摘要:
目的 分析疼痛护理在外伤性骨折患者康复治疗中的应用效果及对依从性的影响.方法 选择2018年2月至2020年2月收治的84例外伤性骨折患者,依照护理方法将其分为对照组(n=42,常规护理)与研究组(n=42,常规护理+疼痛护理).比较两组的护理效果.结果 护理12、24、72 h后,研究组VAS评分低于对照组,差异具有统计学意义(P<0.05).护理后,两组HAMA、HAMD评分均降低,且研究组低于对照组,差异具有统计学意义(P<0.05).研究组生理领域、心理领域、社会领域、环境领域评分均高于对照组,差异具有统计学意义(P<0.05).研究组的总依从率为95.24%,明显高于对照组的76.19%,差异具有统计学意义(P<0.05).结论 疼痛护理用于外伤性骨折患者康复治疗中的效果良好,能够减轻疼痛程度,提高依从性,改善康复质量,值得临床推广.

摘要:
目的 观察骨折患者术后服用药物后骨折愈合情况.方法 记录了骨折患者共计70例,采用回顾性病例观察三花接骨散及双氯芬酸钠双释放肠溶胶囊,分为试验组(35例)和对照组(35例).对照组给予双氯芬酸钠双释放肠溶胶囊,而试验组是在对照组的基础上增加三花接骨散,观察过程均为4周.结果 治疗4周后,评价指标总有效率为88.6%(试验组)高于51.4%(对照组).结论 三花接骨散在一定程度上对患者骨折愈合起到加速作用.

摘要:
目的 分析当归补血汤加味治疗外伤性骨折患者的效果.方法 68例外伤性骨折患者,随机数字表法分为西医常规组与中药治疗组,每组34例.西医常规组的患者给予常规治疗,中药治疗组给予当归补血汤加味治疗.比较两组骨折愈合时间、疼痛缓解时间、治疗前后疼痛评分、总有效率、不良反应发生率.结果 中药治疗组骨折愈合时间(3.21±1.01)个月、疼痛缓解时间(4.27±1.01)d,均短于西医常规组的(4.67±1.56)个月、(6.27±1.01)d,差异有统计学意义(P<0.05).治疗后,两组患者疼痛评分均降低,且中药治疗组低于西医常规组,差异有统计学意义(P<0.05).中药治疗组总有效率为100.00%(34/34),高于西医常规组的76.47%(26/34),差异有统计学意义(P<0.05).中药治疗组不良反应发生率为2.94%(1/34),低于西医常规组的17.65%(6/34),差异有统计学意义(P<0.05).结论 当归补血汤加味治疗用于外伤性骨折患者治疗中的临床效果确切,可有效缓解患者症状,并改善患者的疼痛,加速愈合,不良反应少,值得推广.

目的:探讨疼痛护理在外伤性骨折康复治疗中的作用。方法:收集2020年6月~12月期间就诊于本院骨外科的外伤性骨折患者80例,采用随机数字法将其分为观察组和对照组各40例。视觉模拟评分法(VAS)评估两组患者护理前和护理后24 h、48 h的疼痛改善情况;生存质量量表评估两组患者术后康复效果;汉密尔顿焦虑量表(HAMA)和汉密尔顿抑郁量表(HAMD)评估两组患者护理后焦虑和抑郁程度。结果:护理前两组患者VAS评分、HAMA评分和HAMD评分无显著差异(P>0.05);护理后24 h观察组VAS评分较对照组改善程度更大,且护理后48 h观察组VAS评分减少程度更多,观察组HAMA、HAMD评分也显著降低,差异具有统计学意义(P<0.05);此外,观察组生存质量量表评分均高于对照组,差异具有统计学意义(P<0.05)。结论:外伤性骨折患者术后给予疼痛护理能显著降低患者的疼痛程度,且明显缓解因疼痛而产生的紧张、焦虑和抑郁等不良情绪,患者康复效果更佳。

目的:观察评价在外伤性骨折患者康复治疗中应用疼痛护理模式的效果,以及对提升依从性的影响.方法:本研究选取罹患外伤性骨折患者共120例,病例筛选时间范围为2019年03月~2021年03月,随机分组,实施常规护理干预的60例病患纳入对照组,实施疼痛护理干预的60例病患纳入观察组,就两组干预效果展开评比.结果:观察组干预后0.5d、1d、3d不同时间点的VAS评分经评测相较对照组呈更低显示,总依从率相较于对照组呈更高显示(P<0.05).结论:对罹患外伤性骨折患者来说,在其康复治疗期间辅助疼痛护理干预,可有效减轻患者疼痛,提高其依从性.

摘要:
目的 探究X线平片及MSCT后处理技术在儿童肘关节外伤性骨折中的临床应用价值.方法 回顾性分析我院2017年6月至2019年7月收治的60例肘关节外伤性骨折患儿的临床治疗,观察X线平片检查及MSCT检查的诊断结果并进行讨论和分析;对比经X线平片检查、MSCT检查对儿童肘关节外伤性骨折的诊断准确性、敏感性和特异性.结果 经MSCT检查对儿童肘关节外伤性骨折的诊断灵敏性、特异性和准确性分别为98.33%、86.67%、96.67%,X线平片检查对儿童肘关节外伤性骨折的诊断灵敏性、特异性和准确性分别为83.33%、71.67%、81.67%,MSCT检查对儿童肘关节外伤性骨折的诊断灵敏性、特异性和准确性高于X线平片检查(P<0.05).结论 X线平片和MSCT检查均可有效的显示儿童肘关节外伤性骨折的影像学特点,但是MSCT后处理技术诊断儿童肘关节外伤性骨折的灵敏性、特异性和准确性更好,临床应用价值更高.

摘要:
目的:评价四物汤分期加减治疗外伤性骨折的有效性及对患者疼痛症状的影响.方法:选取2018年10月-2020年7月我院外伤性骨折患者76例,均行骨折整复术,按随机数表法分为研究组(n=38)和对照组(n=38),对照组予以常规治疗,研究组在对照组的基础上予以四物汤分期加减治疗.对比两组疗效、不良反应发生率及治疗前后视觉疼痛模拟评分法(VAS)评分、中医证候积分.结果:研究组总有效率(97.37%,37/38)较对照组(78.95%,30/38)高(P<0.05);治疗后,两组肿胀、瘀斑评分均较治疗前降低,且研究组肿胀、瘀斑评分较对照组低(P<0.05);治疗后,两组VAS评分均较治疗前降低,且研究组VAS评分较对照组低(P<0.05);研究组不良反应发生率(5.26%,2/38)较对照组(21.05%,8/38)低(P<0.05).结论:四物汤应用于外伤性骨折治疗,可提高疗效,改善临床症状,减轻疼痛,降低不良反应.

摘要:
目的 探讨基于活动性疼痛评估对改善外伤性骨折患者生活质量的作用.方法 2018年1月至2019年12月选取该院收治的外伤性骨折手术患者120例,随机分为对照组和观察组,每组60例.对照组术后采取常规护理措施,观察组术后根据活动性疼痛评估结果采取针对性疼痛护理,比较术后疼痛、舒适度评分,比较不良情绪、生活质量评分,比较睡眠指标、护理满意度.结果 术后12~48 h,与对照组比较,观察组疼痛评分明显降低,舒适度评分明显增高,差异有统计学意义(P<0.05).护理后,与对照组比较,观察组焦虑、抑郁、睡眠质量评分均明显降低,入睡潜伏期明显缩短,实际睡眠时长明显延长,生活质量评分明显增高,差异有统计学意义(P<0.05).护理总满意率观察组高于对照组,差异有统计学意义(P<0.05).结论 基于活动性疼痛评估结果实施的护理干预措施,可有效减轻外伤性骨折患者术后疼痛感,提高其舒适度,有利于减轻不良情绪,改善睡眠状况,提升生活质量和满意度.

目的:详细研究分析外伤性骨折 X 线诊断漏诊原因,并制定有效降低漏诊率的有效措施。方法:于本院收治的外伤性骨折接受 X 线检查后判定存在漏诊患者中筛选 40 例作为本次研究对象,对患者临床资料进行分析及研究,判断漏诊原因并制定有效措施降低漏诊率。结果:19 例患者因自身因素导致漏诊,占比 47.50%,诊断原因导致漏诊 9 例,占比 22.50%,检查医师原因漏诊 8 例,占比20.00%,设备原因导致漏诊 4 例,占比 10.00%。结论:外伤性骨折患者接受 X 线检查过程中会受到多种因素影响产生漏诊,主要为患者自身因素及临床诊断因素,因此需要根据工作实际总结漏诊主要影响因素,制定有效解决措施提升诊断准确率,为临床诊治工作提供可靠依据,望重视。

Objective:To discuss the initial applied research of traumatic fracture in long bones of impuberal pigs with low dose CT scanning, to seek and establish the best proposal of low radiation dose CT scanning.Methods:12 healthy and fresh hind leg exvivo of a pig were struck by means of blunt force, in order to produce fracture models.Then these were scanned with different imaging conditions in two multi-slice CT(MSCT), including Philips Brilliance 6 and Philips Brilliance 64. The various scanning parameters were as follows:①6-MSCT:tube voltages were 120kv, 90kv and tube current-time products were 200mAs 150mAs, 100mAs, 50mAs, 25mAs. Kv and mAs were combined in every two parameters and generated 10 groups of imaging conditions.②64-MSCT: tube voltages were 120kv, 80kv. Tube current-time products and the way of combination between Kv and mAs were the same as that of 6-MSCT. All CT images were assessed visually by two independent observers for image quality and fracture conditions using a 5-point scale (1-5)and interobserver concordance test was carried out. The rate of qualification of CT images quality was calculated. The noise level of CT images was demonstrated by standard deviation (SD) of CT value, which was measured by means of setting region of interest (ROI).Fracture conditions were compared between X-ray radiographs and CT images. CTDIv and DLP were recorded inevery scanning and effective dose (ED) was calculated. Statistical analysis was performed with Kruskal-Wallis Test for image scores among 10 groups of imaging conditions inevery CT modality and ANOVA among SD and CTDIv.
