About 11:54 p.m. eastern daylight time on June 26, 1996, a 36-inch diamter211u001eColonial Pipeline Company pipeline ruptured where a corroded section of the 211u001epipeline crossed the Reedy River at Fork Shoals, South Carolina. The ruptured 211u001epipeline released about 957,600 gallons of fuel oil into the Reedy River and 211u001esurrounding areas. The estimated cost to Colonial for cleanup and settlement with 211u001ethe State of South Carolina exceeded $20.5 million. No one was injured in the 211u001eaccident. The major safety issues addressed in the report are as follows: 211u001eeffectiveness of Colonial's operations management in ensuring that the pipeline 211u001eis operated within safe pressure limits; adequacy of the training given to 211u001econtrollers and shift supervisors as it relates to preparing them to recognize 211u001eand effectively respond to abnormal conditions, emergency situations, and leaks 211u001ein the pipeline; and effects of Colonial controller work schedules on safe 211u001epipeline operation.
