    • 摘要: 目的 探讨血清新喋呤、血管内皮生长因子B(vascular endothelial growth factor B,VEGF-B)与新诊断2型糖尿病(type 2 diabetes mellitus,T2DM)患者周围神经病变的相关性。方法 选取2018年6月-2020年5月衡水哈励逊国际和平医院住院治疗的103例新诊断T2DM患者,根据有无糖尿病周围神经病变(diabetic peripheral neurophathy,DPN)分为DPN组(38例)和非DPN组(65例)。应用美国Bio-Tek ELX800多功能酶标仪检测血清新喋呤和VEGF-B水平。绘制受试者工作特征曲线(receiver operating characteristic curve,ROC)并计算曲线下面积(area under the curve,AUC)分析血清新喋呤、VEGF-B对新诊断2型糖尿病患者周围神经病变的预测价值,多因素logistic回归分析探讨新诊断2型糖尿病患者周围神经病变的相关因素。结果 DPN组患者甘油三酯(Triglyceride,TG)水平低于非DPN组,糖化血红蛋白(glycated hemoglobin,HbAlc)、空腹血糖(fasting blood glucose,FPG)水平高于非DPN组(均P<0.05)。DPN组血清新喋呤、VEGF-B水平明显高于非DPN组(P<0.05)。新喋呤、VEGF-B与TG呈负相关(r=-0.461、-0.478,均P<0.05)。新喋呤、VEGF-B与HbAlc、FPG呈正相关(r=0.541、0.502、0.627、0.593,均P<0.05)。血清新喋呤、VEGF-B预测发生DPN的ROC曲线下面积(AUC)分别为0.803(95%CI:0.727-0.874)、0.789(95%CI:0.711-0.858)。在23.5nmol/L的临界值下,新喋呤预测的灵敏度为80.5%,特异度为83.3%;在170.0pg/ml的临界值下,VEGF-B预测的灵敏度为69.4%,特异度为86.8%。新喋呤和VEGF-B联合预测灵敏度为89.6%,特异度为80.1%。多因素logistics回归分析显示,新喋呤≥23.5nmol/L、VEGF-B≥170.0pg/ml是T2DM患者发生DPN独立危险因素,其OR及95%CI分别为1.618(1.126-2.325)、1.441(1.154-1.797)(均P<0.05)。结论 新诊断2型糖尿病并发周围神经病变患者血清新蝶呤、VEGF-B升高,新蝶呤、VEGF-B是新诊断2型糖尿病患者发生周围神经病变的独立预测因素,二者有望作为新诊断2型糖尿病并发周围神经病变的生物标志物。

    • 摘要: 目的:探讨新喋呤在普外科脓毒症患者中的表达水平及其与临床预后的关系.方法:选择174例感染患者为研究对象,按是否患脓毒症分为脓毒症组(n=73)和非脓毒症组(n=101).比较两组患者血清新喋呤水平、血常规及生化指标,分析各指标与临床预后的关系.结果:与非脓毒症组比较,脓毒症组WBC、NEU、血糖、ALT、CRP、PCT、Crea及新喋呤水平增高(P<0.05).脓毒症组血清新喋呤水平与WBC、NEU、Crea、CRP、PCT和SOFA评分呈正相关(P<0.05).脓毒症组以血清新喋呤水平≥83.1 nmol/L为诊断界值,预测脓毒症30 d内死亡的ROC曲线下AUC为0.688(P=0.023).回归分析显示,年龄≥60岁(r=2.236,P=0.038)、新喋呤≥83.1 nmol/L(r=3.464,P=0.017)及SOFA≥3(r=3.793,P=0.014)是脓毒症30 d内死亡的独立危险因素.结论:普外科感染患者中,血清新呤水平在脓毒症患者中增高,且与炎症反应、肾脏受损及临床病情严重程度呈正相关,≥83.1 nmol/L的脓毒症患者住院30 d死亡率增加.

    • 摘要: 目的 观察射血分数保留的心力衰竭(HFpEF)患者血清新喋呤的水平,探讨血清新喋呤对HFpEF的辅助诊断意义.方法 连续入选2016年10月~2017年8月于大连大学附属中山医院心脏中心住院心功能(NYHA分级)Ⅱ~Ⅳ级HFpEF患者共90例(HEpEF组),另选取同期入院基本临床资料相匹配的非心力衰竭患者80例作为对照组(非HFpEF组).采集入选者临床资料并分析血清新喋呤与N末端脑钠肽前体(NT-proBNP)、心脏超声有关参数的相关性,采用受试者工作特征曲线(ROC)评估血清新喋呤对HFpEF的辅助诊断意义.结果 HFpEF组中血清新喋呤、NT-proBNP、肌酐水平均高于非HFpEF组(P<0.05),随着NYHA心功能分级的增加,NT-proBNP与血清新喋呤逐渐升高(P<0.05);Spearman相关性分析显示,新喋呤与NT-proBNP、 E/E'、左房内径(LAD)、室间隔厚度(IVSD)呈显著正相关,而与LVEF、LVDD未见明显相关性;血清新喋呤诊断HFpEF的ROC曲线下面积(AUC)为 0.720 (95%CI:0.600~0.830 ,P<0.05),NT-proBNP为0.928(95%CI:0.863~0.994,P<0.05),二者联合为0.932(95%CI:0.866~0.998,P<0.05).结论 HFpEF患者血清新喋呤明显增高,且与心力衰竭的严重程度相关.%Objective To observe the level of serum neopterin in patients with heart failure with ejection fraction (HFpEF), and to investigate the significance of serum neopterin in the diagnosis of HFpEF. Methods This study recruited 90 patients with HFpEF (HFpEF group) hospitalized in Cardiology center of Zhongshan Hospital Affiliated to Dalian University from October 2016 to August 2017. We divided patients into three groups according to NYHA heart function classification criterion: NYHA Class Ⅱ group (n=37), NYHA ClassⅢ group (n=34), NYHA Class Ⅳ group (n=19). In addition, 80 patients without heart failure matched with similar clinical characteristics were selected as control group (non HFpEF group). We collected the clinical data of patients and measured the plasma NT-proBNP level. The level of serum neopterin was detected by enzyme linked immunosorbent assay. The correlation between serum neopterin and NT-proBNP, the parameters of cardiac ultrasound were analyzed. The receiver operating characteristic curve (ROC) was used to evaluate the significance of serum neopterin in the diagnosis of HFpEF. Results The level of serum neopterin, NT-proBNP and creatinine in the HFpEF group were higher than those in the non HFpEF group (P<0.05). With the increase of NYHA cardiac function classification, the concentration of NT-proBNP and serum neopterin increased gradually (P<0.05). The Spearman correlation analysis showed that serum neopterin was significant positively correlated with NT-proBNP, E/E', LAD, IVSD, and was not correlated with LVEF, LVDD. The area under the ROC curves of serum neopterin diagnosis for HFpEF was 0.720 (P<0.05, 95%CI: 0.600~0.830), and NT-proBNP for HFpEF diagnosis was 0.928 for AUC (P<0.05, 95%CI: 0.863~0.994), and an AUC of 0.932 (P<0.05, 95%CI: 0.866~0.998) for combination of both biomarkers. Conclusion Serum neopterin levels were significantly higher in patients with HFpEF, and were related to the severity of heart failure. Therefore, serum neopterin may become a new biochemical marker for the diagnosis of HFpEF, and united NT-proBNP can improve the diagnostic efficacy of HFpEF.

    • 摘要: 目的 观察急性缺血性脑卒中患者血清过氧化还原蛋白1(PRDX1)、半乳糖凝集素3(GAL3)、新喋呤(Npt)水平变化,并探讨其意义.方法 急性缺血性脑卒中患者90例(观察组,按照NIHSS评分分为轻症者28例、中症者40例、重症者22例,按mRS评分分为预后良好者48例、预后不良者42例),同期健康体检者90例(对照组).采用ELISA法检测两组血清PRDX1、GAL3、Npt,Pearson相关分析法分析观察组血清PRDX1、GAL3、Npt水平与NIHSS评分、mRS评分的相关性.结果 观察组血清PRDX1、GAL3、Npt水平分别为8.9(4.7 ~14.8)ng/mL、(15.6±6.7) ng/mL、18.4(13.1~23.6) nmol/L,对照组分别为3.6(1.2~5.2) ng/mL、(8.2±2.6) ng/mL、4.5(2.2~7.6)nmoL/L,两组比较,P均<0.05.观察组轻症者血清PRDX1、GAL3、Npt水平分别为6.5(5.6 ~7.9) ng/mL、(11.3 ±3.7)ng/mL、12.3(9.1 ~ 15.8) nmol/L,中症者分别为8.2(7.0~ 10.3)ng/mL、(16.2 ±5.5)ng/mL、18.7(16.4~20.3)nmoL/L,重症者分别为13.3(10.7~16.8) ng/mL、(19.9±6.3)ng/mL、24.5(21.6 ~ 28.2) nmol/L,两两比较,P均<0.05.观察组预后良好者血清PRDX1、GAL3、Npt水平分别为7.3(4.9 ~9.7) ng/mL、(12.7±4.4)ng/mL、13.9(11.6 ~ 16.7) nmol/L,预后不良者分别为10.7(8.7~ 15.6) ng/mL、(18.3±6.0) ng/mL、23.8(18.3~27.1) nmol/L,两者比较,P均<0.05.观察组血清PRDX1、GAL3、Npt水平与NIHSS评分呈正相关(r分别为0.124、0.561、0.403,P均<0.05),亦与mRS评分呈正相关(r分别为0.328、0.456、0.512,P均<0.05).结论 急性缺血性脑卒中患者血清PRDX1、GAL3、Npt水平升高,且与NIHSS评分、mRS评分呈正相关,检测血清PRDX1、GAL3、Npt有助于急性缺血性脑卒中病情严重程度和预后判断.

    • 摘要: 目的 探讨新喋呤(neopterin,Np)在脓毒症早期诊断和危险分层中的应用价值.方法 研究纳入于2013年4月至2014年2月就诊于山西医科大学第二医院急诊科的82例患者并分为两组,分别为非脓毒症组8例、脓毒症组74例.患者入院后24 h内进行急性生理学与慢性健康状况Ⅱ评分(APACHEⅡ评分),并检测血浆Np和降钙素原(procalcitonin,PCT)水平,分析两组患者血浆中Np水平、PCT水平以及两者与APACHE Ⅱ评分的关系;74例脓毒症患者根据APACHEⅡ评分分为三组:Ⅰ组(评分< 15分,27例),Ⅱ组(评分15-24分,32例),Ⅲ组(评分≥25分,15例),比较各组间血浆Np水平、PCT水平差异,将Np水平分别与血浆PCT水平和APACHEⅡ评分进行相关性分析.结果 脓毒症组血浆Np水平、PCT水平及APACHEⅡ评分较非脓毒症组有所升高(P<0.05);脓毒症组血浆中Np水平、PCT水平随病情严重程度提高逐渐升高(P<0.05);脓毒症组血浆中Np水平与PCT水平、APACHE Ⅱ评分均呈正相关(P<0.01);Np的受试者工作特征曲线(ROC)下面积为0.79,PCT的ROC下面积为0.75,两者均具有较高诊断准确性.结论 新喋呤在脓毒症早期诊断和危险分层中有重要应用价值.%Objective To explore the value of neopterin (Np) in early diagnosis and risk stratification of sepsis.Methods A total of 82 patients admitted to the emergency department from April 2013 to February 2014 were enrolled in the study.They were divided into two groups:sepsis-free group (n =8) and sepsis group (n =74).Patients' APACHE Ⅱ scores were calculated within 24 hours after admission,and then their plasma levels of Np and procalcitonin (PCT) were detected and analysis was carried out to find the correlation between plasma levels of Np,PCT and acute physiology and chronic health evaluation Ⅱ score (APACHE Ⅱ score) in two groups.And 74 patients in sepsis group were further divided into three subgroups according to their APACHE Ⅱ score:Ⅰ subgroup (score < 15,n =27),Ⅱ subgroup (score 15 to 24,n =32),and Ⅲ subgroup (score > 25 or more,n =15).The differences in plasma Np and PCT levels were compared among three subgroups and correlation analysis of Np levels with PCT levels and APACHE Ⅱ scores was carried out.Results The plasma levels of Np,PCT and APACHE Ⅱ scores in sepsis group were higher than those in sepsis-free group (P < 0.05).In sepsis group,the plasma levels of Np and PCT were gradually increased with the increasing severity of sepsis (P < 0.05);In sepsis group,the plasma levels of Np,PCT and APACHE Ⅱ scores were positively correlated (P <0.01).The area under receiver operating characteristic curve (ROC) of Np was O.79 and the area under ROC of PCT was 0.75,both of which had high diagnostic accuracy.Conclusions Neopterin has greatly useful value in early diagnosis and risk stratification of sepsis.

    • 摘要: 目的:分析阻塞性睡眠呼吸暂停综合征( OSAHS)患者血清新喋呤( NP)水平与白天嗜睡度的相关性。方法选择2014年10月—2015年5月于河北省唐山市协和医院呼吸科就诊的OSAHS患者80例为OSAHS组,根据呼吸暂停低通气指数( AHI)指标分为轻度亚组20例,中度亚组32例,重度亚组28例。选择同期健康体检者50例为健康对照组。测量2组血清新喋呤水平,并采用多导睡眠仪监测其夜间睡眠,记录 AHI、最低血氧饱和度( LSaO2)以及最长呼吸暂停时间( Tmax),进行Epworth嗜睡度量表( ESS)、躯体疲劳等评分。对OSAHS患者NP水平与白天嗜睡度的相关性进行分析。结果 OSAHS组新喋呤水平较健康对照组明显升高[(7 z.7±0.8) ng/ml vs.(3.3±0.4) ng/ml, P <0.05];ESS评分、AHI、Tmax均明显高于健康对照组[(10.6±3.1)分 vs.(5.3±1.7)分,(36.6±5.5)次/min vs.(1.1±0.3)次/min,(55.2±11.5)s vs.(2.9±0.3)s, P均<0.05],而LSaO2低于健康对照组[(79.6±10.9)%vs.(94.5±3.7)%, P <0.05];躯体疲劳、脑力疲劳均明显高于健康对照组[(7.25±1.93)分vs.(5.10±1.87)分,(5.87±0.29)分vs.(3.53±0.68)分, P <0.05]。 OSAHS患者中,轻度亚组NP水平<中度亚组<重度亚组[分别为(11.23±1.82)ng/ml、(13.35±1.87)ng/ml、(15.50±1.86)ng/ml, F =6.058, P =0.016]。相关性分析提示血清NP水平与BMI、AHI、Tmax、LsaO2、躯体疲劳、脑力疲劳均存在一定的正相关( r =0.383、0.207、0.247、0.289、0.507、0.533, P <0.05)。 ESS评分与AHI、Tmax、LsaO2、躯体疲劳评分和脑力疲劳评分存在一定的正相关(r =0.255、0.435、0.380、0.322、0.385, P <0.05);ROC曲线分析提示血清NP预测诊断OSAHS患者的AUC为0.798,cut-off值为6.9 ng/ml,敏感度为84.4%,特异度为86.1%。结论血清新喋呤可以作为评价OSAHS严重程度和疲劳感加重的参考指标。%Objective To analyse the correlation of obstruction sleep apnea hypopnea syndrome ( OSAHS) patients serum neopterin (NP) and the level of daytime sleepiness.Methods From October 2014 toMay 2015,80 patients with OS-AHS in Tangshan City Union Hospital, Hebei Province were enrolled as OSAHS group, according to apnea hypopnea index (AHI) index, they were divided into mild subgroup (20 cases), moderate subgroup (32 cases), severe subgroup (28 ca-ses).Fifty cases of healthy persons were selected as healthy control group.Measured two groups’ serum neopterin levels, and used of polysomnography for monitoring the night sleep, recording AHI, the lowest oxygen saturation ( SaO2 ) and the longest respiratory pause time (Tmax), the Epworth sleepiness metrics score (ESS), physical fatigue.The correlation between NP levels and daytime sleepiness in patients with OSAHS was analyzed.Results The OSAHS group’ s new neopterin levels com-pared with the healthy control group was significantly increased [(7.7 ±0.8) ng/ml vs.(3.3 ±0.4) ng/ml, P <0.05];the ESS score and AHI, Tmax were significantly higher than those in healthy control group [(10.6 ±3.1) vs.(5.3 ±1.7), (36.6 ±5.5) times/min vs.(1.1 ±0.3) times/min, (55.2 ±11.5) s vs.(2.9 ±0.3) s, P <0.05], and LSaO2 was lower than healthy group [(79.6 ±10.9)%vs.(94.5 ±3.7)%, P <0.05];physical fatigue, mental fatigue were signifi-cantly higher than those in healthy control group [(7.25 ±1.93) points vs.(5.10 ±1.87)points, (5.87 ±0.29)points vs. (3 .53 ±0.68)points, P <0.05].In OSAHS patients, mild subgroup NP level

    • 摘要: 目的:探讨慢性牙周炎与冠心病患者血清中新喋呤水平的变化及相关关系。方法选取受检者共120例,分为4组,每组30例:牙周炎合并冠心病组(C+P)、单纯牙周炎组(P)、单纯冠心病组(C)和健康对照组(H)。记录性别、年龄等一般统计学资料。检查牙周指标,包括全口牙齿的数量、探诊深度(PD)、临床附着丧失(CAL)和探诊出血,同时检测血清新喋呤和血脂水平。采用ANOVA法分析以上指标在各组间的差异,Pearson法分析血清各指标与牙周指标间的相关性。结果各组间牙周指标差异有统计学意义(P<0.05)。(C+P)组的血清新喋呤显著高于P组、C组和H组,差异有统计学意义(P<0.05)。P组、C组的血清新喋呤又显著高于H组,差异有统计学意义(P<0.05)。受检者的血清新喋呤水平与PD、CAL呈正相关。结论牙周炎与冠心病存在一定的关系,新喋呤可能与二者的病理机制相关。%Objective To explore the possible relationship of serum neopterin levels with chronic periodontitis and coronary heart disease(CHD). Methods A total of 120 patients were recruited in this study,including 30 patients with periodontitis and coronary heart disease (C+P),30 patients with periodontitis(P),30 patients with coronary heart disease(C),30 healthy controls(H). All parameters, including age,gender and periodontal clinic index,were recorded. Serum neopterin levels and lipid levels were examined at the same time. Differences in parameters among the four groups were analyzed by ANOVA. Correlations of each serum parameters with periodontal indexes were analyzed by Pearson correlation. Results The periodontal indexes were significantly different among the four groups (P<0.05). In(C+P)group,serum neopterin levels were significantly higher than that of P,C and H groups (P<0.05). The serum neopterin levels of P and C groups were significantly higher than that of H group (P<0.05). Serum neopterin levels were significantly correlated to probing depth and clinical attachment loss. Conclusions A certain correlation might exist between periodontitis and CHD,and neopterin might be closely related with the pathogenesis of these two diseases.

    • 摘要: 目的:以新喋呤为起始原料,用有机合成的方法引入二个碳原子形成衍生物后,再连接合适的载体蛋白,最终得到符合要求的新喋呤抗原。方法:以新喋呤和溴乙酸乙酯为起始原料,在新喋呤3位引入二碳原子手臂,得到N3-(2-羧甲基)新喋呤这一衍生物。再将N3-(2-羧甲基)新喋呤同合适的载体蛋白相连接,最终形成N3-(2-羧甲基)新喋呤—载体蛋白复合体。结果:竞争实验表明,该抗原完全达到预期设想的要求。结论:用本文提供的合成方法得到的复合体,性能完全符合预期,同时该制备方法有着反应步骤少,制备周期短,操作简便易控等优点,值得加以推广。

    • 摘要: Objective To evaluate the expression of serum neopterin in nephritic patients and its relationship with the prognosis of acute and chronic nephritis.Methods The levels of serum neopterin were determined in 40 patients with acute nephritis (A1 group)and 30 patients with chronic nephritis (A2 group)by enzyme-linked immunosorbent assay.The contents of serum creatinine and blood urea nitrogen were determined by Hitachi fully automatic biochemical analyzer.50 healthy blood donors were in the con-trol group.Results The levels of serum neopterin in acute nephritis and chronic nephritis were significantly higher than that in the normal group(P <0.05).The levels of serum neopterin in acute nephritis were significantly higher than that in chronic nephritis(P<0.05).The levels of serum creatinine and blood urea nitrogen in two nephritis group were significantly higher than that in the normal group(P <0.05).Conclusion The measurement of the serum neopterin may be a useful sensitive index for early diagnosis. This may be helpful for diagnosis and evaluation of therapy effect.%目的:探讨血清新喋呤(Npt)在肾炎患者病程中的表达及新喋呤与急、慢性肾炎的预后关系。方法对40例急性肾炎患者(A1组)和30例慢性肾炎患者(A2组)按病期采血,分别用酶联免疫吸附试验(ELISA)检测 Npt 的水平,用日立全自动生化分析仪检测血清肌酐(Cr)和尿素氮(BUN)的水平。另取50例健康献血者(B 组)作为对照组(B 组)。结果A1、A2组血清Npt 浓度明显高于 B 组(P <0.05),A1、A2组之间 Npt 水平也有差异(P <0.05),且 A1组、A2组与 B 组比较,Cr 和 BUN 明显增高,差异有统计学意义(P <0.05)。结论血清 Npt 是肾炎患者病情进展及早期诊断的敏感指标,对疾病的诊断及疗效监测有一定的临床价值。

    • 摘要: 目的:探讨冠状动脉粥样硬化性心脏病(简称冠心病)不同分支病变对血清新喋呤( neopterin, Np)、同型半胱氨酸及基质金属蛋白酶的影响。方法选择冠心病148例,根据病变分支数量分为单支组、双支组、多支组,分别有58例、53例、37例。检测血清Np、Hcy及MMP-2、MMP-9、TIMP-2水平,并比较各组中的差别及分析它们之间的相关性。结果多支组Np [(12.45±3.09)nmol/L]和Hcy [(23.71±4.57)μmol/L]均高于单支、双支组,双支组Hcy还高于单支组,差异均有统计学意义( P <0.05)。双支组 MMP-2[(49.83±7.64)ng/L]、MMP-9[(194.81±17.58)ng/L]较单支组均高,而 TIMP-2[(283.70±23.45)ng/L]较单支组则低,差异有统计学意义(P <0.05)。多支组 MMP-2[(63.42±8.59)ng/L]、MMP-9[(249.73±24.16)ng/L]、TIMP-2[(205.61±17.32)ng/L]较单支、双支组变化更加明显,差异有统计学意义(P<0.05)。 Np与MMP-9呈正相关(P<0.05),而与TIMP-2呈负相关(P<0.05);Hcy与MMP-2、MMP-9呈正相关(P<0.05),而与TIMP-2呈负相关(P<0.05)。结论 Hcy与Np是反映冠状动脉粥样硬化范围增加的重要因素,且与反映冠状动脉重构相关的MMPs家族密切相关。%Objective To investigate the effects of serum neopterin and homocysteine levels on substrate metals protease in patients with coronary heart disease in different branches.Methods 148 coronary heart disease patients were divided into single lesion group and double branch lesion group, multivessel lesions group, 58, 53, and 37 cases, respectively.Blood was sampled to detect Hcy, Np, MMP-2, MMP-9 and TIMP-2 levels.Results Hcy in duble lesion group was significantly higher than in single lesion ( P<0.05), Np [(12.45 ±3.09)nmol/L] and Hcy [(23.71 ±4.57) μmol/L] in multilesion group showed significant increase than in the groups A and B (P<0.05).MMP-2[(49.83 ±7.64)ng/L], and MMP-9[(194.81 ±17.58)ng/L] in double lesion group were significantly higher than in single lesion group (P<0.05), TIMP-2 [(283.70 ±23.45)ng/L] in double lesion group significantly de-creased (P<0.05).MMP-2 [(63.42 ±8.59)ng/L], MMP-9 [(249.73 ±24.16) ng/L] and TIMP-2 [(205.61 ±17.32)ng/L] in multilesion group were significantly different from the single and double lesion groups (P<0.05).Np and MMP-9 showed significant positive correlation (P<0.05), and TIMP-2 showed significantly negative correlation (P<0.05), Hcy and MMP-2 and MMP-9 showed significantly positive correlation (P<0.05), and TIMP-2 showd significantly negative correlation (P<0.05).Conclusions Np and Hcy are closely related to the field of coronary heart disease, which are closely related to MMPs indicating the progression of coronary atherosclerosis.
