The present invention concerns a high-speed rotational atherectomy device for opening a stenosis in an artery having a given diameter, comprising:a guide wire (15) having a maximum diameter less than the diameter of the artery;a flexible, elongated, rotatable drive shaft (20) advanceable over the guide wire and having a rotational axis;.a concentric abrasive element (121C) disposed on the drive shaft (20) and having a center of mass located on the rotational axis of the drive shaft;an eccentric distal counterweight (124E) disposed on the drive shaft (20) spaced apart distally from the concentric abrasive element (121C) by a distal spacing and having a center of mass spaced radially away from the rotational axis of the drive shaft; andan eccentric proximal counterweight (123E) disposed on the drive shaft (20) spaced apart proximally from the concentric abrasive element (121C) by a proximal spacing and having a center of mass spaced radially away from the rotational axis of the drive shaft.
