Methods are provided for efficient isolation and use of pluripotent adipose-derived stem cells (PASCs). In certain embodiments, the method includes providing an adipose tissue sample, from which stromal vascular cell groups derived therefrom are co-cultured with the adipocyte fraction. PASCs can be isolated with a high degree of purification and no additional cell enrichment processes (eg cell sorting) are required. PASCs and their conditioned media can be used for tissue regeneration within hours of collection of adipose tissue and no cell growth is required. PASCs can grow as individual cells that float, as a cluster of cells, or attached to the surface (s) of a culture vessel. When transplanted, PASCs do not produce teratomas in vivo and do not induce immune rejection, which achieve high efficiency in grafting.
