PPROBLEM TO BE SOLVED: To use both personal electronic information and paper while securing advantages in inputting various kinds of personal information using the paper and to combine the personal information and the paper by seamless operability. PSOLUTION: A medical information system includes: a printer 100 which outputs a chart form 70 approximately on the whole surface of which a code image obtained by converting code information including medium identification information into an image is printed; an electronic pen 5 for recognizing a user's writing operation to the chart form 70 and a key database (DB) 21 which stores the personal information in association with the medium identification information. Then, a predetermined piece of medium identification information is uniquely specified linked with the writing operation recognized by the electronic pen 5 and access to a predetermined piece of personal information in the key database (DB) 21 is allowed by the predetermined piece of medium identification information to be specified.
