    • 摘要: The First Affiliated Hospital of Sun Yat-sen University, was the first case in China of IT system renovation by implementing an HIE platform compliant with international health IT standards and technical framework. By deploying HIE platform and connecting heterogeneous systems, the renovation achieved the patient-centered EHR information integration and sharing, provided with timely, high-quality data for daily clinical practices, scientific research, service quality management, personnel performance evaluation, etc. From the project management and IT construction points of view, the EMR platform with interoperability has the advantages of manageable project schedule, reasonable cost and little interference with hospital operation due to its minimal invasion into existing systems, decoupling between systems and easy replication of deployment. The HIE product used in the project was developed in compliance with the core syntactic and semantic standards of the industry, following the key function definitions for interoperability and designed similar to the software structure of mainstream HIE products in the market. In comparison to the widely adopted heterogeneous system integration strategies using database and ETL techniques, HIE has the obvious superiority in system stability, timeliness and quality of the data.%中山大学附属第一医院是国内首个采用国际标准和行业技术规范开发HIE平台,进行院内信息化改造的落地案例。通过HIE平台的搭建,接入异构的业务子系统,实现了以病人为中心的电子病历信息的整合和全院共享,为临床业务、临床科研、质量管理、绩效管理等提供实时、高质量的数据共享和各类数据应用。互操作性HIE平台在信息化建设中,因具有对原有系统“侵入”小、系统间解耦、可复制性高的特点,使项目周期可控、建设投入合理,对业务影响小,建设可发展性高。由于HIE平台是采用语法互操作性核心标准、语义互操作性核心标准,按照互操作性核心集成功能规范和国际主流互操作平台架构开发的,相对国内通常采用的数据库加ETL技术实现异构系统集成的模式,在系统稳定性、数据实时、及时性、数据质量上具有无法比拟的优势。

The mobile medical workstation is an important system of mobile medical service platform. The system requires data exchanging and sharing with HIS, LIS, PACS. But these systems may be based on heterogeneous database platforms and operating system platforms, and may also be programmed in different languages. So there are"information isolated island"between these systems. The existing medical information integration standard can not be applied to the information integration between these systems. This paper firstly introduces the concept and the function of the mobile medical workstation, and analyzes the data exchange process of mobile medical workstation with LIS, PACS. Secondly, the paper introduces the technology framework of IHE, and analyzes the process of patient identification based on IHE PIX. Finally, the paper analyzes the process of clinical documentation information sharing based on IHE PIX and XDS. It is very significant for improving efficiency and quality of medical services.%移动医疗服务是基于移动计算的模式,在网络和医疗信息系统基础上产生的一种新型医疗服务模式。移动医生工作站是移动医疗服务平台的一个重要系统,需要与HIS、LIS、PACS等其他医疗信息系统进行数据交换与共享,但是这些系统可能基于异构的数据库平台、操作系统平台,也可能用不同的语言开发,所以在这些系统之间存在“信息孤岛”。现有的医疗信息集成标准不能适应这些系统之间的信息集成。本文首先分析了移动医生工作站与LIS、PACS的数据交换过程;其次,介绍了IHE技术框架,分析了基于IHE PIX的病人身份识别过程;最后,分析了基于IHE PIX和XDS临床文档信息共享的过程。基于IHE PIX和XDS集成框架的移动医疗信息集成方法不仅可以跨不同的临床信息域,而且可以跨不同的医疗机构解决异构的医疗系统集成问题,对提高医疗服务效率和质量都有重要意义。
