Iowa Child Health Systems Development Grant. Healthy Foundations Project. FinalReport and Abstract

The mission of the Iowa Child Health Systems Development project (HealthyFoundations) was to develop a statewide system of assured access to quality primary health care that met the health needs of all children and their families. In order to achieve the mission, two goals were identified in the federal fiscal year 1991 application. The first goal was to develop and implement community models for assuring primary care for children, regardless of the child's or family's entry point into the system. The second goal was to develop the capacity and function for a statewide system of family-centered, community-based primary care services for children in Iowa.
