Investigation of Casing Blowdown Accident and Fatality: West Cameron Block 331,211 Gulf of Mexico, Off the Lousiana Coast, December 24, 1997

On the morning of December 24, 1997, two Sonat employees arrived on Platform A to211u001eperform various duties, one of which was to blow down the casing pressure on Well 211u001eA-1. While one of the employees was attempting to blow down the pressure, a 211u001etubing extension on the blowdown valve assembly rotated rapidly and fatally 211u001estruck the employee. Subsequent to the rotation and fatal blow, the entire 211u001eassembly broke free from a fitting downstream of an open gate valve, thereby 211u001eopening casing gas flow to the atmosphere. The well was secured by the other 211u001eemployee shortly thereafter by closing the gate valve.
