The need for a comprehensive and accessible system of community services bridgingthe gap between social and health services is vital in a society in which the number of older persons is expected to double in the next fifty years. Five grant proposals under the Aging Network Linkages priority area were funded by the Administration on Aging in September, 1987, including: (1) the Linking Networks Project of Multnomah County Aging Services Division, Protland, Oregon; (2) the Bridge to Health Program of the Southwestern Connecticut Agency on Aging, Bridgeport, Connecticut; (3) the Health Care Partnership of the Rhode Island Department of Elder Affairs, Providence, Rhode Island; (4) the Geriatric Assessment Team Project of the Eastern Shore Rural Health Agency, Onancock, Virginia; and (5) the P.E.E.R. Advocate Program of the CrossRoads of Iowa Area Agency, Des Moines, Iowa. The monograph describes each of the demonstration projects in terms of the problem each program addressed, the organizational structure of the program developed in each locality, evaluation issues addressed by each program, data on clients served, and conclusions reached in each program. Finally, a conclusion section delineates the lessons learned by each program and makes suggestions for organizations wishing to replicate these programs in their own area.
