摘要:
目的 探讨并比较可吸收螺钉及微型钢板治疗Bennett骨折的治疗效果.方法 自2009年5月至2014年5月,回顾性分析42例Bennett骨折患者,20例应用可吸收螺钉内固定治疗(A组),22例应用微型钢板内固定治疗(B组).术后分别从手术时间、术后关节功能恢复、骨折愈合情况、并发症等进行评估比较.结果 术后所有患者均获得随访,时间为6~ 12个月,平均8.2个月.按中华医学会手外科学会上肢部分功能评定试用标准评定:A组优12例,良5例,可2例,差1例,优良率为85.0%;B组优17例,良3例,可2例,差0例,优良率为90.9%.两组的术后功能评分、平均手术时间、感染发生率、骨折平均愈合时间比较,差异均无统计学意义(P>0.05).术后开始功能锻炼时间,B组优于A组,差异有统计学意义(P<0.05).结论 可吸收螺钉和微型钢板都是治疗Bennett骨折的有效方法,微型钢板固定可更早期开始功能锻炼.%Objective To evaluate and compare the clinical outcomes of treating Bennett fractures with mini-plate fixation and absorbable screw fixation.Methods A retrospective study was conducted to review 42 cases of Bennett fractures treated from May 2009 to May 2014.Among them 20 fractures were fixed with absorbable screws (group A),while the other 22 fractures were treated with mini-plate fixation (group B).Surgery time,postoperative joint function,fracture union time and complications were compared between the two groups.Results Postoperatively the patients were follow-up for 6 to 12 months,with an average of 8.2 months.According to the upper extremity functional evaluation criteria issued by the Hand Surgery Society of the Chinese Medical Association,the outcomes of group A were rated as excellent in 12 cases,good in 5 cases,fair in 2 cases and poor in 1 case,while those of group B were rated as excellent in 17 cases,good in 3 cases and fair in 2 cases.The overall satisfactory rate was 85.0% for group A and 90.9% for group B.There were no significant differences between the two groups in terms of average surgery time,joint function,infection incidence,and fracture healing time (P > 0.05).However,patients treated with mini-plate fixation started rehabilitation exercises earlier (P < 0.05).Conclusion Mini-plate fixation allows earlier exercises.Both absorbable screws and mini-plates can effectively fix Bennett fractures.
