The present invention relates to a pharmaceutical composition for treating drug-resistant cancer comprising an exosome extracted from stem cells being differentiated as an effective component. The exosome extracted from stem cells being differentiated according to the present invention has an excellent expression level of bioactive factors that influence differentiation, and promotes reprogramming of cancer stem cells, and thus has the effect of differentiating into cancer cells having attenuated drug resistance. In addition, the exosome of the present invention, extracted from stem cells being differentiated, is a cell-derived material, and thus has excellent biocompatibility and can minimize the adverse effects of existing chemical therapeutic agents. In addition, since the exosome itself can act as a carrier, to allow an easy application of the loaded component in a human body, the exosome of the present invention has an advantage when applied as a drug-resistant cancer therapeutic agent. Accordingly, the pharmaceutical composition for treating drug-resistant cancer according to the present invention, comprising an exosome extracted from stem cells being differentiated as an effective component, may be applied as a preparation for treating drug-resistant cancer or a pretreatment composition for cancer treatment.
