A continuity consultation document generation system and a method, and a recording medium for recording a program therefore are provided to present a continuity consultation document to a user, by rendering the continuity consultation document that is automatically generated up to an entry level. A format definition unit(120) generates a basic structure of continuity consultation document consisting of at least one sub templates. A document delivery unit(130) outputs the already generated continuity consultation document or new continuity consultation document according to a request of a medical team's terminal. A data mapping unit(140) maps data transmitted from the medical team's terminal with the sub templates of the continuity consultation document outputted from the document delivery unit. A template generator(150) generates an entry by combining at least one template, and stores the generated entry as continuity consultation document.
