The present invention relates to a questionnaire for career suitability inventory-middle school students (CSI-M). According to the questionnaire for the CSI-M of the present invention, a test is performed by using a questionnaire for suggesting career, university, and job suitable for a middle school student. Items included in the questionnaire for the CSI-M include business performance capability, business performance knowledge, personality, interest, and occupational value, and middle school students are questioned by using the questionnaire including the items, and are measured to provide career, school entrance, job information suitable for individual students. The questionnaire for the CSI-M form questionnaire content based on understanding of middle school students, a job priority suitable for the middle school students can be recognized through the questionnaire content, and recognized job priority information is provided for students. In addition, the questionnaire of the present invention is configured to provide information suitable for middle school students who are examinees in an occupational world by complementarily applying a response for aptitude, interest, personality, value, etc. which are regarded as important by employees who conduct an actual job in various occupational groups on the basis of responses of the middle school students who are examinees.
