20岁的年轻小伙子小王(化名)长期受到头疼的困扰,经过一系列检查,发现病根儿竟然是因为心脏上“破了个洞”!头痛的病根儿原来在心脏小王平时身体很健康,但是在三年前开始经常出现周期性头痛,头痛呈搏动性,伴恶心、乏力等症状,平均每周发作一次。小王以为是自己太累、工作压力太大的缘故,每次头痛发作就卧床休息。这样做后头痛症状虽有所缓解,但还是会反复出现,严重影响了他的工作和生活。

摘要:
偏头痛是临床常见的慢性神经血管性疾病,以偏侧头部搏动性的剧烈头痛为主要临床表现,可合并自主神经功能障碍而伴恶心呕吐、畏声畏光等。据相关研究表明,15%~30%的患者在急性发作前可能有神经系统先兆症状。该病常反复发作,具有高发病率、高致残率的特点。西医主要给予对乙酰氨基酚、布洛芬、奈普芬、双氯芬酸等药物治疗,但由于药物副作用较明显,患者依从性差,笔者临床发现采用针药结合的中医药方法进行治疗,往往能取得较为满意的疗效。现就1例典型临床病例分享经验,以供探讨。

摘要:
患者男性,51岁。主诉“腰背部疼痛3d,加重8h”。查体:血压140/81mmHg(1mmHg=0.133kPa)。腹平软。未触及明显搏动性包块。2015年7月在我院行主动脉电子计算机断层扫描血管造影(computed tomography angiography,CTA)示胸腹主动脉瘤,最大直径6.6cm(见图1A)。

摘要:
患者男性,49岁。因“发现右下腹部搏动性包块4年”于2017年11月16日入院。患者20年前有右大腿枪弹伤史,行伤口“清创缝合”后愈合良好。发现包块后于当地医院诊断为“右股动静脉瘘(arteriovenous fistula,AVF)”并已行“右股动脉造影并覆膜支架植入术”治疗,术后症状无明显缓解。

目的:探讨股静脉搏动性血流频谱(PFVF)与下腔静脉塌陷指数(IVC-CI)之间的关系.方法:选取2016年1月~12月在中日友好医院超声诊断科进行下肢静脉超声检查的患者80例,其中PFVF和非PFVF组患者各40例,分别测量IVC-CI,然后对比分析两组之间的统计学差异.结果:PFVF组和非PFVF组的IVC-CI分别为0.40±0.14和0.63士0.08,两组之间比较差异有统计学意义(t=8.814,P=0.000,差分的95%置信区间:0.176~0.279).结论:股静脉搏动性血流频谱与下腔静脉塌陷指数之间有一定的相关性.%Objective:To investigate the relationship between pulsatile femoral venous flow (PFVF)and inferior vena cava collapse index(IVC-CI).Methods:Eighty cases from January 2016 to December 2016 were enrolled in this study and all the cases had undergone lower extremity vein scans in the Ultrasound Department of China-Japan Friendship Hospital.They were divided into two groups:PFVF group and non-PFVF group,each had 40 cases.The IVC-CIs of both groups were calculated and analysed statistically.The method of IVC-CI calculation:in a supine position,the ultrasound probe was placed on the right side of the xiphoid,then,the anteroposterior diameter of the inferior vena cava was measured at a distance of 2.0 cm from the right atrium.The maximum diameter (Dmax)and the minimum diameter (Dmin)of the the inferior vena cava were recorded respectively.The IVC-CI was measured three times and the average value was calculated.How to determine a PFVF? First,in a supine position and normal respiration,gray-scale ultrasound was applied to find the entrance of right great saphenous vein into the common femoral vein,then Doppler sampling was placed at the top of the entrance 1.0cm~2.0cm.PW mode was opened and the common femoral venous flow spectrum was recorded.PFVF was determined when there was a spectrum periodically changed with the heart beat.Results:The IVC-CI of PFVF group and non-PFVF group were 0.40±0.14 and 0.63±0.08,respectively.There was significant difference between two groups (t=8.814,P=0.000,95% confidence interval for difference:0.176~ 0.279).Conclusion:There is a correlation between IVC-CI and PFVF.

摘要:
提高离心血泵的搏动性能减少血液周身循环阻力,有助于增加血液循环量。现有离心血泵主要工作在恒转速模式,辅助出的主动脉压搏动性较小,而脉压差过低对血管的结构和功能会产生不良的影响。所以针对离心泵搏动性较小的问题,本文建立循环系统和人工心脏血泵的耦合模型,利用模糊PI控制器,通过调制血泵的转速来提高离心泵辅助后主动脉压的搏动性。利用搭建的体外模拟循环实验平台,将血泵和基于STM32的控制器接入模拟循环装置中,辅助后平均主动脉压稳定在100 mmHg,脉压差稳定在20 mmHg。%The increase in pulsatility of centrifugal pumps can decrease the resistance of blood circulation. Most centrifugal pumps in use now work with constant pressure, which provides nonpulsatile perfusion. In non-pulsatile pumps, the pulsatility of the aortic pressure decreases with the increase of rational speed. The low pulsatile value of aortic pressure has a negative effect on the structures and functions of blood vessels. This paper builds the cardiovascular-pump model and simulates the model using MATLAB. The fuzzy PI Controller, which uses the volatility of the speed of pumps to enhance the pulsatility of the aortic pressure, was designed. Then we set up the experiment platform and linked the blood pump and the controller based on STM32 to the mock cardiovascular system. The result of the experiment indicates that the pulse pressure is 20 mmHg and the mean pressure is 100 mmHg, which can meet the hemodynamic requirements of LVAD.

This paper built the mathematical model of a centrifugal blood pump, which was designed by ourselves, combined it with that of the human cardiovascular system and simulated the coupling system using Matlab. Then we set up the experiment platform, linked the blood pump to mock human cardiovascular system in case of three-stage heart failure, and measured aortic pressure and flow under different speed. The comparison between experiment results and simulation results not only indicates the coupling model is correct and the blood pump works wel , but also shows that with the increase of blood pump speed, the pulsation of aortic pressure and flow wil be reduced, this situation wil affect the structure and function of blood vessels.%对自主设计的离心血泵建模,并将其与心血管集中参数模型耦合,在Matlab环境下对耦合系统进行仿真。搭建实验平台,在三级心衰的情况下,将血泵接入模拟循环装置中,测量不同转速下的主动脉压力和流量。通过实验数据和仿真数据的对比,验证了耦合模型的准确性和血泵良好的辅助效果,同时也说明了随着血泵转速的增加,主动脉压力和流量的搏动性会减弱,这种情况会对血管的结构和功能产生影响。

《老友》专家门诊:最近一次吃午饭时,我突然倒地,碗筷也撒了一地。当时我感到似有火星在眼前闪动,头部有搏动性钻痛等。经CT检查左脑有一大片血瘀,是脑梗塞,血压165/80mm Hg,低密度脂蛋白3.48mmol/L,医生叫我服用安内贞。之后还做了磁共振检查,显示左侧颞顶叶脑软化灶,双侧放射冠区多发性腔隙性脑梗塞(陈旧性),缺血性脱髓鞘脑改变。请问:我这样的病应怎样治疗,日常生活怎样调理?
