A patient-facing digital platform designed to promote health literacy and numeracy that helps patients access, interpret, process and contextualize personal health data so that they may manage their health conditions. The platform interprets and simplifies personal health data into simple illustrations; provides substance-interaction and food-interaction information; offers health-education videos and dietary recommendations; and translates medication information into various languages.
