Hydropic leiomyoma of the uterus presenting as a giant abdominal mass

We describe a case of a 35-year-old woman with apedunculated uterine leiomyoma with diffuse hydropicdegeneration presenting as a giant abdominal mass. Thepatient was admitted in the emergency departmentbecause of diffuse abdominal bloating and discomfort.Ultrasonography (US) showed a heterogeneousabdominopelvic mass. Magnetic resonance imaging(MRI) was performed to further characterise and revealeda myometrial pedunculated tumour. Despite its markedT2-signal heterogeneity and volume, there were no othersuspicious findings to suggest a malignant nature;therefore, fertility-sparing myomectomy was performed.Leiomyomas frequently undergo degenerative changesaltering their imaging appearances. Leiomyomas withuncommon degenerative changes may be difficult todifferentiate from malignant myometrial tumours, basedsolely on imaging.
