The gallstone surgery instrument has a trocar sleeve insert (1) with a relatively large free internal diameter (a) and no valve. This allows insertion of a surgical endoscope. On the outside of the trocar insert are mounted elements (2) to increase frictional resistance to axial movement in the body cavity. The length of the trocar insert can be relatively short, the friction elements commencing close to its distal end. The trocar insert may have a holder with an articulator support arm.
