Improving Community Based Services to Chronically Ill and Disabled Children through State and Local Cooperation

摘要

The goals of the project were to establish effective ongoing decision-making and feedback processes at the state, regional, and community levels which were responsive to the needs of chronically ill and disabled children and their families; to facilitate effective community-based services; and to develop and field test a model communications system which established ongoing partnerships between schools and health care providers.
