On January 19, 1999, a 43-year-old male career fire investigator (the victim)211u001edied after being crushed by a chimney that collapsed. On January 22, 1999, the 211u001eU.S. Fire Administration notified NIOSH of the incident, and on February 24, 211u001e1999, a Safety and Occupational Health Specialist from NIOSH investigated the 211u001eincident. The incident was reviewed with the Chief, Safety Officer and Fire 211u001eInvestigator from the Fire Department. Photographs of the incident site were 211u001eobtained and the medical examiner's report was requested.
