Est-il pertinent de faire des prélèvements bactériologiques périphériques à la naissance chez des nouveau-nés ayant pour seul facteur de risque une DOO comprise entre 12 heures et 18 heures ?

The prevention, the screening and the treatment of the neonatal infections are a major preoccupations of the professionals in maternity. These require a fast and adapted care. However the diagnosis is not still evident, the paediatricians and the midwives in the French maternities give an important place to the peripheral bacteriological specimen when the new-born has a criterion in his history for developing a bacterial infection. That is why, the presented work tries to determine if the duration of the PROM between 12 and 18 o'clock isolated is really a risk factor requiring the present care. A retrospective study on files was led whose results find 3,7% of infected. So the only PROM between 12 and 18 o'clock does not increase in a significant way the infectious risk. On the other hand the association with a maternal vaginal colonization by pathogenic bacterium at the time of delivery increases significantly the infectious risk on the newborn. A program of specific screening was proposed for a better identification of the new-born who are the most at risk to have a bacterial infection and to limit the complementary examinations.
