摘要

The Long Term Care National Resource Center at UCLA/USC (LTCNRC) created and promoted stronger linkages among community service agencies, hospitals and residential long-term-care facilities, which constitute the primary components in the continuum of care. To improve the quality of care for older Americans, especially the frail elderly, there is a critical need to: decrease fragmentation, strengthen linkages and promote interactions among major components of the long-term-care system; increase awareness of and promote more appropriate utilization by providers and consumers of the full range of options available to older persons to remain independent; and identify and fill critical gaps in the continuum of care.
