The body 1500 connected to an arm 1550 having a connection opening 1560 configured to allow connection of the body 1500 to a bone plate 1202. The connection opening 1560 having a connection axis spaced from the vertical axis. The connection opening 1560 may be spaced from the bore in a longitudinal direction relative to the longitudinal dimension of the arm 1550. A fastener may connect the bone plate 1202 to arm 1550.
