Clinical Protocol Series for Care Managers in Community Based Long-Term Care.Final Report

摘要

The goals were: (1) to develop clinical protocols for care management practice inresponse to twelve situations commonly encountered with at-risk elders; (2) to ensure input from long term care management experts, practioners, administrators, and policy makers regarding topic selection, protocol development, product design, and dissemination; (3) to ensure that the protocols incorporate practice responses which respect client autonomy, recognize client diversity, and apply to practice in a variety of settings; (4) to develop processes for measuring whether performance was up to these standards; (5) to ensure that the standards and measuring processes fit existing feedback mechanisms which could take corrective actions is necessary; (6) to design and produce materials to effectively disseminate the clinical protocols, training material, and evaluation tool; (7) to disseminate the products to a wide audience of potential users. Each protocol addresses a common client problem and defines the appropriate basic response to the assessor of care manager during each function of care management (i.e. assessment, care planning, care plan implementation, on-going monitoring, and reassessment).
