Fatality Assessment and Control Evaluation (FACE) Report: Maintenance Worker Dies211 After Overhead Crane Collides with Aerial Platform at Construction Equipment 211 Plant, North Carolina, June 13, 1997

A 54-year-old industrial maintenance worker (the victim) was killed when the211u001eaerial platform he was working from tipped and fell 19 feet to the floor of the 211u001econstruction-equipment manufacturing plant. The victim had been grinding the end 211u001eof a shortened runway rail for a floor-operated overhead crane while another 211u001eoverhead crane was operating on an adjacent runway. As the operating crane 211u001etraveled along its runway, it struck the aerial platform, knocking it off balance 211u001eand causing it to tip over to the floor. When the platform and victim hit the 211u001efloor, the victim sustained massive head trauma and internal injuries. Co-workers 211u001enotified the emergency medical service who responded within 3 minutes, and the 211u001evictim was transported to a local hospital where he was pronounced dead.
