This paper analyzes the records of clinical practice of Geriatric Nursing and incidents encountered during such practice, and examines the failures and the factors causing such failures. The failures are identified at each stage of assessment and classified into 11 categories, namely; Information Collection Stage : (1)Lack of information. (2)Lack of concreteness in the collected information. (3)Misinterpretation of the collected information. (4)Mixture of subjective judgment. Information Classification : (5)Mixture of irrelevant information. (6)Oversight of relevant information. Information Analysis : (7)Insufficient analysis. (8)Eagerness to stereotype/ over simplification. (9)Leap of logic. (10)Self-satisfied intention of nursing. (11)Misjudgment. Regarding the factors of failures, there are 6 factors that the logic of assessment bases on, namely ; (1)Approach to the assessment. (2)Communication ability. (3)Observation、(4)Ability to analyze. (5)Knowledge. (6)View of Nursing. and 7 factors that support the students' attitude to engage in the assessment, namely ; (1)Health condition. (2)Motivation for learning. (3)Interest in people. (4)Living condition. (5)Self-perception. (6)Self-efficacy. (7)Cognition tendency. Among all the above factors, Approach to the assessment and Knowledge affect every stage of assessment and cause chain reaction of failures at times. It is considered to be crucial to attend to each individual factor of the students and help them overcome their problems at the early stage of assessment.
