Successful Limb Salvage for a Patient with Acute Aortic Dissection (DBIIIb) and Acute Renel Failure

A 65-year-old male, having previously undergone an abdominal aortic aneurysm repair, was admitted because of a severe low back pain and cyanosis in both legs. His vital signs were stable, but he was anuric. An emergent enhanced CT scan revealed the presence of thoracic aortic dissection, DB IIIb, compromising left renal circulation. Initially, he was conservatively treated, however, since the limb cyanosis extended up to the level of umbilicus, an axillo-bifemoral bypass graft procedure was urgently performed. A continuous hemodia-filtration started immediately after the reconstruction was extremely useful for protection of reperfusion injury. He was uneventfully recovered and discharged in good condition.
