    • 摘要: 目的探讨链霉蛋白酶联合西甲硅油在微探头超声胃镜(MPS)检查中的应用价值。方法选取2021年1-6月于我院消化内镜中心行MPS检查的患者80例,随机分为试验组和对照组,每组40例,试验组患者于检查前20 min口服链霉蛋白酶联合西甲硅油溶液(链霉蛋白酶20000 U、50 g/L碳酸氢钠30 mL、西甲硅油乳剂5 mL溶于65 mL温水),对照组患者检查前20 min口服100 mL温水,对两组患者MPS检查时胃内清洁度、视野清晰度及内镜操作时间进行比较。结果链霉蛋白酶与西甲硅油联合应用可显著提高胃内清洁度总评分和视野清晰度评分(Z=-4.924、-3.542,P<0.05),缩短MPS操作时间(t=3.563,P<0.05)。结论链霉蛋白酶联合西甲硅油能改善MPS检查时的视野清晰度,同时提高胃内清洁度、缩短超声胃镜操作时间,值得在临床推广应用。

    • 摘要: 目的 探讨视网膜脱离术后硅油填充眼复发性视网膜脱离行堤坝式光凝术后取出硅油的临床效果.方法 回顾性分析郑州大学第一附属医院2018年9月至2020年6月行玻璃体切除硅油填充术后出现下方复发性视网膜脱离者41例(41只眼)的临床资料.所有患者在硅油取出术前于视网膜下积液区域后缘行堤坝式激光光凝术1~3次,末次光凝术后1周行硅油取出术.随访3个月,观察术后视力及视网膜情况.结果 硅油取出术后1个月1只眼视网膜下积液消失,视网膜复位;所有患者硅油取出术后3个月视网膜脱离范围均未扩大,2只眼视网膜下积液消失,视网膜复位.硅油取出术后1个月视力(BCVA)无改变(t=1.846,P=0.075),硅油取出术后3个月视力明显优于术前(t=5.964,P<0.001).结论 硅油填充术后复发性视网膜脱离先行堤坝式光凝术后再进行硅油取出能稳定视网膜脱离范围,效果可靠.

    • 摘要: 目的 观察增生型糖尿病性视网膜病变硅油取出术后短期内黄斑区灌注参数的变化.方法 回顾性分析同济大学附属同济医院2018年1至12月增生型糖尿病性视网膜病变进行玻璃体切除硅油填充术32例(32只眼)的临床资料.所有患者均在玻璃体视网膜手术后6个月行硅油取出术,并在硅油取出术前后行光学相干断层扫描血管成像(OCTA)检查,观察视网膜浅层、深层血管密度、黄斑中心区无血管区面积及视力.结果 术前1周、术后1周、1个月及3个月视网膜浅层血管密度分别为(31.41±3.20)%、(35.31±3.45)%、(36.64±3.38)%及(37.11±2.24)%;视网膜深层血管密度分别为(23.67±3.10)%、(27.10±4.13)%、(28.09±4.32)%及(28.64±3.88)%;黄斑中心区无血管区面积分别为(0.39±0.04)mm2、(0.35±0.14)mm2、(0.33±0.73)mm2及(0.34±0.25)mm2.所有患者视网膜浅层血管密度、视网膜深层血管密度及黄斑中心区无血管区面积术后各时间点与术前相比,差异均有统计学意义(P>0.05).术后3个月视力(BCVA,logMAR)1.18±0.65,与术前的0.86±0.57相比,差异无统计学意义(P>0.05).结论 硅油取出术后短期内,增生型糖尿病性视网膜病变患者的视网膜黄斑区血管密度逐渐增加、无血管区面积逐渐缩小,但并非影响术后视力的决定性因素.

    • 摘要: 探讨二甲硅油联合复方聚乙二醇清洁肠道对肠息肉检出率的影响.选取2020年5月-7月间完成肠镜检查的患者300例,按随机分组原则将患者分为2组,聚乙二醇组患者仅采用复方聚乙二醇电解质散2包进行清肠;联合组患者检查前选用二甲硅油联合复方聚乙二醇进行清肠,其中复方聚乙二醇服用时间、方法、用量均较前一致.分别对比2组肠道清洁程度、肠道去泡程度、肠息肉检出率及不良反应等情况.结果显示联合组肠道清洁评分、肠道去泡率及不同大小息肉检出率均高于聚乙二醇组,2组患者短期不良反应发生率差异无统计学意义,但联合组术后腹胀发生率明显降低.可见,二甲硅油联合复方聚乙二醇清洁肠道可明显提高肠道清洁程度及改善术后腹胀,对于提高肠道息肉检出以致预防部分肠道肿瘤的发生有明显成效,方法简单,经济负担小,值得推广.

    • 摘要: 目的 观察硅油填充眼黄斑中心凹脉络膜厚度(SFCT)变化及屈光状态对其的影响.方法 临床自身对照研究.2019年1~7月于解放军中部战区总医院眼科检查确诊并行玻璃体切割手术联合硅油填充的孔源性视网膜脱离患者50例50只眼及对侧眼50只纳入研究.所有患眼手术后视网膜复位.手术后1个月,患眼行医学验光,根据检查结果订制软性角膜接触镜,并佩戴24h.手术前及手术后1个月采用频域OCT测量受检眼SFCT.观察硅油眼屈光矫正前后的SFCT变化.硅油眼与对侧眼SFCT比较采用t检验.结果 手术后1个月,硅油眼平均屈光度数为(+6.38±1.12)D.硅油眼、对侧眼平均SFCT分别为(247.12±17.63)、(276.32.55±17.63) μun;硅油SFCT较对侧眼明显变薄,差异有统计学意义(t=-3.95,P<0.001).硅油眼屈光矫正后平均SFCT为(276.32±24.86) μm;与屈光矫正前比较,差异有统计学意义(t=-4.30,P<0.001);与对侧眼比较,差异无统计学意义(t=0.05,P>0.05).结论 硅油眼SFCT较正常眼明显变薄;屈光矫正后可恢复.

    • 摘要: 目的 观察玻璃体切除硅油填充术后早期高眼压对增生性糖尿病性视网膜病变(PDR)患者角膜内皮细胞的影响.方法 回顾性病例对照研究.大连大学附属中山医院2018年1月至2019年3月玻璃体切除硅油填充术60例(60只眼),其中术后发生早期高眼压者30例(30只眼,A组),术后眼压正常者30例(30只眼,B组).两组进行对比分析.结果 术前两组眼压、角膜内皮细胞密度、细胞多形性变异率及六边形所占比例各参数差异无统计学意义(均P>0.05).术后两组眼压不同时间点总体差异有统计学意义(F=6.252,P=0.036),其中术后3天、1周及2周两组间差异有统计学意义(P0.05).结论 PDR患者玻璃体切除硅油填充术后早期高眼压影响其角膜内皮细胞密度、多形性变异率及六边形细胞所占比例.

    • 摘要: 目的 探讨不同玻璃体腔填充物对玻璃体切除术后20%甘露醇降眼压作用的影响.方法 回顾性病例对照研究.收集2016年1月至2017年1月于内蒙古科技大学包头医学院附属第一医院行玻璃体切除术的玻璃体出血患者,将术后高眼压的68例(72只眼)患者纳入研究,其中男性24例(26只眼),女性44例(46只眼);年龄(45.6±19.3)岁.根据术中不同玻璃体腔填充物分为硅油组23只眼、全氟丙烷(C3F8)组30只眼及平衡盐溶液(BBS)组19只眼.观察患者静脉滴注20%甘露醇前后不同时间点的眼压变化情况.采用t检验和方差分析与组间q检验进行统计学分析.结果 3个组患者用药后眼压均有明显下降,硅油组眼压在用药后75 min内下降达到谷值,从用药前(33.25±2.56)mmHg(1 mmHg=0.133 kPa)降至(23.21±1.85)mmHg,最大降幅达30.10%;B组在用药后75 min眼压下降到谷值,从(33.25±2.84)mmHg降至(12.15±1.12)mmHg,下降幅度为33.44%;C组在用药后45 min便降至最低,从(32.95±2.33)mmHg降至(17.50±1.35)mmHg,下降幅度为45.82%.3个组眼压差值在20、30、45、60 min经方差分析显示总体差异均有统计学意义(F=34.02,112.68,122.07,34.83;P=0.00),组间比较:平衡盐溶液组与硅油组对比差异有统计学意义(q=6.44,13.04,15.00,17.11;P=0.00);平衡盐溶液组与全氟丙烷组对比差异有统计学意义(q=7.68,12.56,12.93,13.61;P=0.00).结论 20%甘露醇对不同玻璃体腔填充物的玻璃体切除术后高眼压患者均有明显降眼压作用.用药后1h内玻璃体腔填充BSS患者的眼压下降最为明显和迅速;用药后75 min眼内填充BSS与硅油和惰性气体比较,眼压下降幅度无明显差异.%Objective To investigate the efficacy of 20% mannitol in reducing intraocular pressure(IOP)in eyes with different intraocular tamponades after pars plana vitrectomy (PPV).Methods Retrospective case-control study.Sixty-eight patients were administered with 20% mannitol and IOP was noted at regular intervals after simple PPV with ocular hypertension,including 24 males (26 eyes) and 44 females (46 eyes),aged (45.6± 19.3) years.These cases were divided into three groups according to different tamponades:silicon-oil tamponade,23 eyes;gas tamponade,30 eyes;balanced salt solution (BSS),19 eyes.The data were analyzed using the t test,variance and q test.Results There was a significant decrease in IOP in all patients after using 20% mannitol.The IOP in the group of silicon-oil decreased from (33.25± 2.56) mmHg(1 mmHg=0.133 kPa) to (23.21 ± 1.85) mmHg,with a maximum decrease of 30.10%;the reduction in the group of C3Fs was from (33.25±2.84) mmHg to (12.15± 1.12) mmHg,with a maximum decrease of 33.44%.The IOP of the two groups dropped to a minimum both at 75 minutes.In the group of BSS,the IOP decreased from (32.95±2.33) mmHg to (17.50± 1.35) mmHg,and the maximum extent of the decrease was 45.82% at 45 minutes.The difference in the IOP among the three groups at 20 min,30 min,45 min and 60 min was statistically significant (F=34.02,112.68,122.07,34.83,all P=0.00).There were significant differences between the BSS group and the silicone-oil group (q=6.44,13.04,15.00,17.11,all P=0.00),and between the BSS group and the C3F8 group (q=7.68,12.56,12.93,13.61,all P=0.00).Conclusion In eyes with different intraocular tamponades,20% mannitol was useful for short-term IOP reduction after vitrectomy,especially in those with BSS within one hour.But after 75 minutes,there was no statistically significant difference between groups.

    • 摘要: 目的 探讨硅油填充眼视网膜脱离的超声诊断特点及诊断价值.方法 选取2015年12月1日至2016年8月1日在北京同仁医院眼科病房住院并拟行手术的硅油填充眼患者257例(257只眼)为研究对象,所有患者均于术前一周内分别在仰卧位及坐位下行眼超声检查,将检查结果与术中所见进行对比,分析两种体位下硅油填充眼视网膜脱离的超声诊断特点及价值.结果 硅油填充眼视网膜脱离表现为眼轴假性延长、玻璃体腔内见与球壁相连的条带状回声.仰卧位下超声检查的诊断准确率为85.6%,坐位下超声检查的诊断准确率为93.8%.结论 硅油填充眼视网膜脱离的超声检查具有一定特征性表现.相比于传统的仰卧位检查,坐位超声检查可进一步提高硅油填充眼视网膜脱离的超声诊断准确率,为临床诊断及鉴别诊断提供可靠依据.%Objective To examine the features and values of ultrasonic diagnosis for retinal detachment in eyes with silicone oil tamponade (SOT). Methods A prospective study. A total of 257 eyes (257 patients) were filled with silicone oil between December 1, 2015 and August 1, 2016 in the Department of Ophthalmology at a hospital in Beijing. All of the patients underwent a color Doppler ultrasound examination in the supine and sitting positions a week prior to surgery. The pre-surgery conditions were compared with the subsequent observations during the surgery to analyze the features and values of ultrasonic diagnosis for retinal detachment in eyes with SOT. Results Ultrasound manifestations of eyes with SOT included pseudo extension of the ocular axis and an arc-shaped echo between the eye wall and silicone oil. Diagnostic accuracy was 85.6% in the supine position and 93.8% in the sitting position. Conclusion Ultrasound diagnosis for retinal detachment in eyes with SOT manifests itself in certain patterns. Compared with the traditional ultrasound examination in the supine position, the sitting position examination can be used to further increase the diagnostic accuracy in the ultrasound diagnosis for retinal detachment in eyes with SOT, providing reliable evidence for clinical and differential diagnosis.

    • 摘要: 目的 对比观察硅油填充眼眼轴长度(AL)浸润式B型超声引导下分段声速测量与光学激光生物测量仪(Lenstar LS900)测量结果,探讨浸润式B型超声引导下分段声速测量对于硅油填充眼AL测量的准确性.方法 前瞻性研究.拟行硅油取出、白内障摘除及人工晶状体植入手术的35例患者38只眼纳入研究.所有患眼均合并不同程度的白内障.手术前采用MD-2300S型眼科A型和(或)B型超声行浸润式B型超声引导下分段声速测量法测量患眼AL.AL为角膜厚度、前房深度、晶状体厚度、硅油泡的表观长度、硅油泡下水层厚度的总和.同时采用LenstarLS900测量患眼AL.所有患眼均顺利完成硅油取出、白内障摘除及人工晶状体植入手术,随诊至手术后3个月均未见并发症发生.手术后3个月,重复应用Lenstar LS900测量患眼AL.对比观察两种测量方法测得AL数据的差异及一致性.结果 38只眼中,手术前经Lenstar LS900测得AL31只眼;因晶状体混浊、AL超过LenstarLS900测量范围未能测得AL7只眼.31只眼的平均AL为(24.12±1.70)nm.手术后3个月经Lenstar LS900测得AL36只眼;AL超过Lenstar LS900测量范围未能测得AL 2只眼.36只眼的平均AL为(24.45±1.89)mm.患眼手术前后AL差值为(0.02±0.07)mm,差异无统计学意义(t=1.752,P>0.05).手术前所有患眼均经浸润式B型超声引导下分段声速测量法测得AL.患眼平均AL为(24.87±2.52)mm.与手术前Lenstar LS900测得AL比较,两者差值为(-0.00±0.09)mm,差异无统计学意义(t=-0.205,P>0.05);与手术后3个月Lenstar LS900测得AL比较,两者差值为(-0.02±0.11)mm,差异无统计学意义(t=-1.280,P>0.05).两种测量方法测量硅油填充眼的AL具有良好的一致性((d)=-0.003,95%可信区间为-0.19~0.18,P<0.05).结论 浸润式B型超声引导下分段声速测量与Lenstar LS900测量对硅油填充眼AL测量结果一致性良好;浸润式B型超声引导下分段声速测量可对硅油填充眼AL进行准确测量.%Objective To evaluate the accuracy of the biometry using immersion B scan and partial coherence interferometry (Lenstar LS900) for the axial length (AL) of silicone oil-filled eyes respectively.Methods Thirty-five silicone oil-filled eyes (38 patients) were included in the study.All of these eyes underwent silicone oil removal,cataract extraction and intraocular lenses implantation.The AL of all the silicone oil-filled eyes was measured with A/B-scan ultrasound and Lenstar LS900 before operation and with Lenstar LS900 after operation.The measured distance was compared respectively.The method of immersion B-scan guided with respective sonic velocity.AL was the sum of corneal thickness,anterior chamber depth,lens thickness,the apparent length of oil bubble (velocity values 996 m/s),the depth of the water layer beneath the oil bubble.Results Thirty-one eyes were measured with Lenstar LS900 before silicone oil removal,and the mean AL was (24.12± 1.70) mm,7 eyes failed to get the results before the operation;36 eyes were measured with Lenstar LS900 after silicone oil removal,and the mean AL was (24.45±1.89) mm.All eyes were measured with B-scan before silicone oil removal,and the mean AL was (24.87±2.52) mm.The difference (31 eyes) of AL measurement before silicone oil removal by two methods was (-0.00±0.09) mm;the difference (31 eyes) between pre-and post-surgical AL measurement with Lenstar LS900 was (0.02±0.07) mm;the difference (36 eyes) between pre-surgical AL measured with B-scan and post-surgical AL measured with Lenstar LS900 was (-0.02±0.11) mm.All the differences were not statistically significant (t=-0.205,1.752,-1.280;P> 0.05).The consistency of the results measured by two methods was well in Bland-Ahamn analysis.Conclusions Measurement results of AL between immersion B-scan guided with respective sonic velocity and Lenstar LS900 are high repeatability on silicone oil-filled eyes.The AL of silicone oil-filled eyes can be measured reliably by immersion B-scan guided with respective sonic velocity.
