n=38例),对照组患者采用常规治疗方案,观察组在对照组基础上增加细辛脑注射液治疗,对比分析不同治疗方案的临床治疗效果.结果:观察组临床治疗总有效率明显高于对照组(P<0.05);两组患者治疗后FEV1、PaO2、PEF均优于治疗前(P<0.05),组间治疗后对比,观察组FEV1、PaO2、PEF指标改善情况优于对照组(P<0.05).结论:支气管哮喘患者在常规治疗方案的基础上增加细辛脑注射液治疗,临床治疗效果显著,具有一定临床运用价值.
