[Problem] To provide a blow-by gas treatment device and an engine provided with the blow-by gas treatment device that makes it possible to inhibit the release of oil, which has been separated from blow-by gas, from an outlet even when the engine is inclined in the front or back direction. [Solution] A blow-by gas treatment device 100 is provided with: a main structure unit 101 which separates oil OL from blow-by gas BG; and an outlet unit 40 which supplies gas G that has been separated from the blow-by gas BG to an induction system. The main structure unit 101 has: a first blow-by gas intake unit 111; a second blow-by gas intake unit 112; a separation unit 330 which separates the blow-by gas BG into the oil OL and the gas G; a first oil guide unit 151 which guides the oil OL to the front side; a second oil guide unit 152 which guides the oil OL to the back side; a first oil drain 161 which drains the oil OL into the engine; and a second oil drain 162 which drains the oil OL into the engine.
