Causes d'échec de la contraception orale et connaissance des femmes quant au maniement de leur pilule contraceptive : enquête menée au CPEF du CHU Antoine Béclère à Clamart dans les Hauts-de-Seine

BACKGROUND. The prescription of oral contraception is too often trivialized in France, neglecting the complexity of its handling. This results in a high number of unwanted pregnancies leading to abortions that could be prevented if women used correctly their contraceptive pills. METHOD. We conducted a study for six months at the planning family center of the Antoine Béclère Hospital among women requesting an abortion after a pill failure and among women consulting for their oral contraceptive monitoring. This study was based on two questionnaires, based on the HAS recommendations. RESULTS. 80 "abortion" questionnaires were collected. They showed that the main causes of contraceptive pill failure were oversight and discontinuation. 123 "contraception" questionnaires were collected. They showed that 77% of women were unable to manage a forgotten pill. 70% of women didn’t know the rule of leading off a new pill pack in case of pill oversight in the last week of the pack. 57% took risks because they didn’t know the proper use of an additional contraceptive method the week following a pill oversight. 51% took risks because they didn’t know how to manage an emergency contraception after a pill oversight. 52% of women didn’t know that the pharmacist could issue the pill even if the prescription was “outdated”. 48% didn’t know that vomiting could cause a contraceptive failure. CONCLUSION. Women ignorance concerning their contraceptive pill handling is alarming. Health professionals have an essential role in the women education when prescribing or renewing oral contraceptive.
