Učestalost, obilježja i liječenje kronične maligne boli u bolesnika s karcinomom pluća ne-malih stanica Frequency, characteristics and managing of chronic malignant pain in non-small-cell lung cancer patients

Pain continues to be a prevalent and undertreated symptom experienced by most patients with non-small-cell lung cancer. A total of 301 patients have been enrolled in prospective study, median age 63 years, most by males (74%), with metastatic disease (57%), good performance status (ECOG 0/1 in 58%). Prevalence of some degree of pain was 100%; moderate and severe more than 70%. Pain intensity was related to stage of disease and patient performance status - higher in patients with advanced disease (28 vs 13 %), worse performance status (46 vs 11 %) and in patients with bone metastases (45 vs 23 %). More than 95% of the patients had inadequate analgesic treatment before study. In the study transdermal fentanil was used as opioid in analgesic protocol. All patients benefit from the opioid therapy during the three month period. Patients with more advanced disease and worse performance status required more time to achieve stable pain control and higher opioid doses than patients with less advanced disease and good performance status. Patients with moderate pain intensity before study, good PS and advanced disease benefit the most. udud
