The document contains the questionnaire and explanatory materials from the BeaverDam Health Outcomes Study (BDHOS). The BDHOS is a longitudinal population based assessment of health status and health-related quality of life in persons aged 47 and older. Between January, 1991 and July, 1992, a total of 1430 persons were questioned using a one-hour structured interview. The protocol assesses health status using the Quality of Well-being Index, the SF-36 from the Medical Outcomes Study, and the method of Time Tradeoffs. An extensive history of Chronic Medical Conditions and Current medications is collected. The first cross-sectional study results are reported in Medical Decision Making, April-June, 1993.
