摘要

The report identifies best practices and innovative strategies to providing coordinated, non-acute care services for people with HIV infection and AIDS. The project involved site visits to Albuquerque, Santa Fe, Boston, Chicago, New York City, and San Francisco; reviews of service demonstration projects; and the compilation of an annotated bibliography of relevant research. Findings include that (1) comprehensive planning and the location of services should precede the acceptance of clients; (2) the coordination of service delivery and a centralized, comprehensive case management system hold the promise of delivering the most appropriate HIV/AIDS-related services; (3) effective service coordination requires a long-term financial commitment, while dependence on short-term funding can lead to a collapse of the system; and (4) for an effective continuum of care, intermediate- and long-term facilities and housing services must be integrated into the system, ideally utilizing preexisting facilities.
