The handpiece, for a medical or cosmetic instrument, has a rotary drive with a tool clamp in a housing for a rotating tool. The axial drilling (50) at the clamping sleeve has local recesses along the longitudinal line of the drilling, to increase the drilling radius. The recesses have a depth at the leading opening (50') of the drilling which is less in the radial direction than the recess depth at the trailing end (50) of the axial drilling. The difference between the two recess depth levels is 0.005-0.02 mm.
