BACKGROUND Coronary artery perforation is a rare but potentially life-threatening complication of percutaneous coronary intervention(PCI),however if recognized and managed promptly,its adverse consequences can be minimized.Risk factors include the use of advanced PCI technique(such as atherectomy and chronic total occlusion interventions)and treatment of severely calcified lesions.Large vessel perforation is usually treated with implantation of a covered stent,whereas distal and collateral vessel perforations are usually treated with embolization of coils,fat,thrombin,or collagen.We describe a novel and cost-effective method of embolisation using a cut remnant of a used angioplasty balloon that was successful in sealing a distal wire perforation.we advocate this method as a simple method of managing distal vessel perforation.CASE SUMMARY A 73-year-old male with previous coronary Bypass graft operation and recurrent angina on minimal exertion had undergone rotablation and PCI to his dominant left circumflex.At the end of the procedure there was evidence of wire perforation at the distal branch and despite prolonged balloon tamponade there continued to be extravasation and the decision was made to seal this perforation.A cut piece of an angioplasty balloon was used and delivered on the original angioplasty wire to before the perforation area and released which resulted in sealing of the perforation with no unwanted clinical consequences.CONCLUSION The use of a balloon remnant for embolization in coronary perforation presents a simple,efficient and cost-effective method for managing coronary perforations and may be an alternative for achieving hemostasis and preventing poor outcome.Prevention remains the most important part with meticulous attention to the distal wire position,particularly with hydrophilic wires.

本研究基于前期基因组水平挖掘的3个产羔数候选基因突变位点,利用KASP分型技术,对由不同产羔数组成的2个绵羊群体阿勒泰羊、吐鲁番黑羊进行检测,并与产羔数进行相关性分析.结果表明,3个突变位点GUCY 1A 1基因g.43266624C>T、COIL基因g.7321466C>G、COIL基因g.7321563G>A在阿勒泰羊中均具有多态性,在吐鲁番黑羊中仅COIL基因g.7321563G>A位点无多态性,两个品种间的基因型频率和基因频率差异不显著(P>0.05).GUCY 1A 1基因g.43266624C>T位点在阿勒泰羊中的CC、CT基因型个体产羔数极显著高于TT基因型个体(P 0.01),在吐鲁番黑羊中的CT基因型个体产羔数显著高于TT基因型个体(P 0.05),其他2个突变位点均差异不显著(P>0.05).GUCY 1A 1基因g.43266624C>T位点与绵羊产羔数密切相关,可望作为显著影响阿勒泰羊、吐鲁番黑羊产羔数的重要位点.

Background:Symptomatic vertebral artery dissecting aneurysm(VADA)is a challenging disease with controversy on treatment strategy due to anatomic configuration and their nature.Moreover,the outcomes of reconstructive treatment have not been well established.Objective:To evaluate the safety and efficacy of reconstructive endovascular treatment(EVT)for symptomatic VADAs with Willis covered stent.Methods:We evaluated retrospectively 13 patients with symptomatic VADAs who treated with Willis covered stent,compared with stent-assisted coiling(SAC)on the characteristics,posttreatment course,angiographic and clinical follow-up outcomes at an average of 14.4 months(range,3-48 months).Results:A total of 33 patients with symptomatic VADAs were reviewed,23 of these patients with ruptured VADAs.The technical successful rate is 100%respectively in Willis covered stent(Group A)and SAC(Group B,n=20).The initial complete occlusion rate was significant higher in group A(100%)than group B(30%)(p0.05).No obvious in-stent stenosis and no re-hemorrhage and delayed ischemic symptoms during the follow-up period.The final angiograms of all survived patients demonstrated the complete occlusion rate was higher in group A(100%)than group B(80%),but no significant statistical difference(p>0.05).Clinical outcomes were favorable in 31(93.9%),severe disability occurred in one in group B,and only one death in group A.The final clinical outcomes were also not significant difference in the two groups(p>0.05).Conclusions:Our initial result demonstrated reconstructive EVT with Willis covered stent provides a viable approach for selected symptomatic VADAs involving the intracranial and extracranial segments,which is similar to favorable results with SAC.However,an expanded clinical experiences and larger cohort studies are needed.

Protein Secondary Structure Prediction (PSSP) is considered as one of the major challenging tasks in bioinformatics, so many solutions have been proposed to solve that problem via trying to achieve more accurate prediction results. The goal of this paper is to develop and implement an intelligent based system to predict secondary structure of a protein from its primary amino acid sequence by using five models of Neural Network (NN). These models are Feed Forward Neural Network (FNN), Learning Vector Quantization (LVQ), Probabilistic Neural Network (PNN), Convolutional Neural Network (CNN), and CNN Fine Tuning for PSSP. To evaluate our approaches two datasets have been used. The first one contains 114 protein samples, and the second one contains 1845 protein samples.

A 43-year-old male presented with severe intermittent headaches since two years accompanied by multiple episodes of generalized seizures and left hand weakness. MRI scan and cerebral angiography confirmed the diagnosis of a giant serpentine cerebral aneurysm arising from the middle cerebral artery. The patient was admitted for a possible cerebral embolization after undergoing a Balloon Occlusion Test (BOT). Since he tolerated the occlusion test, he underwent coil embolization afterwards with successful occlusion followed by an uneventful recovery and no complications. Later on, the patient was discharged home in a stable and good general condition without any neurological deficits. A six-month angiography follow-up revealed a complete occlusion with no re-canalization of the coiled giant serpentine aneurysm. Therefore, we concluded that parent artery occlusion is a good and safe procedure providing that the patient tolerated both the occlusion and the hemodynamic stress tests.

Aneurysm at the origin of a duplication of the middle cerebral artery (DMCA) is very rare, and only 29 treated cases have been reported. All of the cases were treated by direct surgery except a ruptured case treated by intentional partial coil embolization. We report the first unruptured case treated by coil embolization and review the previously published cases. Coil embolization can be alternative treatment for an unruptured aneurysm at the origin of the DMCA. Stable framing to spare the origin of it and prevention of thromboembolic complications are keys for safe treatment.

Efficient Air Conditioning (A/C) system is the key to reducing energy consumption in building operation. In order to decrease the energy consumption in an A/C system, a method to calculate the optimal tube row number of a direct expansion (DX) cooling coil for minimizing the entropy generation in the DX cooling which functioned as evaporator in the A/C system was developed. The optimal tube row numbers were determined based on the entropy generation minimization (EGM) approach. Parametric studies were conducted to demonstrate the application of the analytical calculation method. Optimal tube row number for different air mass flow rates, inlet air temperatures and sensible cooling loads were investigated. It was found that the optimal tube row number of a DX cooling coil was in the range of 5 - 9 under normal operating conditions. The optimal tube row number was less when the mass flow rate and inlet air temperature were increased. The tube row number increased when the sensible cooling load was increased. The exergy loss when using a non-optimal and optimal tube row numbers was compared to show the advantage of using the optimal tube row number. The decrease of exery loss ranged from around 24% to 70%. Therefore the new analytical method developed in this paper offers a good practice guide for the design of DX cooling coils for energy conservation.

本文对二维COIL扩压器流场和三维超声速引射器进行了初步数值模拟,结果表明本文计算方法和研究方案是可行的.对于COIL扩压器流场,在组分和混合物输运特性计算、化学反应源项计算、边界条件处理、内流的网格分布等各方面还需要作进一步的研究.为了更加真实模拟COIL流动,还需要考虑水蒸气的冷凝,光传播方程的耦合计算,以及将计算推广到三维.对于引射器流动,由于超声速引射器流场中存在超声速剪切、湍流/激波和边界层的相互作用、复杂的多波系结构和混合现象,要准确模拟各参数影响,还需进一步选择和发展数值计算方法和湍流模型。
