METHOD OF SELECTING TACTICS OF SURGICAL TREATMENT OF CHRONIC PANCREATITIS, COMPLICATED WITH THROMBOSIS OF PORTAL VEIN

FIELD: medicine.;SUBSTANCE: invention relates to surgery and can be applied for selection of tactics of surgical treatment of chronic pancreatitis, complicated by thrombosis of portal vein. Inferior mesenteric vein is exposed to sigmoid veins and transected at this level. Sigmoid veins are ligated. Exposed lower mesenteric vein is cannulated. Water-soluble radiopaque preparation is introduced until mesenterico-portal venous system is filled. Mesentericoportography is performed. Degree of portal vein occlusion on the section, which underlies pancreas, is determined. In case of complete occlusion of portal vein sectoral resection of pancreas head and longitudinal pancreatomy with opening of lumen of main pancreatic duct is performed. One-row pancreaticoenteroanastomosis is formed. Inferior mesenteric vein is passed above pancreaticoenteroanastomosis to portal vein. Precision mesenterico-portal venous anastomosis is formed.;EFFECT: method makes it possible to eliminate extravasal compression of portal vein affluents, prevent development of portal hypertension in remote terms after operation.;1 ex, 4 dwg
