PROBLEM TO BE SOLVED: To provide a technology for making information to be created in a medical providing facility and information to be created by a patient comprehensively available while securing the traceability of an individual patient under such situations that the use of the personal information of the patient is limited.SOLUTION: Medical information to be acquired by a medical provision side terminal is stored in a medical information collection device. Also, declaration information relating to medication or symptoms declared by a patient which is acquired by a patient side terminal is stored in a declaration information collection device. Then, in response to a request from the patient side terminal, an identifier for the patient is issued by the medical information collection device, and the issued identifier for the patient is registered in the declaration information collection device. The declaration information collection device acquires at least a part of medical information stored by the medical information collection device by using the identifier for the patient instead of the personal information. Then, the declaration information collection device stores the acquired information in association with patient attributes.
