    • 摘要: 目的 观察胰高血糖素样肽-1(glucagon-like peptide-1,GLP-1)的类似物(利拉鲁肽)治疗轻、中度非增生期糖尿病视网膜病变(non-proliferative diabetic retinopathy,NPDR)的临床疗效.方法 收集我院内分泌科治疗的2型糖尿病合并轻、中度NPDR的患者60例.随机分为试验组和对照组,试验组患者采用二甲双胍+胰岛素联合利拉鲁肽强化控制血糖,对照组使用二甲双胍+胰岛素降低血糖.比较治疗前和治疗后3个月所有患者视网膜病变分期、黄斑区视网膜厚度和视网膜电图(electroretinogram,ERG)的振荡电位(oscillatory potentials,OPs) 总波幅值.结果 治疗后3个月2组患者糖尿病视网膜病变(diabetic retinopathy DR)临床分期差异无统计学意义(P>0.05);试验组治疗前及治疗后黄斑区视网膜厚度分别是(284.98±28.54) μm、(278.84±27.98) μm,差异有统计学意义(P<0.01),对照组治疗前后及2组患者治疗后黄斑区视网膜厚度对比差异均无统计学意义(均为P>0.05).试验组OPs总波幅治疗前后分别是(140.99±38.34) μV、(157.18-40.33) μV,差异有统计学意义(P=0.000);对照组OPs总波幅治疗前后分别是(140.80±38.60)μV、(133.76±43.29) μV,差异有统计学意义(P=0.015);治疗后试验组OPs总波幅较对照组高,差异有统计学意义(P =0.006).结论 GLP-1类似物可以延缓轻、中度NPDR的进展,对其预后有着积极的作用.

    • 摘要: OBJECTIVE To evaluate the clinical effect of Bushen Mingmu capsule on non-proliferative dia-betic retinopathy (NPDR) patients in liver-kidney yin deficiency and blood stasis syndrome. METHODS It was a prospective, randomized, controlled trial study. The NPDR patients in liver-kidney yin deficiency and blood stasis syndrome in the second affiliated hospital of Henan University of TCM from January 2016 to January 2017 was se-lected according to the inclusion and exclusion criteria and then randomly divided into observation group and control group evenly. All cases were treated according to the guideline of Chinese diabetes prevention guidelines, and the observation group was administered with Bushen Mingmu capsule while the control group was given Qiming granule. The two groups were followed up for 12 weeks and changes in terms of visual acuity, scores of symptoms of TCM, oc-ular fundus signs, the amplitudes and latent durations of flash electroretinogram (F-ERG) oscillatory potentials (OPs) before and after treatment as well as adverse reactions were recorded. RESULTS There were 80 patients (160 eyes) in total, 40 (80 eyes) in the observation group and 40(80 eyes) in the control group. The effect of their visu-al acuity and symptoms of TCM in the observation group was significantly better than that of the control group (92.50%VS 72.50%, P=0.004;87.50%VS62.50%, P=0.020). The effect of their ocular fundus signs was similar (82.50%VS 80.00%, P=0.809). After treatment, the amplitudes of F-ERG OPs in both groups was improved (P72.50%,P=0.004;中医证候疗效总有效率87.50%>62.50%,P=0.020),眼底体征疗效与对照组大致相当(总有效率82.50%>80.00%,P=0.809).治疗后,两组患者被观察眼的OPs子波振幅均较治疗前提高(P<0.05),子波时值较治疗前缩短(P<0.05);观察组的OP1、OP2振幅及各子波振幅总和明显高于同期对照组(P<0.05),OP1、OP2时值明显低于对照组(P<0.05).治疗期间两组患者空腹血糖及尿糖水平控制良好.检测一般项目(体温、心率、呼吸、血压)、心电图、血尿常规、肝肾功能未见明显异常.对照组在服药期间有5例患者出现腹痛、便溏等胃肠道不适,对症处理后症状缓解.结论 补肾明目胶囊对肝肾阴虚兼血瘀型NPDR有较好疗效,可以提高患眼视力,减轻眼底病变,改善内层视网膜功能及患者的中医症状,但更为确切的结论有待更大样本的多中心研究进行验证.

    • 摘要: 目的暗适应3.0振荡电位预测缺血型视网膜中央静脉阻塞(central retinal vein occlusion,CRVO)的临床价值。方法 2010年6月-2015年1月在延安市人民医院诊断为CRVO患者80例(80只眼),行视力、眼压、眼底及振荡电位、眼底荧光造影检查。随访2-9个月(平均6个月),发生缺血型CRVO为阳性组,未发生为阴性组。采用ROC曲线分析振荡电位在预测缺血型CRVO中的临床价值。结果振荡电位各子波的幅值及总幅值对于预测缺血型CRVO具有一定的临床价值,其中以振荡电位第2个子波(2nd wave of oscillatory potentials,OP2)和振荡电位总幅值的灵敏度及特异度最高。以OP2=18.1μV为截点值,预测缺血型CRVO的灵敏度96.7%,特异度93.3%,曲线下面积0.981(P〈0.001,95%CI:0.897-1.000);以振荡电位总幅值=64.9μV为截点值,预测缺血型CRVO的灵敏度96.7%,特异度90.0%,曲线下面积0.981(P〈0.001,95%CI:0.903-1.000)。结论振荡电位幅值下降程度与视网膜微循环功能受损程度相关,是预测缺血型CRVO的敏感性指标。

    • 摘要: AIM:To investigate the clinical value of dark adaptation 3. 0 oscillatory potentials in central retinal vein occlusion. n METHODS:Eighty-four patients (84 eyes) with central retinal vein occlusion were divided into 4 groups:no wave type ( 16 cases ) , grossly decreased type ( 27 cases ) , middle decreased type (20 cases) and slightly decreased type ( 21 cases ) accorded to the normal value of this experiment. The initial diagnosis was the examination of oscillatory potentials. No wave and grossly decreased type were first diagnosed as ischemic central retinal vein occlusion, middle and slightly decreased type were first diagnosed as non-ischemic central retinal vein occlusion. The patients were followed up for 6mo, recorded and contrasted the examination results between anterior and posterior. There were 76 cases having integrated data ( 8 cases were lost) . n RESULTS: No wave and grossly decreased type were ultimately diagnosed as ischemic central retinal vein occlusion ( 34 cases, 83%) , middle decreased type ( 5 cases, 25%) and slightly decreased type (0 case). There was statistically significant difference between the three groups (χ2=34. 5, P<0. 01). The statistical difference still existed comparing the grossly decreased type and the middle decreased type with slightly decreased type ( P<0.01 ) . However, there was no significant difference between the middle and slightly decreased type (P=0. 036). n CONCLUSION: The early oscillatory potential amplitudes of central retinal vein occlusion is decreased, which has important significance for early classification and prognosis of central retinal vein occlusion.%目的:探讨暗适应3.0振荡电位对视网膜中央静脉阻塞的临床应用价值。n  方法:视网膜中央静脉阻塞患者84例84眼,初诊时行振荡电位检查,依据本实验室正常值进行分组,无波型(16例),重度降低型(27例),中度降低型(20例),轻度降低型(21例)。无波型与重度降低型拟诊缺血型视网膜中央静脉阻塞。轻度降低型与中度降低型拟诊非缺血型视网膜中央静脉阻塞。随访6mo,记录对比检查结果,共76例资料完整(8例失访)。n  结果:重度降低型与无波型共34例(83%),最终诊断为缺血型视网膜中央静脉阻塞,中度降低型5例(25%)、轻度降低型0例最终诊断为缺血型视网膜中央静脉阻塞,三组比较具有统计学差异(χ2=34.5,P<0.01),其中重度降低型与轻度降低型以及中度降低型比较,具有统计学差异(P<0.01),轻度降低型与中度降低型比较,无统计学差异(P=0.036)。n  结论:视网膜中央静脉阻塞早期的振荡电位振幅降低,振荡电位对视网膜中央静脉阻塞早期分型和预后评估有重要意义。

    • 摘要: 目的:研究大鼠甲醇中毒后视网膜电图(electroretinogram,ERG)振荡电位(oscillatory potentials, OPs)的变化特点。方法建立对照组及甲醇中毒3 d组、中毒7 d组的SD大鼠模型,在相应时间点于暗室内进行ERG检测,记录OPs波形特点。结果相对于对照组,中毒3 d组和中毒7 d组的总振幅均下降了约50%,时程分别延长约16%和61%。结论大鼠甲醇中毒后,ERG的OPs呈现时程延长、总振幅下降特点。%Objective To study the characters of oscillatory potentials (OPs) of electroretinogram (ERG) after methanol intoxication in rats. Method The SD rat models of methanol intoxication were established and divided into control group, 3-day intoxication group, 7-day intoxication group. The changes of OPs of ERG were recorded in a dark room. Results The total amplitudes of 3-day and 7-day intoxication groups decreased approximately 50% compared with that of the control group, while the schedule de-layed approximately 16% and 61%, respectively. Conclusion The characters of methanol intoxication in rats included delay in schedule and decline in the total amplitude of OPs.

    • 摘要: 视网膜电图振荡电位(OPs)是叠加在视网膜电图(ERG)中b波上升支的数个高频率、有节律的小波。因为OPs是反应视网膜内层功能的一个敏感指标,国际临床视觉电生理学会(ISCEV)把它列入5项检查指标之一。OPs的准确采集取决于滤波器的正确使用,然而由于提取OPs时滤波器参数设置不当,很多研究未能将OPs中所包含的ERG低频率成分剔除,得到的并非是纯净的OPs。此外,传统的OPs计量方法是将OPs各子波振幅累加而得。这种测量方法受低频率成分的干扰导致可靠性和重复性差,明显影响OPs的分析。正确设置滤波器可得到纯净的OPs。用积分的方法计算OPs信号频谱曲线下的面积即振幅能量的方法可准确计量0Ps。

    • 摘要: Oscillatory potentials (OPs) are a group of regular,high frequency wavelets superimposed on the ascending limb of the electroretinogram (ERG) b-wave.The ERG standard recommended by the International Society for Clinical Electrophysiology of Vision (ISCEV) suggests oscillatory potentials are one of the five routine examinations because OPs are sensitive functional indicators of the inner retina.Extraction of pure OPs depends largely on using filters correctly.However,many OPs studies fail to remove the low frequency components of the ERG simply because of inappropriate filter settings.In addition,the traditional method of OPs measurement summarizes the amplitude of each single wavelet.The reliability and repeatability of the method are significantly compromised by the low frequency contaminants.To extract pure OPs,it is essential to set the filter parameters appropriately.Calculating the area below the single-sided frequency spectrum curve by integration provides an accurate assessment of the total power of OPs.%视网膜电图振荡电位(OPs)是叠加在视网膜电图(ERG)中b波上升支的数个高频率、有节律的小波.因为OPs是反应视网膜内层功能的一个敏感指标,国际临床视觉电生理学会(ISCEV)把它列入5项检查指标之一.OPs的准确采集取决于滤波器的正确使用,然而由于提取OPs时滤波器参数设置不当,很多研究未能将OPs中所包含的ERG低频率成分剔除,得到的并非是纯净的OPs.此外,传统的OPs计量方法是将OPs各子波振幅累加而得.这种测量方法受低频率成分的干扰导致可靠性和重复性差,明显影响OPs的分析.正确设置滤波器可得到纯净的OPs.用积分的方法计算OPs信号频谱曲线下的面积即振幅能量的方法可准确计量OPs.

    • 摘要: 目的:分析在模拟失重状态下,反映视功能的视觉诱发电位(VEP)及反映视网膜供血的振荡电位(OPs)与远近视力的相关性.方法:选取14例男性健康志愿者,-6°头低位模拟失重状态,在受试前、受试第2d及受试第5d记录远近视力、VEP及OPs.VEP标记P100波,OPs按照国际视觉电生理协会的临床电生理标准(2008年修订)记录总和波.结果进行统计学分析.结果:P100波幅及OPs总和波在受试后发生显著的变化(P=0.0152,P=0.0238).将这两项结果与远近视力进行相关性分析,未见明显相关.结论:头低位模拟失重状态导致全身血液重新分布,引起眼部血供的改变,这种供血的改变可以导致视觉电生理的变化,但对远近视力这种基础的视功能影响不大.%AIM: To observe the influence of head - down tilt simulated weightlessness on visual evoked potential (VEP), oscillatory potentials (Ops) and visual acuity, and analyse the relationship among them. METHODS: Head-down tilt for -6° was adopted in 14 healthy volunteers. Distant visual acuity, near visual acuity, VEP and Ops were recorded before, two days and five days after trial. The record procedure of Ops followed the ISCEV standard for full - field clinical electroretinography (2008 update).rnRESULTS: Significant differences were detected in the amplitude of P100 waves and ∑Ops among various time points(P<0. 05). But no relationship was observed among VEP, Ops and visual acuity.rn CONCLUSION: Head - down tilt simulated weightlessness induce the rearrange of blood of the whole body including eyes, which can make the change of visual electrophysiology but not visual acuity.

    • 摘要: 背景 暗适应视网膜电图(ERG)的振荡电位(OPs)在多种视网膜疾病的视功能评价中发挥重要作用,但其波形的确切起源尚未完全明确.OPs波的时域和频域分析及其影响因素的研究对进一步研究OPs的发生和起源具有重要意义. 目的 观察正常大鼠的暗适应ERG OPs与刺激光强度和动物年龄的关系. 方法 选择出生后21、25、32、35、37、46、60及90 d的RCS-rdy+-p+大鼠各3只,采用RETI scan记录系统进行闪光视网膜电图(F-ERG)OPs波记录,记录电极为环形角膜电极,参考电极为不锈钢针状电极,刺激强度分别为-20、-10、-5、0、5 dB,通过Matlab提取OPs成分,分析其频谱特性.结果 上述各种刺激强度下不同年龄组大鼠OPs的最大能量振幅值对应的频率值(即主频)范围为80 ~ 120 Hz.随着刺激光强度的增加,开始出现高频(200 ~ 250 Hz)成分,对应的振幅值随着刺激光强度的增加而增加;低频成分,即主频随着刺激光强度的增加其对应振幅值出现先升高后下降的趋势;当刺激光强度达到O dB时,低频成分出现双峰现象.25日龄大鼠OPs主频振幅最小,60日龄大鼠OPs主频振幅最大;25日龄大鼠OPs主频的带宽最窄,60日龄大鼠OPs主频的带宽最宽.结论 大鼠ERG的OPs频域特性分析说明,刺激光强度对大鼠OPs振幅产生正向的影响,而大鼠的年龄主要影响OPs的主频分布.%Background Oscillatory potentials (OPs) of scotopic electroretinogram (ERG) plays an important role in the evaluation of visual function in multiple retinal diseases.However,the origin of OPs is uncompletely clear.It is essential to analyze the time domain and frequency domain components for the further study of OPs.Objective The present study was to investigate the change characteristics of frequency spectrum of scotopic OPs with age and stimulating intensity.Methods RCS-rdy+-p+rats with the ages of 21,25,32,35,37,46,60,90 days were selected iu this study and 3 rats for each.Scotopic flash ERG were recorded from all the rats with RETI-scan system.Gold-foil ring cornea recording electrode was used as the recording electrode and the steel needle electrode was used as the reference and earth electrode during the record.The intensity of stimulating light was set at-20,-10,-5,0 and 5 dB respectively.Data were output into the computer and processed by the software Matlab7.0.Results The principle frequency corresponding to maximum amplitude component was 80-120 Hz in the various ages of rats under the different stimulating conditions above.With the increase of the intensity of stimulating light,high frequency component (200-250 Hz) began to appear and the amplitudes showed a gradually raise upon the intensity of light.The major component was subdivided into two peaks at 0 dB stimulation.Further,the age affected the major frequency peak with the maximum value at 60-day-old rats and the minimum value at 25-day-old rats.Also,the pass-band width of main amplitude appeared to be maximal at 60-day-old rats and minimal at 25-day-old rats.Conclusions OPs in Rcsrdy+-p+ rats are influenced by stimulating intensity and agc.Stimulating intensity affects the amplitude and age lead to the change of distribution of primary frequency of OPs.It is possible to know the influences of the degeneration of rods and be helpful to diagnosis this kind of disease.
